Available in PDF, EPUB and Kindle. Book excerpt: This e-book will review special features of the cerebral circulation and how they contribute to the physiology of the brain. It describes structural and functional properties of the cerebral circulation that are unique to the brain, an organ with high metabolic demands and the need for tight water and ion homeostasis. Autoregulation is pronounced in the brain, with myogenic, metabolic and neurogenic mechanisms contributing to maintain relatively constant blood flow during both increases and decreases in pressure. In addition, unlike peripheral organs where the majority of vascular resistance resides in small arteries and arterioles, large extracranial and intracranial arteries contribute significantly to vascular resistance in the brain. The prominent role of large arteries in cerebrovascular resistance helps maintain blood flow and protect downstream vessels during changes in perfusion pressure. The cerebral endothelium is also unique in that its barrier properties are in some way more like epithelium than endothelium in the periphery. The cerebral endothelium, known as the blood-brain barrier, has specialized tight junctions that do not allow ions to pass freely and has very low hydraulic conductivity and transcellular transport. This special configuration modifies Starling's forces in the brain microcirculation such that ions retained in the vascular lumen oppose water movement due to hydrostatic pressure. Tight water regulation is necessary in the brain because it has limited capacity for expansion within the skull. Increased intracranial pressure due to vasogenic edema can cause severe neurologic complications and death.

Book excerpt: Every few years a dissertation comes to the area of clinical application of medical technology which carries us forward as on a magic carpet into new regions of understanding and patient care. This book is such a magic carpet. It brings together, in a clear and incisive fashion, important hemodynamic principles with a simple noninvasive method of application to a part of the cerebral vasculature which has been relatively inaccessible. To the lucky and perceptive person who reads this book, a feeling of excitement and hope for progress is engendered. The diligent application of the potentials of transcranial Doppler ultrasound brings new power to our efforts in understanding the cerebral circulation and the causes, treatment and prevention of cerebrovascular disorders. Merrill P. Spencer, M. D. Director Institute of Applied Physiology and Medicine Seattle, Wash. , July 1986 Acknowledgements I am greatly indebted to Prof. He1ge Nornes, Oslo, who introduced me to the fascinating study of cerebral hemodynamics in the early 1970's and since then continually encouraged my interest in this field. It was through his pioneering work on the cerebral circulation-using peroperative electromagnetic flowmetry and Doppler techniques-that the basis was laid for the noninvasive trans cranial approach to the circle of Willis described in this book. I also gratefully acknowledge the stimulating case discussions with Prof. Peter Huber, Berne, at the very early introduction of trans cranial Doppler, the inspiring exchange of ideas with Dr. Merrill P.

Book excerpt: Up to date, the treatment of arteriovenous racemose angiomas of the brain remains unsatisfactory. Intraoperative hemorrhages, post-embolizational or postoperative deficits depending on the site and size of the A VM as well as inoperability of rare angioma types have promoted the technical improvement of diagnostic and therapeutic approaches. Nevertheless, some pathophysiological problems of A VM hemodynamics have not been solved. Many angiographical studies, observations during embolization and operation, dopplersonographical and other perfusion measurements provided some insight. Sufficient animal models have yet to be developed in order to elucidate the pathophysiological mechanisms. This monograph describes A V fistula models in cats and rats, both conventional and newly developed, which allow a better comparison with human cerebral angiomas than previous ones. The most important result is that the model of the breakthrough of arterial pressure waves into the capillaries following a failure of cerebrovascular regulation cannot be confirmed. Rather, according to the findings in precapillary vessels presented here, the regulation functions normally so that a breakdown of regulation cannot be responsible for global brain edema often seen after removal of angiomas. The regulation was demonstrated using different methods, most important of which being the CO response of 2 brain vessels to varying CO contents of the inhaled air. Angiographical, dopplersonographical and 2 perioperative dopplersonographical as well as intraoperative measurements of flow and pressure have been applied.

Available in PDF, EPUB and Kindle. Book excerpt: Cerebral Blood Flow: Quantifying Consciousness Although the heart may be the source of energy needed to generate blood flow, and other organs absolutely essential for normal living, the brain is the reason we are alive. The collected book chapters are aimed at addressing this most fundamental organ and its blood flow. These papers reflect detailed descriptions of similar topics presented over a two-day period as part of the 5th International Symposium on Applied Physiology of the Peripheral Circulation held in Pittsburgh, Pennsylvania, in June 2000. The symposium and this volume were organized into four distinct groupings that follow in a logical fashion. The initial papers focus on the basic unique physiology and pathophysiology of the cerebral circulation, including a historical reviewof the means to measure cerebral blood flow and its implications of the past 30 years. Dr. Walter D. Obrist was one of the original investigators to use radiolabeled markers to assess cerebral blood flow. His equations and initial studies form an impressive introduction to where we are now. As with much of the body's special organs, the brain too has many circulatory features unique to itself. These include the unique blood-brain barrier function ofthe endothelium, local neural regulation control, and intracranial pressure effects. These special issues, plus genetic factors that may predispose individuals to developing cere bral aneurysm, make up the initial section of the monograph.

Chapter 1 is devoted to methodology attempts to explain the special features of a biophysical approach. The biophysical principles of the most commonly used modem methods such as the clearance method and rheoencephalography are described in detail. The second and third chapters examine all the biophysical aspects of the cerebral circulatory system as a functional unit and deal with some principles of its regulation. The results of systems analysis using mathematical modeling are presented in broad outline. Chapter 4 deals with processes related to the regional cerebral circulation. Such investigations focus mainly on the relationship between periodically fluctuating factors affecting limited areas of brain tissue. This text is beneficial to medical students and individuals researching on cerebral circulation.

Book excerpt: The age-standardized mortality rate for hemorrhagic stroke has decreased worldwide over the past two decades, but incidence, number of deaths, and lost disability-adjusted life-years (DALY) continue to increase. Moreover, hemorrhagic stroke occurs half as frequently globally as ischemic stroke, but causes significantly more deaths and lost DALY. Milestone studies of the past decade - STICH, FAST, and INTERACT2 - indicate the importance of hyperacute care for intracerebral hemorrhage; despite this, there is to date no established strategy for patients analogous to reperfusion therapy for ischemic stroke. This book, written by world-renowned experts, covers all current topics related to the diagnosis and management of intracerebral hemorrhage. It will contribute to an improved understanding of current and future aspects concerning optimal management of patients. This is a comprehensive, up-to-date resource for neurologists, neurosurgeons and trainees.
