The first section of the book focuses on oncogenes and chemical effectors that may play key roles in early cell transformation leading to breast cancer. Research discussed includes identification of specific oncogenes which appear to be involved in the disease, study of their activation and expression, examination of the biological effects of various growth factors isolated from breast cancer cell lines, and investigation of the molecular mechanisms by which estrogens promote and stimulate growth of breast cancers. The second group of chapters deals with several other complex factors and phenomena which may influence tumor formation in the breast, for example, expression of abnormalities by fibroblasts, disruption of epithelial-mesenchymal interactions, and loss of ability nili to synthesize normal basal lamina resulting in alterations in the extracellular matrix. Clarification of the processes of normal mammary gland development and differentiation is central to much of this work.

Gains with cytotoxic chemotherapy have been much more modest. Of the approximately 900,000 newly diagnosed cases of cancer each year, 50010 result in death of the patient. The primary cause of these deaths is metastasis. Although the term metastasis was first coined by Recamier in 1829, only in the past ten years have there been intensive scientific investigations into the mechanisms by which tumor cells metastasize. What has emerged is a complex process of host-tumor cell interactions which has been termed the metastatic cascade. Due to the complexity of the metastatic process, the study of metastasis is multifaceted and involves elements of such areas as differentiation, en zymology, genetics, hematology, immunology, membrane biochemistry and molecular biology. The major objectives of this book were to present the most recent advances in our understanding of how tumor cells metastasize to secondary sites by the leading experts in the biology of tumor invasion and metastasis. We hope that this book will lead to new concepts for the treatment of subclinical metastatic cancer. The chapters in this book address both the basic science of metastasis and potential clinical therapies directed toward interruption of the metastatic cascade or toward eradication of subclinical metastases. Many relevant topics have been omitted due to space considerations and thus the topics included reflect the prej udices of the editors.

Book excerpt: The success rate for treatment of primary neoplasms has improved sig nificantly due to improved surgical, radiotherapy, and chemotherapy methods, and by supportive patient care. In contrast, the treatment of cancer metastases, the cause of most cancer deaths, has not been very successful. Approximately 50% or more of patients with primary malignant neoplasms already have established metastases. Consequently, the most important problem in cancer treatment is the destruction or prevention of metastases. Metastases research has obvious clinical importance. Yet it has only been recently that investigators have attempted to study the mechanisms in volved in this process. This is in part due to the complexity of metastases formation. A metastatic colony is the result of a complicated series of steps involving mUltiple tumor host interactions. It is expected that multiple biochemical factors and gene products derived both from the host and the tumor cell may be required for the metastasizing tumor cell to invade, survive host defenses, travel in the circulation, arrest and adhere in the target organ, invade out, and grow as a metastatic colony. Some of these factors have recently been identified by investigators who have focused on individual steps in the metastatic process and have employed new technologies in immunology, biochemistry and molecular biology. The purpose of this volume is to capture some of the excitement in the field of metastases based on such new discoveries.

Book excerpt: The last 30 years have seen little improvement in the age-adjusted mortality rates for most common types of cancer, and until we develop more effective and less damaging treatment modalities for these tumours, selection of each patient's treatment must depend on prognostic pointers. These lead to a calculated trade off between our estimate of likely benefit to the patient, as against cost in terms of quality of life. But changes have occurred recently in our understanding of the traditional prognostic pointers used for selecting such individualised treatment. First, it is increasingly recognised that the stage at which a tumour presents is more related to the chromo logical age of the tumour (how far it has progressed before diagnosis) than to its biological characteristics. While advanced chronological age of the tumour may predict a greater likelihood of early death, only biological criteria can predict the tumour growth rate, the likelihood of prolonged survival, the likely course of the disease after the first recurrence or the likehood of response to systemic therapy. Second, there is increasing use of failure analysis in relating the clinical and biological characteristics of tumours to their response to standard treatments. In the past, the relationship was interpreted mainly in terms of survival rate, but the site and timing of first recurrence and the pattern and timing of subsequent spread provide a better assessment of the control possible from local or systemic therapy.

The mechanism of growth and response of endocrine sensitive tumors and current concepts of the statistics and cost-benefit assessment of adjuvant chemotherapy trials comprise two other sections of the book where original and stimulating ideas were presented.

Book excerpt: Breast cancer, being a disease with very high prevalence in the female population, has permanently attracted the attention of biologists, biochemists, pathologists, surgeons, and oncologists. In the U.S. alone approximately 120,000 new cases are diagnosed and 37,000 deaths a year are recorded as an outcome. Early diagnosis, clear cut prognosis and the hope of new therapeutic approaches has spurred the enthusiasm of researchers, who with the advent of newer methodologies, tried to employ it in different approaches to the breast cancer patient. In spite of the efforts devoted towards the creation of new approaches, reagents and procedures taking advantage of special characteristics of the breast tissue were slow to come. In general, most diagnostic and prognostic techniques were those shared and applied to other carcinomas. In the early seventies we discovered the first sets of antigens associated with breast epithelial tissue both in the mouse and the human. The recognition of such group of molecules, present in the milk fat globule membrane, gave impetus to several studies in these areas. These mammary cell surface antigens proved useful in several diagnostic approaches such as radioimaging, histochemical and serum assay procedures. However, the apparently inherent limitations of polyclonal antibodies reduced the impact of these novel approaches.

Since the first meeting, the field of neuro-oncology has made remarkable progress in understanding both basic and clinical factors of significance to patients with brain tumor. While the earlier meeting dealt to a large extent with clinically oriented studies, this symposium was more heavily weighted toward the biology of brain tumour and improving our understanding at the physiologic, biochemical, pharmacologic, and cellular level. Book excerpt: The development of monoclonal antibodies to human tumor associated antigens has greatly facilitated the application of immunohistochemical techniques to analyze surgically removed tissues. During the last few years this approach has been utilized by a progressively increasing number of investigators to analyze malignant cells. Although monoclonal antibodies to tumor associated antigens have not become yet routine reagents in immunopathology, they have provided new information which could not be obtained with conventional antisera or histochemical procedures. The following are representative examples. TUmor associated antigens have been identified which display a restricted distribution in normal tissues and therefore may represent useful markers for radio imaging and appropriate targets for immunotherapy. In spite of undetec table differences with conventional histopathological approaches hetero geneity has been found in the antigenic profile of tumor cells within a lesion, in autologous lesions removed from different anatomic sites from a given patient and in lesions removed from different patients. Phenotypes of tumor cells have been identified which correlate with the biology of tumor cells and with the clinical course of the disease. From a practical view point the use of monoclonal antibodies in immunopathology has enhanced interactions between pathologists and immunologists, as exemplified by the present book. Such interactions have contributed to the application of basic research to clinical problems. The chapter of this book discuss investigations performed with monoclonal antibodies to antigens expressed by various types of normal and malignant human cells.

Book excerpt: Biochemical Modulation at the present time defines an area of study in which the intracellular metabolism of a given anti cancer agent is modulated (usually by a noncytotoxic agent or a cytotoxic agent at sufficiently low dosage to make it non cytotoxic) in order to either increase the effectiveness of the particular agent against tumor cells or decrease its cytotox icity against normal cells. The major focus of modulation has been the agents 5-fluorouracil (FUra), arabinofuranosylcytosine (ara-C), methotrexate (MTX) and a few alkylating agents. The major thrust of the studies has been to increase the flow of the anticancer agent along the pathway responsible for the formation of the cytotoxic species: for example, FUra to FUTP or ara-C to ara-CTP. While in most cases the application of research re sults to clinical trials does not require the subsequent exper tise of the laboratory researchers, application of biochemical modulatory schemes to clinical protocols necessitate a dramatic break with the past procedures. As shown in the laboratory clinical loop below, close collaboration between the laboratory and clinical investigator is essential. While the laboratory REDEFINE TECHNOLOGY, TESTS OR QUESTIONS FOR FURTHER THERAPEUTIC ADVANCE CLINICAL EXPERIMENTAL PROTOCOL (LABORATORY) RESEARCH STUDIES DEFINE AND TEST APPROPRIATE SCIENTIFIC PARAMETERS results define rationally-based regimens, it is essential that the clinical protocols contain the requirement that clinical material (either tumor or normal tissues) be sampled to deter mine whether the biochemical modulation being proposed is in fact beinq accomplished.
