All patients completed the 12 months follow-up recall visit. At the 12 months of follow-up visit two implants (one internal and one external hexagon implants) failed, resulting in an implant CSR of 97.6%. The prosthetic CSR was 100%. Mean periimplant bone resorption at 12 months of healing was 1.5 mm (SD 1.00) (-1.55mm external and -1.59 mm internal hexagon).At 12 months of follow-up, the mean values of PI, BOP and PD were 2, 0.8 and 2.24 mm respectively for external hexagon implants and 2, 1 and 2.1 mm respectively for internal hexagon implants. 3 screw loosed abutments were identified in the external and 2 in the internal connection implants. The statistical analysis did not reveal differences in bone resorption between external hexagon implants and internal hexagon connections nor for periimplant health parameters (PI, BOP and PD).

Book excerpt: Passive fitness of 50 full arch implant rehabilitations with fully digital work flowBackgroundUp to date, in spite of the numerous procedures described for the registration of the exact three-dimensional position of dental implants in a multiple rehabilitation, it remains to be a challenge for the clinician to stumble upon the most accurate technique used for the impression of multiple rehabilitations in order to guarantee the necessary passive fitness of the implant-supported prosthesis, ensuring in this way the treatment success and the long-term prognosis of the therapy.Although it is not scientifically clear whether osseointegration could be negatively affected by a misfit of the framework, mechanical complications of the prosthesis can be prevented by a correct passive fitness of the structure. To reach a correct adaptation between both sides, implants and prosthesis, a highly accurate impression is mandatory.Since the appearance on the dentistry, the computer-aided design/ computer-aided manufacturing (CAD/CAM) procedures, different methods have been developed to reach precise digital impressions of dental implants. The first one is an indirect method in which the conventional impression is scanned by the laboratory and right after digitalized. On the other hand, the other method is based on a direct impression performed by intraoral scanning with digital photo or video technology to capture images. this procedure is brung about directly by the clinician.Photogrammetry is a novel and reliable choice of obtaining this passive fitness demanded in full arch impact rehabilitations due to the fact that it allows the clinician to register the exact position of the implants by the use of intraoral devices attached to implants and a stereocamera that records implants positions using photogrammetry.AIMThe aim of the present paper is describing the photogrammetry as a reliable technique for registering the exact position of the implants, using 50 full arch implant rehabilitations that demonstrate that a passive fitness can be reach with this novel method ensuring the long-term success of the rehabilitation. Materials an Methods The photogrammetry system used for the impression of the implants consisted in three main devices:u2022 A CAD software (PIC pro, Position Implants Correctly [PIC] Dental, Madrid)u2022 An extra oral stereo camera with an infrared flash (PIC Camera, PIC Dental). Is a3D camera for intra-oral implants impression that captures the interrelated exact position of the patient implants in an open file. Is a high-precision Stereo camera capable of capturing implant positions and angulations interrelated to obtain an open STL file containing every geometry needed to design the final structure.u2022 Black flag-shaped abutments each with four white spots that are positioned differently in each abutment for unique identification purposes (PIC Abutment, PIC Dental). This photogrammetry device takes 64 pictures per second, with an error margin of less than 10 u03bcm in less than 20 seconds. It identifies the spatial position of each implant without making physical contact.The PIC camera is a 3D camera for intra-oral implants impression that captures the interrelated exact position of the patient implants in an open file. Is a high-precision Stereo camera capable of capturing implant positions and angulations interrelated to obtain an open STL file containing every geometry needed to design the final structure.PIC camera is the only device that has effectively proven to capture patient implant positions regardless the number of implants per arch, their position, distances or angulations. PIC camera captures a bigger area to ensure that all implants are scanned at a time, therefore the camera is not introduced inside the patient u0301s mouth making the process more comfortable.The clinician screwed the PIC Abutments, according to each case, into the implants connections and placed the PIC Camera about 20 centimeters away from the patientu2019s mouth.This photogrammetry device takes 64 pictures per second, with an error margin of less than 10 u03bcm in less than 20 seconds. This device identifies the exact spatial position of each implant without making physical contact with them. The final information then is stored in an STL file, called a PIC fileResults A total of 345 dental implants used in 50 patients with full arch implant rehabilitations, 21 men, 29 women, were included in this study.In the moment of loading, all of the prosthesis presented optimal fit and there were no misfitting problems. 24 months after loading, peri-implant tissues were healthy and no marginal bone loss was observed. All of the structures presented a correct fitness and there were no structural complications. ConclusionThis intraoral photogrammetry system to register implant 3-D precise positions and to create a CAD/CAM framework that guarantee an accurate passive fit and minimize the apparition of post-treatment complications. Apart from that, not only is a procedure that can be done fast but also considered to improve patientu2019s comfort and make it easier for the clinicians in comparison with other conventional or digital impression methods. However, the method still requires a second STL file to provide the information related to soft tissue and the use of special equipment that needs an adaptation process for the clinician.

ICC abutments lacking structures for vertical stop might sink axially into the inner surface of the implant. Axial displacement is caused by factors u2014 machining tolerance and the settling and wedge effects. Machining tolerance and the settling effect are inevitable phenomena in both external and ICC implants. However, the wedge effect, which induces concentrated axial compressive force in the direction of abutment sinking, is observed only in ICC implants. Although axial displacement in screw-type prostheses has been reported previously, no information is available regarding axial displacement in cement-retained prostheses. Differences in implantu2013abutment connection type have been known to result in variations in axial displacement in various clinical situations. Three typical implantu2013abutment connectionsu2014the external-butt (EXT), internal-conical (INT), and internal-butt (INT-Butt) connectionsu2014have been known to provide satisfactory results in clinical practice.Aim/Hypothesis:The purpose of this study was to quantitatively evaluate axial displacement in three different implantu2013abutment connections in cement-retained monolithic zirconia and polymethylmethacrylate (PMMA) provisional prostheses with the computer aided designu2013computer aided manufacturing (CAD-CAM) custom abutments.Materials and Methods: Three types of implants with different connectionsu2014external and internal butt connection implants and an internal conical connection implantu2014were evaluated. In each implant group, implant replicas were embedded vertically and angulated at 15u00b0. Sixty titanium custom abutments were fabricated using a CAD-CAM system. The geometries and surface morphologies of custom and ready-madeabutments were comparatively evaluated by scanning electron microscopy. Micro-gaps in abutment connections were scanned byoptical microscopy during screw tightening, re-tightening, and cyclic loading. Zirconia and provisional acrylic prostheses were fabricated and evaluated for internal gap by an indirect silicone technique. Cemented prostheses on abutments were mounted on a universal testing machine and subjected to 250 N sine wave cyclic loads. Cumulative axial displacement was measured at loadingperiods of 3, 10, 100, and 1,000,000 and analyzed by repeated measures analysis of variance (ANOVA). Internal gap width and netchange in axial displacement were analyzed by multi-way ANOVA.Results: There were no significant differences in internal gap widths of prostheses according to implant or prosthesis type. However,measurement site and implant angulation were influencing factors on internal gap. Surface geometries and morphologies of custom abutments varied according to the implantu2013abutment connection. Machining tolerance control and precision fit were difficult to achieve with the custom CAD-CAM abutment. The internal and external conical connection implants exhibited the greatest and lowest axial displacements, respectively. Acrylic prostheses exhibited greater axial displacement than zirconia prostheses. The vertically positioned implant group exhibited greater axial displacement than the angulated implant group in the early cyclic loading periods. Axial displacement in the late cyclic loading period was relatively low.Conclusions and Clinical implications:Custom abutments for cement-retained prostheses exhibited greater axial displacement than screw-retained prostheses. Axial displacement mostly occurred in the early loading period and was self-limited. Axial displacement should be managed by provisional restoration, with consideration of implant connection type and angulation, abutment fabrication method, and occlusal force of the patient.
