Book excerpt: Allergic asthma is a prevalent, chronic respiratory disorder marked by inflammation, elevated inflammatory mediators (e.g., interleukin-5 [IL-5] family cytokines), and increased eosinophil (EOS) numbers. EOS recruitment and inflammatory response contributes to asthma-associated airway damage. Given EOS dependence on IL-5 cytokines for growth, development, and inflammatory behavior, improved treatments for eosinophil-related diseases require further elucidating IL-5 family-induced intracellular signaling. Relative to peripheral blood eosinophils (EOSPB), IL-5 family-induced survival is abated in airway eosinophils (EOSA) though EOSA remain responsive to IL-5 cytokines for certain inflammatory endpoints. Since EOS survival requires intact JAK/STAT (Janus kinase/ Signal Transducer and Activator of Transcription) signaling, I tested the first of two hypotheses: EOSA exhibit a JAK/STAT-specific inhibitory mechanism. Previous findings in inflammatory cells indicate cholesterol rich membrane microdomains (CRMMs) are crucial for selected signaling pathways, especially those dependent upon membrane-associated adaptor molecules (e.g., mitogen activated protein kinase [MAPK] pathways). Therefore, I tested the second of two hypotheses: reducing membrane cholesterol in EOSPB will selectively down-regulate MAPK signaling but not JAK/STAT signaling, which is not dependent on membrane-associated adaptor molecules. Immunoblotting revealed EOSA exhibit attenuated IL-5 family-induced phosphorylated STAT3 and STAT5 (pSTAT3 and pSTAT5, respectively) relative to blood EOS from the same airway allergen-challenged donors (EOSCPB). In EOSPB (unchallenged donors) IL-5 cytokines induce members of the JAK/STAT-targeting suppressors of cytokine signaling (SOCS), CISH and SOCS1. Furthermore, EOSA express more CISH and SOCS1 mRNA (measured by qPCR) and CISH protein than EOSCPB and EOSPB. Furthermore, reducing EOSPB membrane cholesterol (using cholesterol chelating methyl-&beta-cyclodextrin [M&betaCD]) attenuated IL-5-stimulated phosphorylated p38 (p-p38) and extracellular-regulated kinase 1/2 (pERK1/2) and attenuated IL-5-induced interleukin-1&beta (IL-1&beta) mRNA. Elevating EOSPB membrane cholesterol with soluble cholesterol (M&betaCD pre-loaded with cholesterol) elevated basal p-p38, but did not alter IL-5-stimulated pERK1/2, pSTAT5, or pSTAT3. Furthermore, soluble cholesterol pretreatment attenuated IL-5-induced 48-hour survival measured by metabolic activity assay. In summary, these data support a model wherein IL-5 family cytokines trigger a selective JAK/STAT-inhibitory mechanism in EOSA. Additionally, EOSPB membrane cholesterol alters IL-5 induced MAPK signaling and inflammatory endpoints in (a) to-be-determined mechanism(s). These data expand understanding of EOS IL-5-responsiveness and could lead to improved therapeutics for EOS-related allergic diseases.

There are currently 23 different interleukins (numbered from IL-1 to IL-23), although the full extent of the interleukin family will only become clear upon analysis of the human genome sequence. Most important, interleukins are central to the pathogenesis of a wide range of diseases that involve an immune com- nent, including such conditions as rheumatoid arthritis, multiple sclerosis, ulcerative colitis, psoriasis, and asthma. Interleukins have also been imp- cated in other conditions, including cancer, migraine, myocardial infarction, and depression. In essence, when cells are activated by interleukins, a program of gene expression is initiated in the target cell that alters the cell’s phenotype, leading to enhanced immune reactivity, inflammation, and/or proliferation. Interleukins are therefore at the core of the cellular basis for many diseases. They are the subject of intense investigation by biomedical researchers and the targeting or use of interleukins in the clinic is proceeding apace. Approaches such as t- geting IL-4 in asthma or IL-1 in joint disease are being pursued, and it is likely that in the next 5–10 years a number of new therapies based on either inhib- ing or administering interleukins will be available.

Hence, a polymorphism in IL-5 receptor may be implicated in the development of asthma. Many candidate genes that could potentially contribute to the susceptibility to the disease have not been investigated to date, and not all of the polymorphisms of the candidate genes have been tested for a possible association with the disease. Taking this into consideration, IL-5 (together with the IL-5 receptor) polymorphism deserves attention as the subject of further investigations into asthma. In this review, we will address the role of IL-5 and IL-5 receptor polymorphism in asthma, describe the impact of these polymorphisms on the Blood parameters and clinical parameters. Further, give an overview of preclinical and clinical studies targeting the IL-5 and IL-5 receptor pathway.
