Book excerpt: Le trouble bipolaire est une pathologie fréquente touchant entre 1 et 3 % de la population mondiale. Ce trouble est à l’origine d’un retentissement important sur la sphère personnelle, familiale et socio-professionnelle avec un risque suicidaire accru. Le dépistage de la dépression bipolaire est difficile en raison de la multiplicité des tableaux cliniques et des limites des classifications actuelles. Pourtant, ce dépistage permet d'améliorer notablement la prise en charge du trouble. Notre revue de littérature est réalisée à partir d’une recherche MEDLINE sur une période allant de 1981 à Juin 2009 en utilisant les termes principaux : « Bipolar disorder », « Depression », « Depressive disorder », et « Primary health care ». Les résultats de notre travail ont permis de distinguer des facteurs prédictifs de bipolarité chez des patients déprimés. Afin de systématiser son entretien clinique, le médecin généraliste peut s'appuyer sur quatre questionnaires standardisés : le SAD-P, le MDQ, la CLH-32 et la MAThyS. Cependant, ils présentent plusieurs limites, notamment un aspect chronophage en médecine générale, ainsi qu'une spécificité moyenne pour les troubles bipolaires de type II. Chez un patient déprimé, le diagnostic de bipolarité ne pourra donc pas être retenu par un nombre restreint d’indices, il repose sur un faisceau d’arguments au terme d’un entretien clinique systématisé. Le dépistage d'un tel trouble chez un patient déprimé représente un enjeu considérable en ce qui concerne la prise en charge thérapeutique. D'après les conférences de consensus, les thymorégulateurs sont le traitement de première ligne. Si le praticien décide de prescrire un antidépresseur, une période courte est recommandée, sous couvert d'un thymorégulateur. L'amélioration de la prise en charge de la dépression bipolaire passe donc par une optimisation de la formation des professionnels de santé et par une meilleure collaboration entre les médecins généralistes et les psychiatres. Dans cette optique, le développement des réseaux de soins et des centres référents peut se révéler une aide précieuse pour le médecin généraliste.

Afin d'encourager les médecins à s'exprimer, la méthode d'investigation la plus adéquate est qualitative : elle consiste à mener un groupe de discussion, avec l'aide d'un modérateur. L'analyse de la discussion montre que leur priorité n'est pas tant de poser le diagnostic de bipolarité, mais de minimiser le risque suicidaire. Ils montrent une réelle réticence à suivre un questionnaire-type, long et formel, face à un patient dépressif. Le questionnaire élaboré au cours de la réunion est donc court, les questions faciles à poser lors d'une première consultation. Cet outil peut être utilisé en première intention, il semble plus appropriée aux yeux des médecins participants et contribuerait à réduire les délais de diagnostic.

Notre objectif principal était d'évaluer la faisabilité et l'intérêt de l'utilisation des QW en médecine générale en postpartum. Nous avons également voulu promouvoir le dépistage des troubles dépressifs en post-partum par les médecins généralistes. Matériel et méthode : notre travail est une enquête transversale descriptive en deux temps. Deux questionnaires ont été distribués aux médecins généralistes Bas-Normands, à deux mois d'intervalle durant laquelle ils étaient invités à utiliser les QW en consultation. Résultats : la majorité des médecins sous-estiment la prévalence des dépressions du postpartum. Après avoir intégré les QW dans leur pratique, la majorité des médecins les recommande pour une utilisation systématique en médecine générale (63,33%) et compte les réutiliser (66,66%). Sur une échelle sur quatre, les retours sont positifs concernant l'acceptabilité (3,22 ± 0,97), le temps nécessaire (3,27 ± 0,93) et leur facilité (3,33 ± 0,86), mais plutôt neutres concernant l'utilité (2,90 ± 0,97) et le nombre de consultations supplémentaires engendrées (2,38 ± 1,02). Pendant l'étude, huit dépressions du post-partum ont été diagnostiquées en utilisant les QW (11,11%). Conclusion : les retours des médecins concernant les QW ont été majoritairement positifs. Les QW, facilement intégrables dans une discussion avec la jeune mère, peuvent contribuer au dépistage des dépressions. Notre étude a permis en même temps de sensibiliser les médecins concernant la dépression du post-partum.

Seulement 15 à 50% de ces personnes dépressives seraient diagnostiquées. Face au vieillissement de la population et donc à l'augmentation du nombre de patients déments à l'avenir, il serait intéressant d'améliorer le dépistage de la dépression. L'objectif principal de cette étude est de montrer l'intérêt d'utiliser un test de dépistage de la dépression chez les patients déments par un outil validé. METHODES : Nous avons mené une étude comparative, longitudinale, mono-centrique, avec un recrutement dans un CEG. Nous avons comparé les observations des gériatres n'utilisant pas de test validé, avec le résultat de l'échelle de Cornell, considérée comme le Gold-Standard. Cette échelle, a été remplie en interrogeant l'aidant principal, par contact téléphonique. Elle est validée pour le dépistage de la dépression chez les personnes démentes. Elle comprend 19 questions, elles-mêmes regroupées en 5 sous parties. RESULTATS : Cinquante-deux patients ont été inclus entre le premier juillet et le dix décembre 2014. Une plus grande sensibilité a été retrouvée par l'échelle de Cornell : 60% des patients étaient diagnostiqués comme dépressifs contre 23% par la clinique. Il n'apparaissait pas de profil dépressif particulier dans la population diagnostiquée comme dépressive par la Cornell, entre ceux diagnostiqués par le clinicien, et ceux qui ne l'étaient pas. CONCLUSION : Cette étude montre l'importance d'utiliser un outil validé pour dépister le syndrome dépressif dans une population démente. L'échelle de Cornell, pourrait être une aide précieuse pour améliorer ce dépistage en médecine générale.
