The reported inpatient fall result in injury up to 33% out of that 4-6% is serious leading to impaired rehabilitation, co-morbidity and even death. Falls increases length of stay in the hospitals and health care costs (Choi, Lawler, Boenecke, Ponatoski, and Zimring, 2011). The objectives of this study is to reduce falls at Sunrise Hospital's Acute Stroke and Epilepsy Care Unit (ASECU), which has the highest rate of falls compared to all other units. The aim of the project is to create more awareness among nurses and other care givers about fall risk assessment, fall prevention strategies and modify the caring environment with learned knowledge and skills of the staff. An educational manual development and education program for nurses about the importance of accurate fall risk assessment and multifaceted intervention strategies from evidence can reduce falls at ASECU. Implementation of multifaceted interventions can increase the safety and quality of care associated with decreased health care costs. Spot checks and chart audits daily for two weeks, then weekly on all high risk for fall patients can be done to evaluate the compliance. The success of the program is significant reduction in fall rate and that is evaluated every month, every three month, every six month and then yearly following the implementation. The results are disseminated to nurses and other health care providers inside the hospital through staff meetings and e-letter monthly. To the community it is accessible through magazine H2U, flyers, and brochures. The video-on-demand and website are further tools for dissemination to the stakeholders. Falls can be reduced or prevented with proper and accurate assessments, intervention strategies and following the guidelines at ASECU, at Sunrise Hospital.

However, falls continue to be a major health concern for older adults in nursing homes. Purpose: The purpose of this evidence-based quality improvement project was to impact the knowledge and skills of direct care workers regarding falls in older adults residing in Holt Senior Care and Rehabilitation Center in Holt, Michigan. The main goal of the project was to provide education to direct care workers and to reduce falls and fall-related injuries of the older adult population at the Holt facility. Methods: A pre- and post-test design was distributed to direct care workers to assess their knowledge of fall prevention before and after an in-person educational PowerPoint presentation. Educational handouts were also provided to all direct care workers. Scores were analyzed after the education session to determine whether any changes occurred. Implications/Conclusion: Primary outcomes were that direct care workers demonstrated improvement in post-knowledge and attitudes scores post-intervention. Secondary outcomes were that Minimal Data Set statistics on falls improved in subsequent quarters of survey assessment at the facility.

Fall rates were analyzed for a five month period after implementation of the new scale, from May 1, 2016 – September 30, 2016, and compared to the same months the previous year to see if fall rates decrease with using the evidenced-based scale. Fall prevention is multi-factorial and begins with an assessment of the patients risk for a fall, and interventions which are personalized for each patient. Fall prevention should include all staff, as anyone can make a difference in preventing falls. In this study, fall rates did start to slightly decrease in the months following implementation of the new scale. Further research is needed to see if the fall rates continued to decline long term.

An improvement project utilizing the model for improvement and Plan-Do-Study-Act (PDSA) cycles was conducted on a 40-bed unit that provides care to cardiac, and medical-surgical patients. The current fall prevention quality council team is composed of nurses and interdisciplinary clinicians, who focus on improving clinical and financial outcomes related to fall prevention and potential injuries. A Clinical Nurse Leader (CNL) was asked to lead a practice improvement project in response to chart audits and observational rounding that indicated a gap in patient awareness of fall prevention activities and lack of consistent implementation of established fall prevention measures by nursing staff. The aim of this project was to increase awareness and knowledge of patients related to fall prevention and staff education to improve reliability of implementation actions to reduce falls. Data was collected over a three-month period and new interventions including a fall prevention tool kit and communication campaign - "Call Before You Fall" were developed and implemented to reduce fall rates by 50% every month. Results from staff education are expected to improve based on pre and post data comparison of education provided during shift huddles. Data results post intervention proved to decrease the rate of falls. Clearly, fall prevention activities need to address both communication and education interventions to improve fall reduction outcomes.

Fall-related injuries increase the healthcare expense related to hospitalizations and home healthcare services. Since the home health agencies must provide safe operational spaces for patients, they initiate interventions to improve patients' fall outcomes. This study obtained data from a home health agency based in Tracy, California, regarding fall risk among elderly people over one month. The data were obtained from the fall risk assessment completed upon admission of the patient to the home health agency and the data collection were repeated after completion of the program, to determine if the interventions to reduce falls were effective. The data analyzed 194 participants in community dwellings through a paired sample t-test. The results indicated that both men and women were identically vulnerable to falls based on pre-fall prevention program implementation data. This study discusses the components of the fall risk assessment tool used for the findings to determine the competency of the fall prevention program in reducing the risk of falls for the total sample size. In the pre-fall prevention program intervention period, the risk of falls was elevated, and it reduced significantly after the intervention period for the total sample size. The overall recommendation is that interventions to reduce falls are effective and should thus be applied at home health agencies.

Abstract: Background: Falls are one of the most common and serious problems facing the elderly and are known to be associated with significant mortality, morbidity, decreased functioning and premature institutionalization. In Hong Kong, the prevalence of falls among community-dwelling older adults is 19.3%. Moreover, the incidence of falls among older people in institutions is almost three times the fall rates for the community-dwelling elderly. Institutional falls are regarded as common adverse events in hospitalized older patients. Significant mortality, morbidity and healthcare costs associated with institutional falls led institutions to recognize falls as a high-priority safety risk for hospitalized patients. This demonstrated the significance of providing the health care providers with an evidenced-based practice guideline of an effective multifactorial fall prevention programme in order to prevent in-patient falls. Objectives: The objectives of the study are to systematically review and present the best evidence for the effectiveness of multifactorial fall prevention interventions in reducing falls in hospitals, to translate the reviewed evidence and to develop evidence-based practice guidelines for the multifactorial fall prevention programme as well as to develop a plan for implementing and evaluating the multifactorial fall prevention programme. Methods: The relevant literature was searched by several electronic databases. The related literature was then retrieved, reviewed and synthesized. The quality assessment of the studies was performed according to the methodological checklist for controlled trials designed by the Scottish intercollegiate Guideline Network (SIGN). Evidenced-based practice guidelines for the multifactorial fall prevention programme were then synthesized according to the findings of the reviewed literature, while the implementation potential being assessed in terms of transferability, feasibility and the cost-benefit ratio. Results: Five studies were identified according to the inclusion and exclusion criteria set. "Evidence-based guidelines of fall prevention programme for hospitalized older patients" were formulated based on the review of the selected studies. Fourteen recommendations of the evidence-based guidelines are formulated and graded according to the grading system of Scottish Intercollegiate Guidelines Network (SIGN). The evidence-based recommendations can offer nurses and other health care professionals the standards and strategies required for implementing multifactorial fall risk assessment and multifactorial fall prevention interventions, including environmental modifications, knowledge, medication reviews and exercise. A communication plan for various parties in hospitals including a pilot test for determining the feasibility of the innovation and an evaluation plan to determine the effectiveness of the fall prevention programme were subsequently developed. Conclusion: This study reviewed evidence for the effectiveness of the multifactorial fall prevention programme in reducing the incidence of falls, translated the reviewed evidence and developed evidence-based guidelines for a multifactorial fall prevention programme, which can provide the health care practitioners with an evidence-based approach in fall risk assessment and management so as to prevent in-patient falls. Fall fatality rate for people age 65 and older in the United States is 36.8 per 100,000 people, whereas in Canada, mortality rate for the same age group is 9.4 per 100,000. Mortality rate for people age 50 and older in Finland is 55.44 for men and 43.1 for women per 100,000. Foreground: Falls are a widespread health problem posing serious threats to patient safety. Accidental falls account for the most incidents reported in hospitals, complicating longer stays and burdening the healthcare system. Substatial costs are associated with falls, including costs of patient care associated with increased length of stay and liability. Fall-related injuries in older adults often reduce mobility, independence, and result in increased risk of premature death. EBP Framework: The Larrabee Model for evidence-based practice change and Roger's Diffusion of Innovation served as frameworks for the project. Methods: the process included an educational intervention, recruitment of subjects, and management of data. The EBP Beliefs Scale including a pre- and post- knowledge test was the source of data, collected through various means. Records were kept of the group of nurses who attended each educational session. The impact of the educational intervention and the Knowledge, skills, and confidence of the nurses were measured, allowing the investigator to compare the results before and after the intervention to decipher if a change has occurred. Descriptive statistics and frequencies were utilized to analyze the data obtained from the pre- and post-test forms. Findings/Results: The findings show a positive change in nurses' perceptions levels in attitudes, behaviors, and knowledge of Hendrich II Falls Risk Assessment Tool (HFRM) as evidenced by an improvement in pretest to posttest EBP Beliefs Scale survey scores. All categories show increase in nurses' beliefs. Conclusion/Recommendations: The design of the project was quantitative; the descriptive analysis show positive change in EBP Beliefs scale scores of nurses' perception of Hendrich II Fall Risk assessment Tool. In keeping with the Larrabee Model, Roger's diffusion of innovations theory and PDSA Model, HFRM education intervention should continue and become part of the facility's Fall Policy as more cycles are completed. A mandated full educational module presented by Staff Development should be instituted for all staff and disciplines involved in the educational intervention, implemented with staff competency evaluation.

Book excerpt: An in-hospital fall is a devastating event for patients and care providers resulting in injuries, physiological and psychological patient declines, and sometimes deaths. Furthermore, fall related costs greatly increase hospitals0́9 expenses and cause massive distress to caregivers and health providers. Many patient-specific, organizational, and seemingly erratic factors contribute to the occurrence of in-hospital falls. Despite this, hospitals must undertake consistent evidence-based measures to prevent fall occurrences as much as possible. The traditional nursing approach to falls prevention is not sufficient to control fall rates because the issue is too complex and must be approached from multiple perspectives rather than just nursing. Therefore, innovative practices and approaches were explored to provide additional fall preventive measures. International and local U.S. practices emphasize the importance of an interdisciplinary approach to falls prevention. This model includes multiple clinicians with diverse backgrounds, such as physicians, physical therapists, and pharmacists, who assess patient fall risks together and implement measures to effectively decrease the risks. Limited guidelines and lack of conclusive research about the interdisciplinary approach lead the team of Master of Science in Nursing Clinical Nurse Leader students to investigate the roles of multidisciplinary team members for falls prevention in great detail. As a result, a blueprint of team role descriptors was created and offered for implementation in the medical-surgical elderly client population unit. This project provides innovative guidelines for falls prevention. These guidelines have the potential to decrease patient falls by 20-30 % in the hospital. This paper is particularly focused on the physician0́9s role in fall prevention. In addition, this paper represents a modified compilation of current innovative hospital practices and evidence-based research findings.
