The intensive care unit (ICU) is a particularly challenging environment to provide excellent end of life care. The aim of this study was to investigate intensive care nurse perceptions of the quality of dying and death of patients who died receiving end of life care and their perspectives as to whether this care met the needs of the patient's families. An ethically-approved descriptive study was conducted, involving 35 nurses reporting via online survey on the most recent patient that they cared for in the ICU, and who subsequently died receiving end of life care. Overall, statistical analysis of quantitative survey items revealed that quality of dying and death was perceived by nurses as good, whereas satisfaction with meeting family needs ranged from fair to good. Following an inductive analysis of the open-ended survey items, six key categories of meeting family needs emerged: quality of care, emotional support, communicating and providing information, spending time with loved one, physical and environmental comfort, and spiritual care. The quantitative and qualitative results of the current study also identified several areas where nurses perceive quality of dying and death and meeting family needs intersect: symptom control, presence of family, dignity, and spiritual care. The development and testing of tools that measure the quality of end of life care in an Australian ICU setting is an important priority for future research.

Book excerpt: Purpose/Aims: This pilot study explored nurse family member perceptions of nursing care and patient quality of death and dying during and after withdrawal of life-sustaining treatment within Intensive Care Units (ICU). Rationale: Past research that examined physician, nurse, and family member experiences of End-of-Life (EOL) care within the ICU found significant differences between the three groups. Background: Approximately 500,000 people die within an ICU each year in the United States. End-of-Life and Palliative Care programs focused primarily on physician interventions to improve quality of EOL care have had little impact on family member perceptions of quality of care or patient death, with nurses rating quality lower than family or physicians. Methods: This descriptive study examined the perceptions of nursing care and patient quality of death and dying using the family member version of the Quality of Death and Dying (QODD) instrument and Family Satisfaction in the ICU (FS-ICU). Qualitative data related to the nurse family member experience was elicited through additional open-ended questions. Findings: Respondents (N=17) tended to view the quality of patient death somewhat favorably with a total QODD mean score of 60.75 (SD = 21.05) on a scale of 1-100. Results also indicated a moderate level of satisfaction with care within the ICU, with a total FS-ICU mean score of 67.75 (SD = 21.51) on a scale of 1-100. The FS-ICU care subscale total mean score was 67.82 (SD = 21.05), while the FS-ICU decision-making subscale mean score was 69.30 (SD = 23.48), indicating the highest level of satisfaction was with decision making within the ICU. Qualitative extreme positive and negative data supported total QODD, total FS-ICU, and FS-ICU subscale scores. In comparison to previous research, nurse family member satisfaction with care and perceptions of quality of patient death and dying tended to align with nurse rather than family member scores. Implications: Nurse family members perceive the quality of care and patient death and dying differently than non-nursing family members. To improve their experience of EOL care, further examination of the particular needs of nurse family members is warranted.

Unfortunately, the experience of dying in the United States is often characterized by fragmented care, inadequate treatment of distressing symptoms, frequent transitions among care settings, and enormous care responsibilities for families. According to this report, the current health care system of rendering more intensive services than are necessary and desired by patients, and the lack of coordination among programs increases risks to patients and creates avoidable burdens on them and their families. Dying in America is a study of the current state of health care for persons of all ages who are nearing the end of life. Death is not a strictly medical event. Ideally, health care for those nearing the end of life harmonizes with social, psychological, and spiritual support. All people with advanced illnesses who may be approaching the end of life are entitled to access to high-quality, compassionate, evidence-based care, consistent with their wishes. Dying in America evaluates strategies to integrate care into a person- and family-centered, team-based framework, and makes recommendations to create a system that coordinates care and supports and respects the choices of patients and their families. The findings and recommendations of this report will address the needs of patients and their families and assist policy makers, clinicians and their educational and credentialing bodies, leaders of health care delivery and financing organizations, researchers, public and private funders, religious and community leaders, advocates of better care, journalists, and the public to provide the best care possible for people nearing the end of life.

"At least 1 in 5 Americans die while using intensive care service and–a number that is expected to increase as society ages" (Espinosa, Young, Symes, Haile, and Walsh, 2010, pages273). Nurses in the ICU are now required to not only provide life-sustaining care but also be competent in the complexities of end-of-life (EOL) care including withdrawing life support, symptom management, and emotional support for that patient and their loved ones. However, in spite of these statistics critical care nurses perceptions of providing EOL care in the ICU are not without concern. Research shows that critical care nurses feel that the number one barrier in providing EOL care is lack of knowledge (Beckstrand and Kirchhoff, 2005). Nurses also hold self-perceived deficiencies in EOL care competencies and require unique educational needs (Montagnini, Smith, and Balistrieri, 2012). The presented capstone project describes a pilot program that includes the implementation of EOL care education and standardized policy, procedure, and order sets in the ICU. The proposed education is tailored to critical care nurses and uses the Endof- Life Care Nursing Education Consortium train the trainer curriculum. The aim of the program is improve the quality of care at EOL in the ICU through education and the development of policy, procedure, and stander order sets.

In the intensive care unit (ICU), it can be both necessary and appropriate to evaluate family perceptions of quality to determine satisfaction with care. Objectives: To evaluate the quality of nursing care received in ICUs practicing PFCC as perceived by patients' families and to examine if this satisfaction positively correlates with satisfaction in the overall hospitalization and intent to recommend services to others. Design: A quality improvement study was conducted using a modified version of the Patient Satisfaction with Nursing Care Quality Questionnaire (PSNCQQ). Surveys were administered to a convenience sample of 31 family members of patients receiving care in three, PFCC, ICUs in a Detroit, metropolitan, teaching hospital. Results: Total PSNCQQ mean for the study sample was rated a "very good" (M=4.187, SD=.142). Ideally, EOLC should be appropriately planned, implemented and informed by robust evidence; however the ICU is a setting where a patient's health can deteriorate rapidly. Consequently, families (including family members, relatives and carers) are particularly vulnerable, and may experience shock, uncertainty and powerlessness in this technical and unfamiliar environment during EOLC. Families are optimally positioned to identify the key components of care at the end of life in the ICU, however their views and needs are under-represented in key policy and guidance documents. A potentially rich source of families' views is qualitative studies. By capturing the complex experiential aspects of healthcare phenomena, qualitative evidence can provide knowledge that may inform the delivery of healthcare. A number of qualitative studies have examined the perspective of families on EOLC in the ICU, however, a systematic review of this evidence has not been undertaken. Therefore, a systematic review and meta-synthesis was conducted to identify the experiences and perceptions of patients' families, of the EOLC delivered in the ICU setting, when life-sustaining treatments are withheld or withdrawn. The findings of this systematic review will provide policy-makers, clinicians and researchers with robust evidence to: (1) inform the development of future policy and evidence-based clinical guidelines; (2) promote quality and consistency of EOLC delivery, and; (3) assist with optimising the EOLC experiences of the patient's family in the ICU, when life-sustaining treatments are withheld or withdrawn. Methods: Systematic review and meta-synthesis was conducted using meta-aggregation. A comprehensive search for eligible published and unpublished qualitative studies (January 2006 - May 2016) was conducted across seventeen electronic databases. Quality assessment, data extraction, data synthesis and ConQual (a method used to rate the confidence of the synthesised findings) were conducted using an adaption of the methods of meta-aggregation. Thematic analysis processes were added to achieve analytic depth. Results: Thirteen studies of variable quality met the inclusion criteria. No Australasian studies were identified. Data collection methods across the studies included face-to-face interviews and focus groups. Meta-aggregation and thematic analysis resulted in 14 Page 13 of 229 categories and five synthesised findings, as follows: (1) the dying person: valued attributes of patient care; (2) communication with the family and within the healthcare team; (3) preparing the family and promoting a peaceful family-centred death; (4) family-centred support and care, and; (5) bereavement care. A conceptual model of preparedness was developed based on these findings that reflects the elements of EOLC care that are most valued by families. Central to preparedness was communication. Conclusion: The synthesised findings showed that families experienced both positive and negative aspects of EOLC, and have unmet needs for communication, family-centred support and care and bereavement care. The conceptual model of preparedness, which reflects families' views of high-quality EOLC in the ICU, can be utilised to target areas for practice improvement. As defined in this study, the concept of preparedness in the ICU, explains the circumstance in which families can best manage the death, their sadness, loss, and grief. The findings show that nurses in particular are optimally positioned to provide individualised support and care that aligns with the values, needs and wishes of each family. Achieving a state of preparedness in the ICU setting requires a unique combination of care and support, as reflected in the model. Research, policy and education recommendations are suggested based on the findings of this systematic review. An Australasian qualitative, multi-centre study that captures this healthcare phenomenon would provide robust evidence to inform region-specific policy and guideline development. In addition, a qualitative synthesis that incorporates evidence on the views of survivors of critical illness, families of survivors, and clinicians about the quality of EOLC delivered in the ICU setting, will contribute to our understanding of the barriers and facilitators to effective EOLC. Development of a practice standard on the delivery of EOLC in the ICU is also recommended, that is aligned with key performance indicators and mandated by registration bodies and governing critical care nursing/medicine agencies. Finally, undergraduate and postgraduate courses should consider preparing nurses to provide individualised care across a range of clinical settings to the dying person and their family, including effective communication and bereavement care.

Advance care planning and initiation of palliative care services bring hope to the patient population to enhance the quality of life at their end stages of life. According to the study shows, there are 22% of death happens in the intensive care unit in the United States (Troug et al, 2008), so the pursuit of quality care has increased interest in the designated area. Based on the research studies, the advance care planning and the comfort care services enhance quality of life, decreased use of life sustaining treatments, decreased hospital length of stay, increased use of hospice or palliative care services, and better patient/family cope up and satisfaction. The main objective of the study is to enhance the quality of life among patients with terminal and chronic life limiting illness by implementing a strategic plan of end of life care. The implementation plan includes the use of Physician orders for life sustaining treatment form (POLST) for all ICU patients, family conferences, routine palliative care consultation, and an added educational training of the health care workers, can provide a holistic care to the dying patients. The comfort theory will be incorporated into the project by assessing the patient needs, nursing interventions, and the evaluation of patient comfort. An end of life care survey has developed to evaluate the family and staff satisfaction with the above implementation plan. A dissemination of the project outcome will be communicated to the stakeholders through open meetings/discussions, staff development program, and make use of the organizational website and newsletters.
