Book excerpt: With burgeoning middle class populations and changing lifestyles, diabetes is rapidly emerging as a major health concern worldwide. A related complication, Diabetic Retinopathy (DR), affects approximately 1 of 3 people with diabetes and is the leading cause of adult blindness worldwide. Since DR often goes undiagnosed, there has been great interest in using artificial intelligence in the form of Deep Learning algorithms to automatically predict DR using retina (fundus) images. However, the practical application of these algorithms is impeded by the different levels of DR severity, limited algorithm reproducibility, and a large variability in fundus imaging devices. To address these concerns, I present the development of an image processing pipeline and a neural net algorithm that automatically tests image quality (based on brightness, color, and amount of blur), rejects poor quality images, and re-formats each image into a standard image resolution. In order to create a generalized model, I used a public Kaggle database or approximately 35,000 retina images and applied a transfer learning approach using the Inception v3 architecture, to build a convolutional neural net (CNN) model that predicts referable DR. As expected, the performance of the resulting model depended on the severity of DR, with AUCs ranging from 0.96 for severe DR to 0.74 for mild DR. I further customize the model using a smaller data set of 1156 smart-phone images from our clinical partner in India. On the Kaggle dataset, the best model achieved a sensitivity = 0.85 and specificity = 0.87. On the smaller dataset, the model attained a sensitivity = 0.82 and specificity = 0.80. Finally, as a possible alternative to fundus imaging, I explore the use of iris images to explore possible correlations with diabetic retinopathy and diabetes. Using cluster analysis and generating plots with PCA and T-SNE, we observed possible clusters in using GLCM features; however, there was insufficient data from healthy individuals to be able to draw any significant conclusions.

The application of artificial intelligence has not received much attention in the diagnosis, prediction and monitoring of diabetic retinopathy (DR). The hybrid artificial intelligent system includes image processing techniques and neural network trained with back propagation algorithm is proposed in this research work. Combined Cross Number points (CCN) technique which uses a 5×5 window embedded with Artificial Neural Network (ANN) technique has been proposed in the vasculature detection in order to detect the combination of bifurcation and crossover points in (RFI). On the other hand, three techniques are proposed in the ONH detection, namely simple thresholding technique and hybrid numerical differential approach as well as exponential histogram technique.Performance analysis of the system shows that ANN based technique for vascular intersection points detection achieves 100% accuracy on simulated images and a minimum of 92% accuracy on RFI obtained from DRIVE database. Thus, the simulated images have been used to train the artificial neural network (ANN) and on convergence the network is used to test (RFI) from DRIVE database. In the ONH detection, hybrid numerical differential and exponential histogram technique produces 90 % of accuracy as compared to the simple thresholding technique which only achieves 53% of accuracy.

CARA telemedicine platform based on AI program from Diagnos.inc was used for mass screening, triage and management of the different follow up visit.Methods: 79000 Patients with diabetes and pre-diabetes condition were screened (32%male, 67% female and 1% other gender not specified), a mean age (59.8 u00b113.4 Years), all the participants underwent automatic screening by CARA Tele-Retinal system, the picture acquisitions were performed by a qualified professional in a darkened room acquired by an non-mydriatic fundus camera, the median time for screening for DR was (10mn u00b12). The picture was taken from each eye (OD and OS) with the macula in the center (45 degree field of view) according to clinical gold standard protocol and Early Treatment of Diabetic Retinopathy (ETDRS) criteria, semi-automatic classification was performed by CARA algorithm DR0 (normal fundus), DR1 (Mild NPDR), DR2 (Moderate NPDR), DR3 (Severe NPDR) and DR4 (PDR). The receiver operator characteristic curve (AUC) was used to assess the improvement in detection of any DR by automatic detection and classification mode using CARA algorithm compared to masked trained optometrist mode. Results: 53.04% of the patients had history of diabetes condition, 46.96% of the patients had no history of diabetes, while 37.7% of the patients were found with no DR, DR was automatically detected by CARA algorithm in 61.5% of the patients, those with DR1 condition were (36.8%), DR2 (11.5%), DR3 (8.9%) and DR4 condition were (4.3%). Therefore, 0.8% of the patients there had unclassified images of retinal fundus, while 7% of the patients detected with severe condition of DR were referred urgently to the specialist for treatment. The sensitivity of detecting any DR by automatic grading system (CARA algorithm) was 79.8% (95% CI, 72.1% -87.5%), the specificity was 87.7% (95% CI, 82.3%-93.0%) with an AUC of 88.5%. Conclusion: Telemedicine program through AI application was successfully implemented and executed on a large scale population with significant improvements, allowing faster and comfortable DR detection programs for larger population with high efficiency in the safety way. These results, highlights the strength and simplicity of such an automatic telemedicine program with the potential to maximize access and standardized monitoring during the fellow up visits with best cost clinical effectiveness and cost-effectiveness.

In particular, we concentrate on the development of the algorithmic components of an automatic screening system for diabetic retinopathy. The techniques developed can be categorized in: quality assessment and improvement, lesion segmentation and diagnosis. For the first category, we present a fast algorithm to numerically estimate the quality of a single image by employing vasculature and colour-based features; additionally, we show how it is possible to increase the image quality and remove reflection artefacts by merging information gathered in multiple fundus images (which are captured by changing the stare point of the patient). For the second category, two families of lesion are targeted: exudate and microaneurysms; two new algorithms which work on single fundus images are proposed and compared with existing techniques in order to prove their efficacy; in the microaneurysms case, a new Radon transform-based operator was developed. In the last diagnosis category, we have developed an algorithm that diagnoses diabetic retinopathy and diabetic macular edema based on the lesions segmented; starting from a single unseen image, our algorithm can generate a diabetic retinopathy and ma cular edema diagnosis in _22 seconds on a 1.6 GHz machine with 4 GB of RAM; additionally, we show the first results of a macular edema detection algorithm based on multiple fundus images, which can potentially identify the swelling of the macula even when no lesions are visible.
