L'incidence et la prévalence de l'IRC sont en progression constante du fait du vieillissement de la population et de l'augmentation des pathologies vasculaires ou métaboliques ayant un retentissement rénal. Une prise en charge néphrologique tardive de l'IRC, observée encore dans près de 20 à 30% des cas en dehors de tout problème d'accès aux soins, se révèle préjudiciable pour les patients. Afin d'appréhender les facteurs associés à cette prise en charge néphrologique tardive, un questionnaire spécifique a été adressé à un échantillon représentatif de 500 médecins : généralistes lorrains : le taux de réponse est de 43%. Outre le sentiment d'être jugé négativement par les néphrologues et la crainte de perdre de vue le patient adressé au néphrologue, les difficultés pour le médecin généraliste sont nombreuses : Symptomatologie silencieuse, trompeuse et souvent tardive de l'IRC. Formation médicale insuffisante. Méconnaissance des référentiels de l'ANAES sur le diagnostic de l'IRC et sa prise en charge. Méconnaissance des possibilités thérapeutiques avant le stade terminal, mais aussi au stade terminal (notamment chez le sujet âgé). Gestion des craintes et réticences du patient. Eloignement du néphrologue et délais plutôt longs des consultations de néphrologie. Absence de communication.
