Until the 1950s, chronic renal failure was considered to be an inexorably lethal condition. This is no longer the case. In addition, the disease, severe uremic syndrome, is now extremely rare, if existent at all, in industrialized countries. Physicians of my generation who saw patients hospitalized with hemor raghes, pericarditis, severe anemia, cardiac failure, "malignant hypertension," pruritus, vomiting, generalized edema, and convulsions are particularly grate ful for this progress. I well remember seeing such patients hospitalized in the last days or weeks of their lives and also remember the sense of impotence I suffered for the com plete lack of efficient measures I had at my disposal to manage their condition. Nowadays, hemodialysis, peritoneal dialysis, and kidney transplantation allow patients with chronic renal failure to survive for very long periods of time in a satisfactory condition. Why then is there still a sense of dissatisfaction and why should we study dietary management? The drawbacks of dialysis and transplantation are the main reasons, but the certainty that dietary therapy is complementary to dialysis and even better than dialysis in certain conditions, is also very important.

The impetus for the study was an article in the Los Angeles Times about architectural interventions that were planned for several noise-impacted schools under the air corridor of Los Angeles Interna tional Airport. These interventions created an opportunity to study the same chil dren during noise exposure and then later after the exposure had been attenuated. The study was designed to test the generality of several noise effects that had been well established in laboratory experimental studies. It focused on three areas: the relationship between noise and personal control, noise and attention, and noise and cardiovascular response. Two years later, a second study, designed to replicate and extend findings from the first, was conducted.

Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.

With contributions from more than a dozen well-renowned researchers in political science, law and sociology, this book analyzes IPBES functioning and challenges in terms of the knowledge selection process and actors involved. The book reveals that, through its conceptual framework, IPBES promotes a pluralistic view of nature that calls for a broadening of the disciplinary frontiers. It combines natural science and social science research and also includes indigenous and local knowledge. IPBES is considered to represent the institutionalization of a permanent knowledge assessment on biodiversity and is often referred to as an IPCC success story, constituting a new stage in global environmental governance. In analyzing the knowledge selection process for IPBES decision making, the book better situates IPBES within the biodiversity and global governance domain. Book excerpt: The U.S. population is aging, and occupational therapy practitioners working with older adults must have the skills to address multiple health issues, including vision loss. Although typical aging does not cause low vision, four major causes of vision impairment are directly related to the aging process: (1) age-related macular degeneration (AMD), (2) diabetic retinopathy, (3) glaucoma, and (4) cataracts. Instances of these impairments are expected to double by the year 2050, making low vision a significant public health issue. This Practice Guideline includes occupational therapy assessment and intervention guidelines for older adults who have visual acuity impairments, visual field impairments, or both as a result of a low vision diagnosis. Screening, referral, and evaluation are covered for clients for central visual field impairments, such as AMD and cataract; peripheral field impairments, such as glaucoma, hemianopsia, and quadrantanopsia; and mixed visual field loss, such as diabetic retinopathy. This work can help occupational therapists and occupational therapy assistants, as well as individuals who manage, reimburse, or set policy regarding occupational therapy services, understand the contribution of occupational therapy to treating older adults with low vision. Appendixes include selected diagnostic and billing codes and evidence tables.
