Book excerpt: The Department of Health introduced a target to reduce MRSA across all NHS trusts by 50 per cent by 2008 and C. difficile by 30 per cent by 2010-11. MRSA had been reduced by 57 per cent by the end of March 2008 and C. difficile by 41 per cent. While a quarter of trusts have reduced MRSA by more than 80 per cent, in 12 per cent of trusts there has been an increase in MRSA infections. Twenty-nine per cent of trusts have reduced C. difficile by more than 50 per cent, but in 19 per cent of hospital trusts the numbers of C. difficile infections have increased. Since the introduction of the targets, the Department has spent some £120 million tackling healthcare associated infections. There have also been unquantifiable administrative costs and local expenditure on the drive to reduce infection rates. These initiatives have led to savings on treatment of between £141 million and £263 million, as well as reducing discomfort, disability and, for some, death that might have been caused by these avoidable infections. There has also been a perceptible change in trust leadership on tackling infections. The impact has not, however, been the same in all trusts or for other infections. Following the Department's intervention to improve recording on death certificates where MRSA or C. difficile was the underlying cause or a contributory factor, in 2007 around 9,000 people were reported as having died in such circumstances. There is still no national information on deaths from other healthcare associated infections such as urinary tract infections and pneumonia and blood stream infections due to other causes may be increasing.

Book excerpt: In recent years a great deal of effort has been put into improving risk management in departments and in 2002 this was given further impetus when a two year risk programme was launched by the Prime Minister. This report looks at the progress made since the previous NAO report in 2000 (HC 864 1999-2000, ISBN 0105569488). It is based on a survey of the 20 main Whitehall departments, focus groups of 27 departmental risk managers, comparisons with the private sector, academic research and five case studies. The general conclusion is that significant improvement has been made but more needs to be done to make effective risk management a central part of general management processes. The ability to take risks and innovate, keep projects on track and handle complex service delivery needs to be further developed.

This book was released on 2004 with total page 68 pages. Available in PDF, EPUB and Kindle. Book excerpt: Following on its previous report on the management and control of hospital acquired infections in the NHS (HCP 230, session 1999-00, ISBN 0105566659) published in February 2000, this report by the National Audit Office examines: the extent of progress made towards implementation of good practice recommendations; whether there have been any discernible changes in patient outcomes; and how other countries are addressing these issues. Findings include that implementation and compliance has been patchy, with notable progress at trust level in the strategic prioritisation of infection control processes, although wider factors continue to impede good practice, including the lack of sufficient information on the extent and costs of hospital acquired infections. Further action is required using a range of approaches to change staff behaviour to reduce risks, and a number of recommendations are made to the Department of Health, the Healthcare Commission and NHS trusts which aim to help overcome identified constraints.

Available in PDF, EPUB and Kindle. Book excerpt: Medical microbiology concerns the nature, distribution and activities of microbes and how they impact on health and wellbeing, most particularly as agents of infection. Infections remain a major global cause of mortality and in most hospitals around one in ten of those admitted will suffer from an infection acquired during their stay. The evolution of microbes presents a massive challenge to modern medicine and public health. The constant changes in viruses such as influenza, HIV, tuberculosis, malaria and SARS demand vigilance and insight into the underlying process. Building on the huge success of previous editions, Medical Microbiology 18/e will inform and inspire a new generation of readers. Now fully revised and updated, initial sections cover the basic biology of microbes, infection and immunity and are followed by a systematic review of infective agents, their associated diseases and their control. A final integrating section addresses the essential principles of diagnosis, treatment and management. An unrivalled collection of international contributors continues to ensure the relevance of the book worldwide and complementary access to the complete online version on Student Consult further enhances the learning experience. Medical Microbiology is explicitly geared to clinical practice and is an ideal textbook for medical and biomedical students and specialist trainees. It will also prove invaluable to medical laboratory scientists and all other busy professionals who require a clear, current and most trusted guide to this fascinating field.

Book excerpt: The introduction of clinical governance gives National Health Service organisations a powerful incentive to focus on serious failures in health care. This report reviews what is known about the scale and nature of these failures, examining the extent to which the NHS has the capacity to learn from them when they do occur, and recommending measures which could help to minimise the likelihood of repeated failures in service in the future. Information was drawn from industry, aviation and academic research.
