Book excerpt: "Uncontrolled diabetes causes target end organ damage that is devastating to multiple body systems including cardiovascular, neurologic, and renal systems. Adequate management of diabetes is required to prevent these health complications but unfortunately includes many lifestyle changes that are difficult to maintain. Weekly support in the form of telehealth has been demonstrated in the literature as a method to help patients with diabetes improve self-efficacy and glycemic control. This evidence-based practice change project implemented telehealth in the form of phone calls, text messages, or emails among a group of participants (N=15) with type 2 diabetes at a privately owned health clinic in a large metropolitan area in a northeastern state. Blood glucose levels decreased and self-efficacy scores increased after the implementation of the telehealth intervention. Project findings led to recommendations for the practice site, and implications for policy and health promotion. It is recommended that future implementation of this intervention occurs among a larger, heterogenous population and that more methods of telehealth be included. Book excerpt: This dissertation evaluates the effectiveness of a telehealth remote patient monitoring (RPM) intervention that combines structured self-monitoring of blood glucose (SMBG) and nurse care coordination (NCC) to improve outcomes in persons with type 2 diabetes. The primary aims were to compare differences between glycated hemoglobin (A1C), self-management, self-efficacy, and knowledge compared to usual care after 12 weeks. A two-group randomized clinical trial comparing usual care and RPM--with structured SMBG called paired testing--enrolled 90 people with type 2 diabetes, aged 30-70 years, not taking insulin, in NCC for 12 months with A1C between 7.5%-10.9%. The RPM system includes a computer connected to the Internet to transmit glucose data. Participants received education in paired glucose testing, glucose goals, and problem-solving. Paired testing--checking glucose before and two hours after a meal or physical activity for one week--created action plans for behavioral changes. Glucose data were analyzed weekly using pattern analysis software summarized by premeal, postmeal, and change values, with feedback via electronic health record for asynchronous communication. Nurses telephoned participants monthly to discuss goal achievement. Glucose patterns not reflecting improvement after four weeks required shared decision-making medication options. Separate mixed-effects models were used to determine if intervention or control improved over time, and if intervention showed greater improvement. Data (n = 87) included mean (SD) age 55.7 ± 10.6, mean years with diabetes 8.2 ± 5.4, body mass index 34.1 ± 6.7, and mean A1C 8.3 ± 1.1. There was a significant difference in linear rate of change in A1C indicating the treatment group had on average a faster rate of decline and a faster mean rate of improvement. The treatment group showed greater improvement in summary of diabetes self-care activities subscales of carbohydrate spacing, monitoring blood glucose, and foot care. Neither group improved knowledge or self-efficacy. This intervention shows promise for implementing a complete feedback loop to engage participants in gathering actionable data, using RPM to transmit and analyze structured data, and providing a mechanism for clinicians to virtually communicate to support timely behavior and treatment changes.

Book excerpt: Despite both pharmacological and lifestyle-based treatment options, almost half of U.S. adults diagnosed with diabetes fall short of recommended glycemic targets. Evidence shows diabetes self-management education and support (DSMES) increases adherence to DSM behaviors resulting in improved glycemic control and increased quality of life. One of the core content areas in DSMES curriculum is healthy eating. Home meal preparation, associated with higher diet quality, has been steadily declining in the U.S. over the past 30 years. Incorporating evidence-based cooking interventions into DSMES may be a feasible and efficacious approach for promotion of positive behavior change, however current research in this area is limited. The objective of this study was to determine whether a cooking course augmented with a DSMES component (Cooking Matters® for Diabetes) improves glycemic control (primary outcome), adherence to DSM behaviors, health-related QOL (HRQOL), diet quality, and food security (secondary outcomes) compared to standard of care (SOC). Twenty-eight adult participants diagnosed with type 1 or type 2 diabetes were randomized to a 6-week Cooking Matters® for Diabetes (CMFD) intervention or SOC at an urban outpatient clinic. Hemoglobin A1c (A1c), validated measurements of DSM behaviors (Summary of Diabetes Self-Care Activities, Diabetes Self-Management Questionnaire, Diabetes Empowerment Scale Short Form, Stanford Diabetes Self-Efficacy scale), HRQOL (SF-12), diet quality (HEI-2015), and food security (U.S. Adult Food Security Module) were evaluated at baseline, post-intervention, and 3-months post-intervention. Between group differences in mean measurements adjusted for sex and baseline measures were compared via linear regression modeling. A1c trended lower (-0.45%) in CMFD participants compared to control at 3-months post-intervention (p=0.098). Glucose testing (+1.68, 0-7 scale), general diet (+1.98, 0-7 scale), foot care (+1.63, 0-7 scale), glucose management (+2.89, 0-10 scale), overall DSM (+1.42, 0-10 scale), and mental HRQOL (+8.28, 0-100 scale) were significantly higher (all p≤0.05) in the CMFD arm post-intervention. Participants in CMFD also showed positive trends in self-efficacy, empowerment, diet quality, and food security. However, most differences declined at 3-months post-intervention. The positive impacts of CMFD observed in this study support the incorporation of evidence-based cooking interventions as part of DSMES, while the declining impact of the intervention over time suggests the need for extended programming to maintain effects and future research to improve long-term efficacy.

This scientific anthology is the first of its kind, in that it connects health promotion with the salutogenic theory of health throughout the chapters. the authors here expand the understanding of health promotion beyond health protection and disease prevention. The book focuses on describing and explaining salutogenesis as an umbrella concept, not only as the key concept of sense of coherence.
