Currently, however, prices and promotions often encourage selection of foods that undermine healthy diets. To address this challenge, and to support Member States in implementing policy measures, as recommended by the Framework for Action from the 2014 Second International Conference on Nutrition, the World Health Organization (WHO) is in the process of developing evidence-informed policy guidelines on the food environment, including fiscal and pricing policies. This review on contextual factors to be considered in the implementation of fiscal and pricing policies was prepared as part of the required process for WHO guideline development.

These guiding principles take a holistic approach to diets; they consider international nutrition recommendations; the environmental cost of food production and consumption; and the adaptability to local social, cultural and economic contexts. Book excerpt: To address the increasing number of requests from Member States for guidance on how to design fiscal policies on diet, WHO convened a technical meeting of global experts in fiscal policies on 5-6 May 2015 in Geneva. The main objectives of the meeting were to review evidence and existing guidance, discuss country case studies and provide considerations with regards to the scope, design, and implementation of effective fiscal policies on diet. It was concluded that there is reasonable and increasing evidence that appropriately designed taxes on sugar-sweetened beverages would result in proportional reductions in consumption, especially if aimed at raising the retail price by 20% or more. There is similar strong evidence that subsidies for fresh fruits and vegetables that reduce prices by 10-30% are effective in increasing fruit and vegetable consumption. Vulnerable populations, including low-income consumers, are most price-responsive and, in terms of health, benefit most from changes in the relative prices of foods and beverages. Consistent with the evidence on tobacco taxes, specific excise taxes -- as opposed to sales or other taxes -- based on a percentage of retail price, are likely to be most effective. In countries with strong tax administration, taxes that are calculated based on nutrient content can have greater impact. A proper situation analysis, good political advocacy, appropriate objective setting and evaluation, should be part of the multidisciplinary development and implementation of such policies. It is also important in this process to be proactive in counteracting the industry arguments and efforts to oppose the development and implementation of tax measures or attenuate their effects. There are evidence gaps that could be addressed, with more countries developing and implementing such fiscal policies. Lack of standards or criteria experienced by countries and the development of a nutrient profile model for designing and implementing fiscal policies was recommended. In addition, there was a call for a manual on developing and implementing fiscal policies for diet.

Book excerpt: In Chapter 2, I modelled the cost of healthy and current diets for an average Dutch household. On average, healthy diets were 24% more expensive than current diets and this difference increased by increasing educational level. The findings in Chapter 3 support those in Chapter 2 as I found that dietary cost partly explains socioeconomic inequalities in dietary quality measures. In Chapter 4, I investigated the mediating role of other diet-related material and psychosocial factors in the relation between SEP indicators and dietary quality. When combining the results from Chapters 3 and 4, I found that dietary cost as a material resource and cooking skills and resilience to the unhealthy food environment as psychosocial resources mediated around 30% of the association between SEP indicators and dietary quality. Food expenditure, perceptions of healthy food accessibility, healthfulness of the food retail environment, insensitivity to food cues and eating habits did not mediate this association. Thus, I concluded that diet-related material and psychosocial factors only partly explain socio-economic inequalities in dietary behaviours. Then, we developed a virtual supermarket in which nudging and pricing strategies were implemented to investigate the separate and combined effectiveness of these strategies on the proportion of healthy food purchases. As described in Chapter 6, a combination of different recruitment strategies led to the inclusion of 455 participants, of whom only a small proportion was truly low SEP. The virtual supermarket experiment showed that salient nudging strategies as well as pricing strategies alone did not lead to increases in healthy food purchases (Chapter 7). However, actively communicating these pricing strategies and combining these with nudges led to an approximately 3% increase in the proportion of healthy food purchases. The aim of this report is to establish an evidence base for the application of food and beverage taxes and complementary measures to encourage dietary substitution towards healthier, local food products in Fiji.
