Book excerpt: The problem facing Westmont Fire Department is how to implement an Automated External Defibrillator (AED) program within the Village of Westmont and the Westmont Fire Department for the purpose of increasing Advanced Life Support (ALS) services to the community. The Westmont Fire Department is challenged on a daily basis to keep pace with the changes in the emergency medical services of the fire service today. The purpose of the Applied Research Project (APR) is to investigate the effect of having Automated External Defibrillators on fire department apparatus and vehicles as well as the development of an operational plan for AEDs within the Village of Westmont. The research process was facilitated using action and evaluation research to answer the following questions: 1. Would an AED program improve emergency services to the community? 2. Would the development and implementation of an AED program be cost effective? 3. How effective are AEDs in actual field use? 4. What are the advantages of having an AED program? The process used to answer the above questions included a ten-question survey mailed to a total of forty fire departments, twenty in the State of Illinois and twenty to the Executive Fire Officer participants across the United States (Appendix A). Thirty-three of the forty questionnaires (82.5%) were completed and returned (Appendix B). The Illinois Fire Chiefs Association and Executive Fire Officer participant departments were chosen because they represent a sample selection of the fire departments found in Illinois and across the nation. The results of this project demonstrated positive effects for the implementation of an AED program within the Village of Westmont Fire Department. The results also indicated the need to strategically implement a plan to place AEDs in businesses, organizational groups, and schools, all of which have high probability for sudden cardiac arrests. It is the author's recommendation that an Automated External Defibrillator program to improve Emergency Medical Services and increase the survival rate of sudden cardiac arrest (SCA) victims be implemented for the Village of Westmont. The purpose of this applied research project was to determine whether or not it is feasible for the Channahon Fire Protection District to implement an early defibrillation program for its jurisdiction. A descriptive research methodology was used to answer the following research questions, while action research methodology was used to develop the framework for implementing an early defibrillation program: 1. Can the Channahon Fire Protection District impact the survival rate of heart attack victims through an early automatic external defibrillation program? 2. How can the Channahon Fire Protection District financially support an early automatic external defibrillation program? 3. How can the Channahon Fire Protection District make the public aware of the need for an early automatic external defibrillation program? 4. What can the Channahon Fire Protection District do to educate the public first responders on the proper use of automatic external defibrillators? The principal procedures utilized to complete the research project were a review of the fire service literature contained in magazine articles, fire service journals, and resources of the United States Fire Administration. Also referenced was literature available through the American Medical Association. The major results of this research were that successful early defibrillation programs involved delivery strategies that focused on rapid intervention as time was the major component to quality in an automatic external defibrillator program (Browett, 1998). Experts believed that more than three hundred lives a day could be saved if the maximum time to defibrillation were in the five to seven minute range (Newman,1997). Survival rates for out-of-hospital cardiac arrest are as low as 1.4% (Olson, Fontanarosa,1999) with current intervention techniques. Other factors that might influence the placement of AED's included call volume, response-area demography and the distribution of advanced cardiac life support (ACLS) and basic life support (BLS) response times (Becker,Mandell,1991). The recommendations resulting from this research included the Channahon Fire Protection District developing a comprehensive plan and framework for implementing an early defibrillation program, specific to its resouces and particular needs. Simple to operate, minimally trained non-medical personnel may successfully operate these devices. Due to their proven accuracy and reliability in treating sudden cardiac arrest (SCA), the American Heart Association recognizes them as essential in treating victims of sudden cardiac arrest in certain situations. Because of these factors, AEDs have gained in popularity among municipal service providers as well as private industry as a means to broaden the availability of defibrillation to the general populace and to reduce the time it takes to bring a defibrillator to the side of a stricken patient. Acknowledging that the City of Naperville has yet to develop a program to promote public access defibrillation (PAD) within the community, research was undertaken through descriptive method to conduct an assessment to evaluate the need for implementing a PAD program in Naperville. The project attempted to answer the following questions. 1. What is the prevalence of sudden cardiac arrest patients treated by the Naperville Fire Department? 2. What are the essential components of a public access defibrillation program? 3. What community groups should be considered for inclusion in an implementation plan? 4. What have other organizations done to successfully implement public access defibrillation programs? Research procedures combined the use of two retrospective studies and a survey questionnaire. One study consisted of a three-year review of patient care reports to determine the location of sudden cardiac arrest events treated by fire department personnel. A second look-back study sought to quantify cardiac arrest survival data for patients treated in 1999. The third research effort included the distribution of a survey questionnaire to organizations, both public and private, that had previously implemented PAD programs. Results indicated that the Naperville Fire Department demonstrated a hospital discharge rate of 5.8 percent for individuals treated for out-of-hospital sudden cardiac arrest. This contrasted with a successful resuscitation rate of 15.15 percent reported by questionnaire survey participants using AEDs. Despite the posting a relatively rapid response time for ALS services to SCA events (averaging 3:25 minutes), most cardiac arrest patients treated by Naperville paramedics presented with an initial EKG of asystole. The majority of arrests occurred in the home (62.24%) and appeared to result from a cardiac etiology 84.06 percent of the time. Based upon the demonstrated survival rate and the apparent successes of police agencies' participating in the survey, recommendations were made to promote the implementation of a PAD program. With the Fire Department serving as the lead agency, target priorities for deployment included police department patrol vehicles, appropriate public access government buildings, institutional residential care occupancies, and large corporate campuses. Automated external defibrillator (AED) was set in public area to decreasing the time to defibrillation in Taiwan since 2013. However the usage and effect of automated external defibrillator remained unknown. Methods:The general information of OHCA patients was acquired from Taichung Fire Department. The AED geographic data was collect from national registered system in m Ministry of Health and Welfare. We matched the location between AED and OHCA patients to know the usage and effect of AED. Result:The OHCA patients attempted to resuscitation by emergency medical technician was 2609 in Taichung city since 2017/1/1-2017/12/31. 395 cases (15.1%) were collapsed in the public area and 260 cases (10%) were shock by Emergency Medical Technician. 52 cases (1.9%) had good neurologic outcome. There were 952-registered AED at the same time in Taichung city. The usage of AED is only once in 2017. However, there were 1108 patients with AED within a distance of less than 300 meters.Conclusion:Simply setting AED does not seem to increase usage. Maybe need volunteer, education or other device to improving the usage of AED.
