An implementation plan (Cash 1991) responding to the Defense Nuclear Facilities Safety Board Recommendation 90-7 (FR 1990) was issued in March 1991 describing the activities that were planned and underway to address each of the six parts of Recommendation 90-7. A revision to the original plan was transmitted to US Department of Energy by Westinghouse Hanford Company in December 1992. Milestones completed this quarter are described in this report. Contents of this report include: Introduction; Defense Nuclear Facilities Safety Board Implementation Plan Task Activities (Defense Nuclear Facilities Safety Board Recommendation for enhanced temperature measurement, Recommendation for continuous temperature monitoring, Recommendation for cover gas monitoring, Recommendation for ferrocyanide waste characterization, Recommendation for chemical reaction studies, and Recommendation for emergency response planning); Schedules; and References. All actions recommended by the Defense Nuclear Facilities Safety Board for emergency planning by Hanford Site emergency preparedness organizations have been completed.

The baseline IPP was issued as DOEs̀ Defense Nuclear Facilities Safety Board (DNFSB) Recommendation 94-1 Implementation Plan (IP), which was transmitted to the DNFSB on February 28, 1995. The IPP is being further developed to include complex-wide requirements for research and development and a long-range facility requirements section. The planned additions to the baseline IPP are being developed based on a systems engineering approach that integrates facilities and capabilities at the various DOE sites and focuses on attaining safe interim storage with minimum safety risks and environmental impacts. Each affected DOE site has developed a Site Integrated Stabilization Management Plan (SISMP) to identify individual site plans to implement the DNFSB Recommendation 94-1 and to provide a basis for formulating planned additions to the IPP. The SISMPs were developed based on the objectives, requirements, and commitments identified in the baseline DNFSB Recommendation 94-1 IPP. The SISMPs will be periodically updated to reflect improved integration between DOE sites as identified during the IPP systems engineering evaluations.

The DNFSB expressed concern about the existing storage conditions for these materials and the slow pace at which the conditions were being remediated. The DNFSB also expressed its belief that additional delays in stabilizing these fissile materials would be accompanied by further deterioration of safety and unnecessary increased risks to workers and the public. In February 1995, DOE issued the DNFSB Recommendation 94-1 Implementation Plan (O'Leary 1995) to address the concerns identified in DNFSB Recommendation 94-1. The Implementation Plan (IP) identifies several DOE commitments to achieve safe interim storage for the legacy fissile materials, and constitutes DOE's baseline DNFSB Recommendation 94-1 Integrated Program Plan (IPP). The IPP describes the actions DOE plans to implement within the DOE complex to convert its excess fissile materials to forms or conditions suitable for safe interim storage. The IPP was subsequently supplemented with an Integrated Facilities Plan and a Research and Development Plan, which further develop complex-wide research and development and long-range facility requirements and plans. The additions to the baseline IPP were developed based on a systems engineering approach that integrated facilities and capabilities at the various DOE sites and focused on attaining safe interim storage with minimum safety risks and environmental impacts. Book excerpt: In June of this year, the Department of Energy (DOE) issued directives DOE O 414.1C and DOE G 414.1-4 to improve quality assurance programs, processes, and procedures among its safety contractors. Specifically, guidance entitled, ''Safety Software Guide for use with 10 CFR 830 Subpart A, Quality Assurance Requirements, and DOE O 414.1C, Quality Assurance, DOE G 414.1-4'', provides information and acceptable methods to comply with safety software quality assurance (SQA) requirements. The guidance provides a roadmap for meeting DOE O 414.1C, ''Quality Assurance'', and the quality assurance program (QAP) requirements of Title 10 Code of Federal Regulations (CFR) 830, Subpart A, Quality Assurance, for DOE nuclear facilities and software application activities. [1, 2] The order and guide are part of a comprehensive implementation plan that addresses issues and concerns documented in Defense Nuclear Facilities Safety Board (DNFSB) Recommendation 2002-1. [3] Safety SQA requirements for DOE as well as National Nuclear Security Administration contractors are necessary to implement effective quality assurance (QA) processes and achieve safe nuclear facility operations. DOE G 414.1-4 was developed to provide guidance on establishing and implementing effective QA processes tied specifically to nuclear facility safety software applications. The Guide includes software application practices covered by appropriate national and international consensus standards and various processes currently in use at DOE facilities. While the safety software guidance is considered to be of sufficient rigor and depth to ensure acceptable reliability of safety software at all DOE nuclear facilities, new nuclear facilities are well suited to take advantage of the guide to ensure compliant programs and processes are implemented. Attributes such as the facility life-cycle stage and the hazardous nature of each facility operations are considered, along with the category and level of importance of the software. The discussion provided herein illustrates benefits of applying the Safety Software Guide to work activities dependent on software applications and directed toward the design of new nuclear facilities. In particular, the Guide-based systematic approach with software enables design processes to effectively proceed and reduce the likelihood of rework activities. Several application examples are provided for the new facility.

The DNFSB an independent oversight organization within the executive branch, was created by Congress in 1988 to provide advice and recommendations to the secretary of energy regarding public health and safety at the defense nuclear facilities managed by the Department of Energy. This study captures how the Board met the competing national security, health and safety, environmental, government, and public demands placed upon DOE's defense nuclear facilities, explicating the principles and techniques the Board employed to efficiently function as a federal agency and effectively fulfill the Board's unique mandate under the Atomic Energy Act of 1954, as amended. While it chronicles events, the study also serves as the tutorial for those charged with the future administration of the Board's enabling legislation, making available to the current and future leadership the philosophical and jurisprudential underpinnings of the form of governance captured in the Board's enabling legislation and the resolution of changing and sometimes opposing national requirements. The first chapter discusses the handling of safety issues in the defense nuclear complex prior to the creation of the Board. The chapter also examines historical circumstances that produced pressures to move toward more external regulation, including major accidents involving nuclear technology (especially Chernobyl), the waning of the nuclear arms race, and the lifting of the secrecy about the safety risks and environmental damage. The second chapter reviews the debates in Congress that led up to the legislative compromise that created the Board as an expert body that would act as an independent adviser to the secretary of energy, rather than as a regulator. The third chapter describes the development of the Board's manner of proceeding when it conducted oversight, in particular, how it interacted with the Department of Energy, congressional oversight committees, and the public, and how it wielded the tools that Congress granted it to exert authority that was effectively action-forcing. The fourth chapter examines important recommendations on nuclear safety, both site-specific and complex-wide, that the Board issued to the secretary of energy, and how their follow-up was handled.
