Hourly rounds also give the patient and nurse to discuss any concerns they may have about the plan of care. The process of hourly rounds would require educating the nursing staff about the process. Education materials such as pamphlets and in services would be needed to inform everyone of the new process. Step by step instructions on how to do hourly rounds will be presented. To evaluate the effectiveness of the hourly rounding process patients will fill out questionnaires about the hourly rounds. That information can be used to determine if hourly rounds increase patient satisfaction and safety. Nurses will also be interviewed about the hourly rounding process. The process will be reviewed and determined the method is effective I increasing patient satisfaction and safety.

Book excerpt: Effective implementation of nursing staff proactive rounding, a structured method of consistent proactive inquiry and responsiveness to patient needs, has been linked to improved patient satisfaction. Barriers and challenges to proactive rounding processes must be overcome for successful and effective implementation and sustainment to take place. The purpose of this evidence-based quality improvement practice project was to pilot implementation of structured proactive nursing staff hourly rounding and effectively integrate it into current practice to improve patient satisfaction. The project utilized a quasi-experimental nonequivalent group design on a 39-bed medical surgical/telemetry unit to compare top box Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) patient satisfaction scores before and after implementation of a structured proactive rounding process. A total of 59 nursing staff participants took part in the educational sessions. This included clinical care supervisors, RNs, and patient care assistants. Two-sample t-tests performed on HCAHPS top box composites performed on August 2015-August 2016 data (including project month), revealed that the t-statistics were not significant at the .00625 Bonferroni corrected critical alpha level. The differences in top box scores were not significant. A clinically significant increase was noted in the domain “Response of Hospital Staff” from July to August 2016, increasing from 51.7 to 58.9 percent although less than the score reported from August 2015 (61.5 percent). Clinically significant increases in HCAHPS composite top box scores during the three-week timespan of returned surveys were noted in all but three top box scores: Care Transitions reduced from 35.6 to 16.7 percent, Responsiveness of Hospital Staff scores fluctuated between 50 to 100 percent, and Hospital Environment fluctuated between 50 percent and 75 percent. Leader rounding patient satisfaction data revealed clinically significant percentage changes. One-sample t-tests between proportions were performed for each of the leader rounding questions results. The difference between yes responses and no or na/no response answers for each question both before and after the start of the project were significant at the Bonferroni corrected critical alpha level of .007. Two-sample t-tests between proportions were performed on each leader-rounding question to determine whether there was a significant difference between the yes responses for each question both before and after project implementation. The t-statistic was not significant at the Bonferroni corrected critical alpha level of .007. The difference between yes responses were not significant. Clinical significance could be observed through increased positive responses from before project implementation to after in the areas of pain control, staff rounding every hour, staff assisting timely, and whether patients perceived they were getting rest at night. Reductions in responses were noted in the areas of whether caregivers are explaining what they are doing and why, and an increase was noted in the number who had questions regarding their discharge. Communication, understanding, accountability, and engagement are core essentials that can be utilized in the development of processes that contribute to patient satisfaction, including proactive rounding. Process development, implementation, and evaluation are shared actions involving both nursing staff and leaders. Further research should assess sustainability of proactive rounding and competency of the practice through a long-term study utilizing multiple units or multiple sites.

Book excerpt: The purpose of this practice improvement project was to determine if a relationship exists between purposeful hourly rounding and incidence of patient falls and patient satisfaction with information received from nurses for male and female patients over 18 years in one 60-bed acute rehabilitation setting. The first question that guided this project was: Are patient falls in an acute rehabilitation setting reduced when purposeful hourly rounding occurs? The second question was: Do patient satisfaction scores improve when purposeful hourly rounding was implemented in acute rehabilitation setting? This project implemented purposeful hourly rounding by nursing staff over a three-week intervention period in one rehabilitation hospital with an average daily census of 40 male and female patients. Employee education occurred in February, 2017. Fall rates were gathered from the hospital's internal incident reporting system. Patient satisfaction scores were obtained from Press Ganey surveys. The patient satisfaction results and the fall rates of 39 patients who did not experience purposeful hourly rounding were compared to the patient satisfaction and fall rates of 68 patients who experienced the purposeful hourly rounding. This project used a quasi-experimental correlational design. An independent t-test analysis did not demonstrate a difference between the patient satisfaction scores on 10 surveys received from the first group of patients and the 5 surveys received from the patients who experienced rounding. A chi-square analysis of the fall rates did not demonstrate a statistical difference between the groups. Although the results were not statistically significant, the project provided a baseline for future projects such as identifying barriers for staff to perform purposeful hourly rounding and improving the preparation for staff to perform hourly rounding.

Planned hourly rounding is a proactive approach to patient care. Hourly rounding reduces patient fall, skin break downs, call light use, noise level and also improve patient satisfaction. Studies on hourly rounding to increase the patient safety and patient satisfaction showed a 52% decline in call light use after implementing hourly rounding and unit was quiet and nurses could spend more time for patient care and logs (Ford, 2010). Intentional rounding researches showed significant reduction in fall rate with one hour rounding. With the implementation of hourly rounding on the stroke and cardiac inpatient unit of the Texas Health Resources Hospital, the purpose is to prevent unwanted events and to improve patient satisfaction and safety.

In this microsystem, there are many indicators that a more efficient workflow is necessary to be established. There is the constant sound of call lights ringing about an estimated 12-15 in an eight hour shift for non-urgent needs. Upon interviewing many staff, I found there was an inconsistent way and lack of education on how hourly rounds should be performed. Basic patient needs were not addressed within each visit from a staff member. By using a process map, timeline, PDSA, Lewin's Change Theory, SWOT analysis and graphs, I can show you my process of improving staff responsiveness within the nursing unit. In conclusion, as a Clinical Nurse Leader and patient advocate, I have utilized the information gained from the microsystem data and implementation of hourly rounding, thereby proving how to improve quality, safety and patient care experiences within the busy, medical surgical nursing unit with the evidence based practice of hourly rounding.

The staff can also benefit by reinforcing the importance of hourly rounding through the yearly house side mandatory review session accessed through the hospital's intranet website. This proposal will include an implementation plan that shows the positive benefits implementing hourly rounding re-education given to nurses and nursing assistants has improved patient outcomes, specifically improved patient safety as seen by a decrease in falls, and improved patient satisfaction. In addition, evaluation of the findings as well as disseminating them to all unit department directors and the nursing community will be discussed.

Book excerpt: The focus of this CNL project is quality improvement using high quality hourly rounds to achieve improved patient satisfaction and communication with nurses. It was determined by reviewing survey results that patient satisfaction and nurse communication is below National levels. Data shows that rounds are not being done well on a medical unit before the improvement project. It is proven that patients receive and perceive higher quality care and communication with increased nurse presence at the bedside. The population of the medical unit has high numbers of confused/dementia, and dual-diagnosis patients. It was determined that the unit would benefit from improved rounding and proof of the actual rounding with education on patient-centered rounds and a bedside checklist to document practice. At the time of implementation, the hospital had a crisis which monopolized all teaching, changes, and focus in order to continue operations. The CNL project was permitted to continue with a limited number of patients and only when the CNL intern was present on the unit. The bedside rounding tool was used with positive results aligned to the evidence used to support the project. Patients were able to indicate that rounds were being done regularly. The nursing relevance of patient-centered rounding is that it supports the significance of caring that takes place at the bedside by fostering trust and accountability. It can be assumed that use of this tool in similar microsystems would achieve positive outcomes similar to these outcomes.

Patient safety and fall prevention are a top priority for nursing. Hourly or purposeful rounding has been suggested as an intervention to combat patient falls. Research shows that hourly rounding reduces patient falls, decreases call light use and helps to improve patient satisfaction scores. A systematic review of literature was conducted to determine the effectiveness of hourly rounding in preventing patient falls as oppose to no rounding or current clinical practices. Fifteen peer-reviewed research articles on the subject were examined, including 13 quantitative or qualitative studies and two literature review studies. Overall, based on the research, hourly rounding helped to reduce patient falls. Therefore, hourly rounding should be incorporated into nursing work flow as an evidence-based best practice.
