Evidence-based practices for pressure ulcer prevention are numerous and easy to find, yet implementation by nursing staff is lagging. When patients develop pressure ulcers, the wound team may be consulted to advise on treatment options. In this study, the wound team implemented methods to prevent pressure ulcers. This research project was conducted in a 96-bed long-term care facility. The project focused on the effectiveness of a wound team in implementing evidence based practices in a long-term care facility and reducing the incidence of pressure ulcers in that facility. This program utilized a multidisciplinary approach to patient care that could easily be transferred to other long-term care facilities. Over a 3-month period the incidence of facility-acquired pressure ulcers was reduced by more than 25%.

This amounts to 25 000 fractures per day or 9 million per year. The financial costs are reported to be: 32 billion EUR per year in Europe and 20 billon USD in the United States. As the population of China ages, the cost of hip fracture care there is likely to reach 1.25 billion USD by 2020 and 265 billion by 2050 (International Osteoporosis Foundation 2016). Consequently, the need for nursing for patients with fragility fracture across the world is immense. Fragility fracture is one of the foremost challenges for health care providers, and the impact of each one of those expected 9 million hip fractures is significant pain, disability, reduced quality of life, loss of independence and decreased life expectancy. There is a need for coordinated, multi-disciplinary models of care for secondary fracture prevention based on the increasing evidence that such models make a difference. There is also a need to promote and facilitate high quality, evidence-based effective care to those who suffer a fragility fracture with a focus on the best outcomes for recovery, rehabilitation and secondary prevention of further fracture. The care community has to understand better the experience of fragility fracture from the perspective of the patient so that direct improvements in care can be based on the perspectives of the users. This book supports these needs by providing a comprehensive approach to nursing practice in fragility fracture care.

Nurses need to adhere to the protocols and rules and regulations in order to provide quality care to their patients as well as their respective families. Education and training for the staff health care providers regarding identifying the risks of pressure ulcers, prevention and treatment should be taken into consideration routinely to keep staff competent and current with evidence-based practice. Education is a vital step in an effective implementation of the prevention of pressure ulcers. As a nurse leader or a coordinator, nurses have an opportunity to educate other staff, patients, and families about the risks having to do with pressure ulcers and how to prevent it.

Never less, non-adherence to prescribed guidelines are often reported, and data obtained from National Nursing-Sensitive Indicators shows that at-risk patients continue to develop hospital acquired pressure ulcers (HAPU) at an unacceptable rate. The Aims of this Capstone project are to initiate a pressure ulcer education and prevention program in a medium-sized, community health care organization, and to change the attitudes and knowledge base of nurses by providing nursing staff with evidence-based knowledge, education, and skills regarding pressure ulcer development and prevention. This project consists of educating nurses on pressure ulcer pathophysiology and the importance of using early evidence-based pressure ulcer prevention education and techniques. Implementation of this project requires approval from key stakeholders. Applying Kurt Lewin's theory of change first requires the recognition that change needs to occur (Cresia and Friburg, 2011). Implementation consists of scheduled staff in-services, monthly meetings, and use of multi-technique pressure ulcer prevention products, risk-assessment, and screening forms. Data collection of pressure ulcer prevalence, for three months prior to implementation and for 3 months post-implementation, will be collected to evaluate the effectiveness of the program. Evaluation of the program will also consist of an evaluation of nursing staff's pre and post-implementation pressure ulcer prevention knowledge and skill. Dissemination of evidence to key stakeholders, health care organizations and to the greater nursing community will occur with the aim to increase knowledge of evidence-based pressure ulcer prevention techniques that will contribute to improved patient outcomes. This project will demonstrate how pressure ulcer prevalence rates, in the hospital-setting can be decreased through the consistent and knowledgeable use of evidence-based pressure ulcer prevention guidelines. PICOT Question: For the at-risk adult hospitalized patient (P), will the implementation of early, evidence-based pressure ulcer prevention interventions by nurses, who are educated in skills and knowledge regarding pressure ulcer development and prevention (I), as compared to those not educated, nor using early evidence-based pressure ulcer prevention interventions (C), decrease the probability of hospital acquired pressure ulcer development (O), in a three-month period (T).

The direct care costs associated with pressure ulcers in LTCF exceed $3.5 billion, while costs related to legal liability are even more substantial. Litigations in LTCF are likely to favor LTCF residents in up to 87% of cases while prevention of pressure ulcers offers high-quality, cost-effective outcomes. A systematic review of healthcare evidence reveals that while the development of pressure ulcers can have a devastating impact of LTCF populations, simple EBP steps such as using supporting surfaces, repositioning the patient, optimizing nutritional status, and moisturizing sacral skin can have a positive impact on reducing physical and emotional pain and associated healthcare costs. Purpose and Scope: The purpose of this Evidence-Based Practice (EBP) project was to describe the level of healthcare knowledge regarding pressue ulcer prevention (PUP) before and after participation in a PUP teaching intervention for LTCF staff. Using descriptive-interventional methodology, the study variable of nursing knowledge regarding pressure ulcer prevention was measured before and after participation in the PUP teaching intervention by the Pressure Ulcer Knowledge Tool. The instrumentation included a 47-item questionnaire with 3 subscales assessing knowledge regarding ulcers, wounds, and prevention/risk. Goal and Objectives: The goal of the project was to successfully implement a PUP teaching intervention to LTCF staff. The objectives were: (a) to describe aggregate demographic characteristics of the LTCF participants, (b) to describe PUP pre-intervention level of knowledge, (c) to describe PUP post-intervention level of knowledge, and (d) to describe PUP level of knowledge before and after LTCF staff participation in a PUP teaching intervention. Plan and Evaluation: Phase One: Conduct a literary needs assessment using a published evidence-based critique template. Phase Two: Design a PUP teaching intervention using the EBP approach. Phase Three: Implement a PUP teaching intervention for selected LTCF staff. Phase Four: Evaluate the level of PUP knowledge before and after LTCF staff participation in the PUP teaching intervention. Ouotcomes and Results: Pre-intervention and post-intervention meaures revealed a statistically significant increase in PUP knowledge from an aggregate percentage performance of .81 at pre-test to an aggregate percentage performance of .98 at post-test. Moreover, scaled reliability estimates based on internal consistency for the full instrument achieved significance with regard to the tool's performance in this interventional design. Although further inquiry is warranted, the findings suggest that LTCF staff can increase PUP knowledge with focused educational intervention regarding the concept, and that PUP education in LTCF may serve as a practice change outcome worthy of further consideration.

Many researches revealed that implementation of Skin Care Bundle can prevent PU from occurring and the writer reiterates the importance of Skin Care Bundle through this paper. The execution of care bundle approach in delivering basic healthcare practice is widely accepted and providing substantial care with positive outcomes for the patient. A care bundle includes few interventions for patient to whom it is applied and this must be based on evidence as to how it will reduce the risk. This paper talked about the strategy for developing and implementing pressure ulcer prevention care bundle plan into practice. The usefulness of this plan can be monitored with an audit tool when it is in use in the practice setting. The occurrence of hospital acquired PU significantly dropped from 12.6% to 2.6% over a period of 6 years of post program because of the implementation of an evidence based PU prevention program (Barker and others, 2012).

Abstract: Older adults are particularly vulnerable to the development of pressure ulcers (PUs) as a result of skin changes and reduced mobility (Knox, Anderson & Anderson, 1994; Russell et al., 2003). This is associated with diminished quality of life, longer hospitalisations and increased morbidity and mortality (Margolis et al., 2002).In view of the high incidence of PUs (grade 1-4) occurring in elderly patients in acute care, a systematic review of related studies was conducted in August 2011. The optimal frequency and methods of repositioning are described in the literature. With well-designed implementation and evaluation plans, the proposed repositioning guidelines are likely to reduce the incidence of PUs (grade 1-4), while in turn lessening the healthcare burden and preserving patients' quality of life. Pilot testing, a trial run to test the feasibility of the innovation, will be carried out in the proposed medical ward, and training provided to all staff before the intervention. To proceed with the change, the intervention must be cost-effective and beneficial to all stakeholders. Outcome evaluation determines the number of goals achieved by the innovation and to what degree, and is very important (Melnyk & Fineout-Overholt, 2005).The PU incidence (grade 1-4) is expected to be different after the implementation of the innovation. Despite a magnitude of research on patient risk factors and evidence-based practice (EBP) interventions, continuity of care remains problematic and suggests that nurses are inconsistently caring for patients at risk for PU formation (Waugh, 2014). In an attempt to better understand the barriers between evidence based nursing care and PU prevention, this author has critically appraised 15 research articles aimed at inspecting nurses' attitudes and knowledge surrounding PU prevention. These articles attempt to discover gaps in care and improve patient outcomes. On the notion that nursing knowledge and attitudes affects implementation of EBP towards PU prevention, this paper proposes to examine the knowledge and attitudes of registered nurses (RNs) at Mercy General Hospital in an effort to heighten awareness surrounding the topic of PU prevention and decrease PU prevalence. Along with presenting current research and describing the proposed problem, enclosed in this proposal is an implementation, evaluation and dissemination plan to help facilitate change and monitor outcomes.

Losing the ability to move freely and independently may put a patient at increased risk of pressure ulcers. The focus of this capstone project is two interventions that have been shown to decrease the occurrence of pressure ulcers and also to develop a plan that can be used in a facility to decrease the number of patients that develop pressure ulcers. A PICO statement was developed that addressed the population, interventions, comparator, and the outcome. From this statement a group of evidenced based, scientific research studies were reviewed and evaluated for their ability to describe the problem of pressure ulcers and support the use of pressure relieving mattresses and good nutrition to prevent pressure ulcers. A theory statement was developed using Kurt Lewin09́s theory of change to support the proposal of creating change within facility to prevent the occurrence of pressure ulcers by using pressure relieving mattresses and ensuring patients received proper nutrition. Using the information found in the study and the current organizations policies and abilities to deal with the problem of pressure ulcers, an implementation plan was developed which shows the process of obtaining approval for the proposed change all the way down to how the change will be initiated in the facility. Following the implementation plan, an evaluation plan was developed to show how the implementation plan would be followed up and ensure that the staff involved were receptive and had the materials and resources needed to continue on with the new plan in place.
