Book excerpt: [Résumé en français] La parole des patients sourds pré- et péri-linguaux subit des déformations importantes qui nuisent à son intelligibilité. La pose d'un implant cochléaire, notamment lorsqu'elle a lieu de façon précoce, permet d'améliorer cette intelligibilité. Néanmoins, le nombre de patients sourds pré et péri-linguaux implantés tardivement - après 7 ans - est en augmentation en France. La principale attente vis-à-vis de l'implant de la part de cette population est l'amélioration de la communication et de l'intelligibilité. Pour savoir si l'implant a un impact sur l'intelligibilité, il faut pouvoir l'évaluer. Evaluer l'intelligibilité nécessite un test qui réponde à une double contrainte : faisabilité et représentativité. Le bilan phonétique, que nous avons conçu à cette fin à partir du bilan articulatoire de Azzarello M. (2003), est composé de 124 mots dans lesquels tous les phonèmes et groupes consonantiques sont présents dans toutes les positions où ils peuvent apparaître dans la langue française. La passation est rapide (moins de 15 minutes), le patient est en effet enregistré en lisant les 124 mots. La notation est simple et rapide : à partir de l'enregistrement, une orthophoniste juge de la justesse des phonèmes ou groupes consonantiques et attribue une note sur 124. Il répond donc à la 1ère exigence de faisabilité. Pour répondre à la 2ème exigence, nous avons étudié sa corrélation avec un test d'intelligibilité dont le protocole est validé par de nombreuses études, chez des sujets implantés tardivement. La corrélation entre les deux tests est bonne (85%).Nous pouvons donc affirmer que le bilan phonétique est un bon outil pour estimer de l'intelligibilité globale des patients sourds pré et péri-linguaux implantés tardivement. Ainsi ce bilan phonétique, administré avant l'implantation puis longitudinalement en post-implantation, pourrait permettre d'objectiver l'évolution de l'intelligibilité chez des patients sourds pré- et péri-linguaux implantés tardivement.
