Book Synopsis The Contribution of Impaired Glucose Metabolism to Cardiovascular Disease and Mortality in Australians by : Elizabeth Barr
Download or read book The Contribution of Impaired Glucose Metabolism to Cardiovascular Disease and Mortality in Australians written by Elizabeth Barr and published by . This book was released on 2009 with total page 466 pages. Available in PDF, EPUB and Kindle. Book excerpt: Diabetes and cardiovascular disease (CVD) together represent one of the greatest public health and economic burdens for the Australian population. There is strong evidence that diabetes increases the risk of CVD and mortality, and emerging evidence from overseas populations indicates that the risk of CVD and mortality extends below the threshold to diagnose diabetes. By analysing data from the Australian Diabetes, Obesity and Lifestyle (AusDiab) study, this thesis explored the relationships between hyperglycaemia and (i) all-cause mortality, (ii) CVD mortality, and (iii) CVD events, including myocardial infarction, stroke, percutaneous transluminal coronary artery angioplasty and coronary artery bypass surgery, and the extent to which these relationships were independent of other traditional CVD risk factors, including insulin sensitivity. The AusDiab study is a national prospective cohort study of 11,247 men and women aged 25 years and over which incorporated a 75 gm oral glucose tolerance test and three indices of blood glucose: fasting plasma glucose (FPG), two-hour post-load plasma glucose (2hPG) and haemoglobin A1c (HbA1c). The updated homeostasis model assessment was also used to calculate an index of insulin sensitivity (HOMA-%S). Data on all-cause and CVD mortality were obtained through data linkage with the National Death Index, and self-reported non-fatal CVD events were verified through medical record adjudication; the methods of which were validated by linking a cohort of AusDiab participants from Western Australia to the Western Australian Hospital Morbidity Database. Participants were followed for a median of five to six years, and the association between glucose metabolism, analysed as both continuous and categorical variables, and mortality and CVD were analysed with Cox proportional regression. The findings confirmed the strong association between previously diagnosed diabetes and all-cause mortality, CVD mortality and coronary heart disease, which was independent of traditional CVD risk factors. The risk for all-cause mortality and fatal or non-fatal myocardial infarction was also significantly increased among those with intermediate hyperglycaemia, although for CVD mortality, stronger associations were observed for impairments in FPG than 2hPG. In those without diagnosed diabetes, a continuous relationship between 2hPG and HbA1c and both all-cause and CVD mortality was observed. For FPG, the relationships were J-shaped, with the risks being increased at both high and low glucose concentrations. FPG was a better predictor of CVD mortality compared with 2hPG or HbA1c, but none of the glucose measures improved CVD risk prediction above that provided by traditional CVD risk factors. There was a continuous relationship between HOMA-%S and fatal or nonfatal CVD events, but this was mainly explained by its association with high-density lipoprotein cholesterol. FPG, but not HOMA-%S, significantly improved the prediction of CVD beyond that of other risk factors. Finally, a stronger relationship was observed between abnormal glucose metabolism and fatal or non-fatal myocardial infarction than with coronary artery revascularisation or stroke. Taken together, the findings of this thesis indicate that abnormal glucose metabolism, including both diabetes and intermediate hyperglycaemia, play an important role in the development of CVD, and suggest that CVD prevention strategies should not only target diabetes, but all levels of hyperglycaemia.

This book was released on 2017-11-17 with total page 948 pages. Available in PDF, EPUB and Kindle. Book excerpt: Cardiovascular, respiratory, and related conditions cause more than 40 percent of all deaths globally, and their substantial burden is rising, particularly in low- and middle-income countries (LMICs). Their burden extends well beyond health effects to include significant economic and societal consequences. Most of these conditions are related, share risk factors, and have common control measures at the clinical, population, and policy levels. Lives can be extended and improved when these diseases are prevented, detected, and managed. This volume summarizes current knowledge and presents evidence-based interventions that are effective, cost-effective, and scalable in LMICs.

This book was released on 2008-01-01 with total page 189 pages. Available in PDF, EPUB and Kindle. Book excerpt: The prevalence of obesity, metabolic syndrome and diabetes - three links of the same 'atherothrombotic chain' - has reached pandemic proportions worldwide. As a result, our civilization is at war against a threatening enemy: cardio-diabetes. Several independent physiological processes underlie the clustering of cardio-diabetes, including central obesity, insulin resistance, dyslipidemia, inflammation, impaired glucose tolerance, and hypertension. Early detection is of overwhelming importance for public health. The complex and intimate relationship between cardiovascular disease and diabetes from basic science to clinical and therapeutic concerns is discussed in this outstanding book. Beginning with molecular, biochemical, inflammatory and cellular aspects, this publication continues with histological and pathophysiological issues, details particular problems in specific metabolic and clinical settings, and finally analyzes several aspects of clinical pharmacology focusing on the optimal management of combined dyslipidemia and non-insulin antidiabetic therapy in cardiac diabetic patients. This book will be a gain in knowledge for every cardiologist, diabetologist, specialist in internal medicine, nutritionalist, general physician and medical student.

Book Synopsis Honolulu Heart Program by : Abraham Kagan
Download or read book Honolulu Heart Program written by Abraham Kagan and published by CRC Press. This book was released on 1996-08-01 with total page 222 pages. Available in PDF, EPUB and Kindle. Book excerpt: The Director of this study, Abraham Kagan, has comprehensively summarized the design and main finndings of the study in this book. The Honolulu Heart Program compared and contrasted ethnic Japanese men living in different cultural environments--Honolulu and mainland Japan--assessed their relative risk factors. The study supported many of the existing views on risk factors but also showed suprising trends. One of the trends shows moderate alcohol consumption is a preventative factor. In recent years the risk factors for cardiovascular diseases have become common knowledge. The recently completed Honolulu Heart Program is the largest targeted study to evaluate scientifically such risk factors.

Book Synopsis International Textbook of Diabetes Mellitus by : R. A. DeFronzo
Download or read book International Textbook of Diabetes Mellitus written by R. A. DeFronzo and published by John Wiley & Sons. This book was released on 2015-03-11 with total page 1200 pages. Available in PDF, EPUB and Kindle. Book excerpt: The International Textbook of Diabetes Mellitus has been a successful, well-respected medical textbook for almost 20 years, over 3 editions. Encyclopaedic and international in scope, the textbook covers all aspects of diabetes ensuring a truly multidisciplinary and global approach. Sections covered include epidemiology, diagnosis, pathogenesis, management and complications of diabetes and public health issues worldwide. It incorporates a vast amount of new data regarding the scientific understanding and clinical management of this disease, with each new edition always reflecting the substantial advances in the field. Whereas other diabetes textbooks are primarily clinical with less focus on the basic science behind diabetes, ITDM's primary philosophy has always been to comprehensively cover the basic science of metabolism, linking this closely to the pathophysiology and clinical aspects of the disease. Edited by four world-famous diabetes specialists, the book is divided into 13 sections, each section edited by a section editor of major international prominence. As well as covering all aspects of diabetes, from epidemiology and pathophysiology to the management of the condition and the complications that arise, this fourth edition also includes two new sections on NAFLD, NASH and non-traditional associations with diabetes, and clinical trial evidence in diabetes. This fourth edition of an internationally recognised textbook will once again provide all those involved in diabetes research and development, as well as diabetes specialists with the most comprehensive scientific reference book on diabetes available.
