L'objectif de cette étude était d'évaluer l'impact de l'implémentation de recommandations nationales 2011 sur les prescriptions antibiotiques pour IORA.Méthode. Une étude multicentrique, quasi-expérimentale de type série chronologique interrompue avec modèle de régression segmentée a été mise en place, incluant sept services d'urgences pédiatriques. Tout enfant consultant entre Novembre 2009 et Octobre 2014 avec un diagnostic d'IORA a été inclus. L'implémentation a consisté en la rédaction de protocoles locaux, des sessions de formations et un retour d'information systématique. Le critère de jugement principal était le taux de prescription d'antibiotiques pour IORA pour 1000 passages aux urgences pédiatriques.Résultats. 242 534 patients avec IORA ont été inclus. L'intervention a été associée à une baisse du taux de prescription d'antibiotiques pour 1 000 passages (-0.4% par période de 15 jours, p=0.04). La baisse cumulée à la fin de l'étude était de 30.9% (IC 95% [-45.2 ; -20.1]), soit 13 136 prescriptions d'antibiotiques évitées. Les prescriptions d'antibiotiques à large spectre ont fortement chuté (62.7% de baisse relative, IC 95% [-92.8; -32.7]). Conclusion. L'implémentation des recommandations nationales 2011 a permis une réduction des prescriptions d'antibiotiques pour IORA aux urgences pédiatriques et une baisse majeure des prescriptions d'antibiotiques à large spectre.

Many of these patients are prescribed antibiotics that are inappropriate based on clinical practice guidelines (CPGs). Over the last 15+ years the Center for Disease Control (CDC) has conducted the Get Smart about Antibiotics (GSA) campaign to reduce over-prescribing to patients with self-limiting ARTIs that are non-indicated for antibiotics. I analyzed the impact of three nationally-focused GSA campaign activities (i.e., publishing CPGs, a national media campaign, and re-publishing material for Spanish-speaking populations) and two state-level GSA campaign activities (i.e., state-level funding and state-level participation in the GSA Week) on antibiotic prescribing in adult patients with office-diagnosed, non-indicated ARTIs (i.e., acute bronchitis, acute pharyngitis, acute rhino-sinusitis, and the common cold or acute non-specific upper respiratory tract infections). The analyses used two primary datasets: (1) Medical Expenditure Panel Survey (MEPS), which is nationally representative, and (2) the MarketScan claims database, which includes patients enrolled in large employer-sponsored health insurance plans. The odds that any antibiotics were prescribed were reduced after the publication of CPGs (MEPS: OR=0.68, p-value=0.01) and after the national media campaign (MEPS: ORR=0.76, p-value=0.01), which was driven by lower antibiotic prescribing to patients diagnosed with the common cold or acute non-specific upper respiratory tract infections. I did not find evidence of code-shifting into antibiotic-indicated ARTIs (i.e., streptococcal pharyngitis and pneumonia) associated with these campaign events. I also found that the odds of antibiotics prescribing were reduced with each additional year of state-level funding (MEPS: ORR=0.96, p-value=0.03; MarketScan: ORR=0.97, p-value

Upper respiratory tract infections (URTIs) include the common cold, laryngitis, pharyngitis/tonsillitis, acute rhinitis, acute rhinosinusitis and acute otitis media. Lower respiratory tract infections (LRTIs) include acute bronchitis, bronchiolitis, pneumonia and tracheitis. Antibiotics are commonly prescribed for RTIs in adults and children in primary care. General practice consultation rates in England and Wales show that a quarter of the population will visit their GP because of an RTI each year. RTIs are the reason for 60% of all antibiotic prescribing in general practice, and this constitutes a significant cost to the NHS. Annual prescribing costs for acute cough alone exceed 15 million. There is evidence from randomised placebo-controlled trials (RCTs) that antibiotics have limited efficacy in treating a large proportion of RTIs in adults and children. These include acute otitis media (AOM), acute cough/acute bronchitis, acute sore throat/acute pharyngitis/acute tonsillitis, acute rhinosinusitis and the common cold. These conditions are largely self-limiting and complications are likely to be rare if antibiotics are withheld. Therefore, these five common RTIs are the focus of this guideline. The inappropriate prescribing of antibiotics has the potential to cause drug-related adverse events, escalate the prevalence of antibiotic-resistant organisms in the community and increase primary care consultation rates for minor illness.
