Book excerpt: There has been a tremendous increase in interest in the neuropathogenicity of viruses during the past decade as we have come to recognize that the human immunodeficiency virus, which causes the acquired immunodeficiency syndrome (AIDS), can infect glial cells and cause neurological disease. Yet this increase has not been limited to AIDS but has extended to viruses that infect either or both the central and peripheral nervous systems. The changes examined here include both neurological and psychological diseases or syndromes. Moreover, the chapters in this volume review the interaction of the host immune system with the viruses examined and how such interactions may increase or decrease the neuropatho genicity of the viruses. Questions regarding viral neuropathogenesis include: (I) What is the mode of transmission of virus to the nervous system? (2) What types of cells are infected, and do they contain receptors for the virus? (3) What is the extent of damage that results from viral infection? (4) What are the immunologic mecha nisms by which damage is mediated or limited? Many of these questions remain unanswered, but this volume delves into efforts to provide some answers.

While current therapies (IVIg and plasma exchange) have yielded some improvements in patients, they are non-specific and are commonly associated with considerable side effects. It is thus imperative to better understand disease pathophysiology and to explore new treatment strategies that target alternate mechanisms. Macrophages are immune cells with important functions in tissue homeostasis, contribute to various pathologies and resolution of diseases. Experimental and clinical investigations have demonstrated that macrophages are the predominant leukocyte population found in diseased peripheral nerves. Thus, it is important to investigate the potential of harnessing macrophage physiology to treat GBS. In this thesis, we aim to understand the pathogenic mechanisms of monocytes and macrophages, in synergy with CD8+ T cells, in the development of autoimmune peripheral neuropathy (APN) in mice.By using B7.2 (L31) transgenic mice and viral infection as animal models of APN, this thesis revealed the presence, requirement, and distinctive contribution of different subsets of nerve macrophages in demyelination and axonal damage in peripheral nerves. In L31 mice, TLR4 and its endogenous ligand HMGB1 were highly expressed, in circulating immune cells and in peripheral nerves. TAK 242, a selective TLR4 inhibitor successfully blocked the enhanced TLR4 signaling, which not only inhibited monocyte, macrophage, and CD8+ T cell activation but also reduced the release of pro-inflammatory cytokines. TAK 242 protected mice from severe myelin and axon loss, resulting in a remarkable improvement in mouse motor and sensory functions. TAK 242 was effective in alleviating the disease in both preventive and reversal paradigms. We also demonstrated that CX3CR1+ monocytes/macrophages and CCR2+ monocytes/macrophages contribute differently to disease onset. CX3CR1, but not CCR2 deficiency prevented inflammatory pathology, nerve structure damage, and preserved normal motor/sensory function in mice. We found that CX3CR1+ resident macrophages have crucial functions in the early phase of disease. The absence of CCR2 signaling resulted in an almost complete abolishment of monocyte egress from bone marrow, and subsequent trafficking to nerve tissues. When there was no blood-derived macrophage recruitment, resident nerve macrophages proliferated robustly, and the increase of macrophages in the nerve was sufficient to drive inflammatory neuropathy and nerve structure damage in L31 mice. Furthermore, our results highlighted the critical contribution of nerve macrophage phagocytosis in APN. Since GBS has been considered to be highly associated with infectious diseases, where cytomegalovirus (CMV) is one of the most frequent pathogens, we investigated the consequence of a systemic infection of murine CMV in naïve mice. We showed that CMV preferentially attacked peripheral nerves, which was associated with a significant macrophage and CD8+ T cell accumulation and activation. However, there was no nervous tissue destruction in the acute phase, and functional disturbance was transient. Further investigation will be needed to understand the long-term consequence of CMV infection on peripheral nerves. Overall, this thesis expands our knowledge on the molecular and cellular mechanisms involved in APN. We proposed a macrophage-centered therapeutic strategy in the treatment of APN. Specifically, blockade of TLR4 and CX3CR1 signaling could offer new therapeutic opportunities in GBS patients"--
