Book excerpt: Epidemiological and experimental evidence supports the concept that chronic inflammation promotes and enhances cancerous growth through several key mechanisms, although these processes are not well understood. Several important mechanisms by which inflammation may initiate and support malignant progression have been previously described, such as the induction of DNA damage, the promotion of angiogenesis and new vasculature, and the production of growth and survival factors. The purpose of this study is to identify a novel mechanism by which chronic inflammation may support and advance tumor progression, through the induction and expansion of immune suppressive mechanisms. We demonstrate that chronic inflammation induces tumor-associated immune-suppression, by enhancing the accumulation of a population of immature myeloid-derived suppressor cells (MDSC), which down regulate and inhibit anti-tumor immunity, allowing for the proliferation and outgrowth of transformed cells. To study the association between inflammation and immune suppression in the context of tumor progression, the 4T1 mammary carcinoma cell line engineered to secrete the pro-inflammatory cytokine interleukin 1 beta (4T1/IL-1beta) was used to create an inflammatory tumor microenvironment. Additionally, IL-1 receptor (IL-1R)-deficient mice, which have a reduced potential for inflammation, and IL-1 receptor antagonist (IL-1Ralpha)-deficient mice, which have an increased potential for inflammation, were used to modulate the inflammatory milieu, and the effects of inflammation on primary and metastatic tumor progression and immune suppression were examined. The presence of IL-1beta in the tumor microenvironment promotes the induction and expansion of a more potent suppressive population of MDSC, thereby enhancing tumor growth and reducing survival.

Book excerpt: "With the rise of immunotherapies as a treatment for various cancers, the need to understand the mechanisms through which cancers evade the immune system is at an all-time high. The transcription factor Signal Transducer and Activator of Transcription 3 (Stat3) is constitutively activated in 35-60% of breast cancer cases and has been linked to immunosuppression. Using a conditional Stat3 knockout, Polyoma virus middle T-driven mouse model, our lab demonstrated that Stat3 deficient mice still develop mammary gland lesions and hyperplasias. However, these lesions are rapidly cleared by a robust immune response headlined by CD8+ cytotoxic T cells and anti-tumor M1 macrophages. This suggests that Stat3 is dispensable for tumor initiation, but is essential for suppressing the immune system and promoting tumor progression.In order to identify the underlying molecular mechanism of Stat3-mediated immunosuppression, we compared the transcriptional profiles of Stat3 wild type (WT) and knockout mice. Preliminary data indicates that the secreted cytokine Chitinase-3-like-protein 1 (Chi3l1, YKL-40 in humans) is the mediator of Stat3-induced immunosuppression. Our study has demonstrated that Chi3l1is a direct Stat3 target that is secreted by hyperplastic epithelial cells early on during cancer progression. It then binds to its receptor on the surface of macrophages, potentially polarizing them to the pro-tumor M2 state, which in turn supports tumor growth and metastasis. Pharmaceutical inhibition of Chi3l1 using a neutralizing antibody shifted macrophage polarization towards the M1 state and increased CD8+ and CD4+ T cell recruitment and activation, mimicking the Stat3 knockout phenotype. We also show that Chi3l1 is expressed in 40% of a Triple negative breast cancer TMA (tissue microarray) and its expression is correlated to inflammation and M2 macrophages. Overall, our results highlight Chi3l1’s immune-suppressive properties and suggest that its inhibition could be a novel immunotherapeutic intervention for breast cancer patients"--

Although several examples of chronic inflammatory conditions, often induced by persistent irritation and/or infection, developing into cancer have been known for some time, there has been a notable resistance to contemplate the possibility that this association may apply in a causative way to other cancers. Examples for such progression from chronic inflammation to cancer are colon carcinoma developing with increased frequency in patients with ulcerative colitis, and the increased incidence of bladder cancer in patients suffering from chronic Schistosoma infection. Inflammation and cancer have been recognized to be linked in another context for many years, i.e., with regards to pathologies resembling chronic lacerations or 'wounds that do not heal.' More recently, the immunology of wound healing has given us clues as to the mechanistic link between inflammation and cancer, in as much as wounds and chronic inflammation turn off local cell-mediated immune responses and switch on growth factor release as well the growth of new blood vessels - angiogenesis. Both of these are features of most types of tumours, which suggest that tumours may require an immunologically shielded milieu and a growth factor-rich environment.

Although regulatory T (Treg) cells induce immune tolerance by suppressing host immune responses against self- or non self-antigens, thus playing critical roles in preventing autoimmune diseases, they might inhibit antitumor immunity and promote tumor growth. Recent studies demonstrate that elevated proportions of Treg cells are present in various types of cancers and suppress antitumor immunity. Furthermore, tumor-specific Treg cells can inhibit immune responses only when they are exposed to antigens presented by tumor cells. Therefore, Treg cells at tumor sites have detrimental effects on immunotherapy directed to cancer.

Scientists can sometimes be pictured as deep sea divers, who plunge into the unknown in search of a hitherto unknown world. Their descent is exciting, but eventually they must come up for air and integrate their experiences with others who also had to resurface. This book intends to collect and, where possible, to collate recent, but sometimes seemingly unrelated information. To quote Stephane Mallarme: "Everything in the world exists to end up in a book". Even if this is a tad cynical, it might not be far from the truth. If a little knowledge is a dangerous commodity, one can also add - tongue in cheek - that a vast amount of knowledge can be truly hazardous. It is likely that what might seem as entangled data is confusing, especially for those satisfied with the comfortable interpretation of Tamoxifen as an antiestrogen which has long been found insufficient. The complexity of its mechanisms and effects defies simple explanations and may even seem capricious, but only because of our ignorance.
