Book excerpt: BackgroundAdvances in dental implant technology, implant surgery techniques and the increasing focus on esthetics have created a shift in treatment outcomes from implant survival to success in the esthetic zone. Immediate implant placement along with immediate provisionalization has already been identified as a reliable technique. The concept of this technique combined with the extraction of a tooth with a periapical pathology in the esthetic area, is a matter of debate. Aim/HypothesisThe aim of this poster is to present a clinical case where immediate placement of a single implant, simultaneously with immediate provisionalization, was performed in a periapically infected central incisor.Materials and MethodsA 42-years-old female patient presented to our dental clinic with pain in tooth 11. After clinical examination, abnormality in gingival level and presence of fistula were observed. The radiographic examination showed a root canal treatment with no lesion, making the diagnosis very demanding. There was a high suspicion of presence of vertical fracture. The treatment plan in order to resolve this case included the extraction of the tooth, immediate implant placement and immediate provisionalization optimizing the final restoration esthetics. After a non-atraumatic tooth extraction and decontamination of the area, a 4x13mm implant (AnyRidge) was placed. The immediate provizionalization was performed using the extracted crown piece, which was adapted to the titanium temporary abutment using a resin cement. Radiographic data were collected right after the implant placement. Amoxicillin 500mg and mouth rinse of chlorhexidine 0,2% were prescribed for 7 and 14 days respectively.ResultsGenerally, healing period was uneventful without any complications and the patient reported mild postoperative pain with no discomfort. The surgery and the treatment time as well as the healing period were reduced. At the regular 6 months follow-up examination the implant was osseointegrated with satisfactory functional and esthetic conditions without any lesion or periapical pathology. The patient reported that the whole procedure was worthwhile. Conclusions and Clinical implicationsIn conclusion, immediate implant placement and provisionalization can successfully be used in periapical infected sites in the esthetic zone. It is crucial to emphasize the fact that successful use of this approach requires advanced clinical experience and surgical judgement. Moreover, there is further need for long-term follow-up studies reporting on implants placed into sites with periapical lesions.
