Book excerpt: Invasion is a crucial component of the complex process of metastasis that marks the transition of breast cancer from local to life threatening disease. The approach we have taken to identify the molecular pathology underlying the onset of invasion, is to apply a combined microdissection and molecular approaches to a unique tissue bank resource. This enables us to isolate mRNA and directly compare gene expression profiles from pathologically defined regions of DCIS and early invasive tumor cells. We have microdissected and identified a number of genes that show differential expression between DCIS and invasive components (in 5 tumors by subtraction hybridization and 8 tumors by membrane filter cDNA array techniques), as well as between low and high grade DCIS. We have also pursued 3 specific genes, previously unexplored in relation to breast cancer, that show patterns of differential expression consistent with a role in the process of invasion - Psoriasin (S100A7), Lumican, (a small leucine-rich proteoglycan) and Angio-Associated Migratory Protein (AAMP). We conclude that our approach is productive in identifying novel genes that are previously unexplored in relation to early breast tumor progression and believe that these may provide markers of progression to invasive disease.

It has been proven to detect breast cancer at an earlier stage and to reduce the number of women dying from the disease. However, it has a number of limitations. These current limitations in early breast cancer detection technology are driving a surge of new technological developments, from modifications of x-ray mammography such as computer programs that can indicate suspicious areas, to newer methods of detection such as magnetic resonance imaging (MRI) or biochemical tests on breast fluids. To explore the merits and drawbacks of these new breast cancer detection techniques, the Institute of Medicine of the National Academy of Sciences convened a committee of experts. During its year of operation, the committee examined the peer-reviewed literature, consulted with other experts in the field, and held two public workshops. In addition to identifying promising new technologies for early detection, the committee explored potential barriers that might prevent the development of new detection methods and their common usage. Such barriers could include lack of funding from agencies that support research and lack of investment in the commercial sector; complicated, inconsistent, or unpredictable federal regulations; inadequate insurance reimbursement; and limited access to or unacceptability of breast cancer detection technology for women and their doctors. Based on the findings of their study, the committee prepared a report entitled Mammography and Beyond: Developing Technology for Early Detection of Breast Cancer, which was published in the spring of 2001. This is a non-technical summary of that report.

These results were obtained by loss of heterozygosity (LOM) studies in breast tumors. Metastasis and poor outcome are the cancer phenotypes associated with lip' 5.5. Since breast cancer patients usually do not die as a result of the primary tumor, but from the progression to a metastatic phenotype, we have embarked on a study to identify% the gene(s) responsible on lip15.5. We have constructed a detailed physical map across the region, participated in sequencing the region, generated a transcript map across the region and have begun to access the genes in this region as candidates for tumor suppressor genes. The candidate region has been reduced to 400 kb and contains about 4-6 potential genes. We have cloned the region and the molecular characterization has begun. Interestingly, the 11p15.5 .5 region has been shown to be located in an imprinted domain. We have evidence to suggest preferential 11pl 5.5 maternal loss in breast tumors suggesting that breast cancer in this region has an imprinting component. We have cloned the homologous imprinted region from the mouse and are testing for imprinted expression in fetal and adult tissues including mammary gland.

Book excerpt: "Over the past decade, genomic technologies have promised to revolutionize breast cancer research with better predictions of disease progression and patient prognosis through the identification of novel molecular targets. These technologies have led to the discovery of numerous different genomic subtyping schemes, each purported to offer relevant information beyond traditional clinical features. Yet it remains unclear what these schemes have contributed to our understanding of breast cancer biology and the reasons for disease recurrence. To address this issue, we have built a novel framework termed Breast Signature Analysis Tool (BreSAT), along with a companion collection of breast cancer datasets, highly annotated signatures, statistics, and visualizations, designed for accurate and ease-of-use application in breast cancer. This framework represents a new way of cataloguing tumors according to their biological properties, as opposed to broad gene expression profiles. We have applied BreSAT and its associated catalogue of signatures to thousands of breast tumor samples, and identify that tumor properties associated with recurrence are confounded by association with other clinicopathological variables such as estrogen receptor status and the genomic subtype. In addition to identifying properties associated with recurrence in breast cancer, we have sought to discover the molecular markers that drive different tumor phenotypes. We identified that expression of the oncogene MET in mouse mammary glands leads to the generation of tumors with characteristics of human triple-negative breast cancer. Additionally, synergy between MET and loss of p53 in similar mouse model leads the development of tumors a claudin-low phenotype, that arise with a higher penetrance and lower latency. Moreover, MET activity is required for maintenance of the claudin-low morphological phenotype and metastatic capacity of cell lines, suggesting that MET may represent an avenue for targeted therapeutics in human patients with claudin-low breast tumors. Finally, we have discovered that molecular features of breast cancer progression from a non-invasive to an invasive state are confounded by tumor subtype. To find more accurate markers of disease progression, we identified tumor properties that differentiate non-invasive tumors from invasive ones within each subtype. We observed that there is little overlap, suggesting that distinct properties drive tumor progression in different subtypes. Furthermore, we were able to identify a small number of non-invasive breast tumors with molecular features that make them more likely to progress. Together, these discoveries are leading to a more comprehensive understanding of the molecular features that drive breast cancer biology, disease progression, and patient outcome." --

Therefore, we would like to initiate discussions on viewing EMT and its downstream signaling networks as a fulcrum of cellular plasticity, and a facilitator of the adaptive responses of cancer cells to distant organ microenvironments and various therapeutic assaults. We hereby invite scientists who have prominently contributed to this field, and whose valuable insights have led to the appreciation of epithelial-mesenchymal plasticity as a more comprehensive mediator of the adaptive response of cancer cells, with huge implications in metastasis, drug resistance, tumor relapse, and patient survival.
