They are, however, not usually classified as being occupationally exposed in the sense of the general ICRP system for radiation protection of workers applied on Earth. The exposure assessment and risk-related approach described in this report is clearly restricted to the special situation in space, and should not be applied to any other exposure situation on Earth. The report describes the terms and methods used to assess the radiation exposure of astronauts, and provides data for the assessment of organ doses.

This applies to all exposure situations (planned, emergency and existing) and all categories of exposure (occupational, medical, and public). Optimisation of protection is intended to reduce exposures to levels that are as low as reasonably achievable, economic and societal considerations being taken into account, and to manage medical exposures commensurate with the medical purpose.

This foundation of knowledge of stem cells is used in the main text of the report to provide a biological insight to issues such as the linear-no-threshold (LNT) model, cancer risk among tissues, dose-rate effects and changes in the risk of radiation carcinogenesis by age at exposure and attained age.

There has been general neglect of radiological protection coverage of fluoroscopy machines used outside the imaging departments. Lack of radiological protection training of those working with fluoroscopy outside imaging departments can increase the radiation risk to workers and patients. Procedures such as endovascular aneurysm repair (EVAR), renal angioplasty, iliac angioplasty, ureteric stent placement, therapeutic endoscopic retrograde cholangio-pancreatography (ERCP) and bile duct stenting and drainage have the potential to impart skin doses exceeding 1 Gy. Although tissue reactions among patients and workers from fluoroscopy procedures have so far been reported only in interventional radiology and cardiology, the level of usage of fluoroscopy outside radiology departments creates potential for such injuries.

Environmental radiation fields of monoenergetic photon and electron sources were firstly computed using the Monte Carlo radiation transport code PHITS for source geometries representing environmental radionuclide exposures including planar sources on and within the ground at different depths (representing radionuclide ground contamination from fall-out or naturally occurring terrestrial sources), volumetric sources in air (representing a radioactive cloud), and uniformly distributed sources in simulated contaminated water.

The incident radiations and energy ranges considered were external beams of mono-energetic photons of 10 keV–10 GeV, electrons and positrons of 50 keV–10 GeV, neutrons of 0.001 eV–10 GeV, protons of 1 MeV–10 GeV, pions (negative/positive) of 1 MeV–200 GeV, muons (negative/positive) of 1 MeV–10 GeV, and helium ions of 1 MeV/u–100 GeV/u. This document updates and extends the earlier publication and includes quantitative guidance on intervention levels. This guidance covers the introduction of such protective actions over very short times, their introduction and continuation following periodic review over protracted timescales lasting perhaps years and intervention over larger areas. All accidents are different, as are the approaches of national organizations having responsibility for response to an accident. It is intended that these general guidelines should be translated into appropriate emergency response plans by competent national authorities.

SAFs are given for uniform distributions of monoenergetic photons, electrons, alpha particles, and fission-spectrum neutrons over a range of relevant energies. Electron SAFs include both their collision and radiative components of energy deposition. SAF data are matched to source and target organs of the biokinetic models of the OIR publication series. The document further outlines the computational methodology and nomenclature for assessment of internal dose in a manner consistent with that used for nuclear medicine applications. Numerical data for particle specific and energy dependent SAFs are given in electronic format for numerical coupling to the respiratory tract, alimentary tract, and systemic biokinetic models of the OIR publication series.
