Book excerpt: "Conventional wisdom suggests aircraft midair collisions to be random events, governed by the laws of Brownian Motion, and best analyzed by stochastic methods. An alternative hypothesis, that such accidents are deterministic in nature, and that specific factors leading to midair collisions can be identified and mitigated, forms the basis for this Dissertation. A predictive model using case control theory is developed for assessing Risk Index, a criterion measure of midair collision likelihood, for any General Aviation flight, actual or hypothetical. Generating the model requires statistical validation of two independent near midair collision databases, and identifying within them those aircraft, aircrew and airspace characteristics most closely associated with collision risk. Calibration of the model shows reality to fall somewhere between the stochastic and deterministic assumptions. A statistically significant correlation is found between predicted and observed Risk Index for a sizable random sample of flights, with a resulting Coefficient of Determination of 0.25. This suggests that we have identified 25% of the source of variance in midair collision risk, the remaining 75% being random. Therefore we can realistically hope to reduce midair collisions by roughly 25%. Strategies for mitigating the identified causal factors are proposed. Measures to reduce the random, remaining 75% of collision risk are also explored. However, these appear to require a significant overhaul of Air Traffic Control procedures, which must be approached with caution, to guard against the attendant possibility of curtailing capacity in the Air Transportation System."--Page 1-2

The aim was to determine the effectiveness of such trails as conspicuity aids under daytime VFR conditions. Analytical review of the literature covers detection of various clouds, dusts, smokes and mists, their capability of indicating flight path, and their possible use with information coding techniques. Technical and operational considerations in using trails are also considered. The evidence provides both favorable and unfavorable indications of the merit of trails as aids for lessening the likelihood of collisions. In good weather and with crossing flight paths, detection range is increased. Technical operational disadvantages are: (a) high cost of installation and maintenance; (b) toxicity and other hazards to aircraft and communities; (c) severe weight requirements; and (d) unreliability of the visible signal. These disadvantages are great enough to preclude recommending a requirement that trails be generated for use in preventing mid-air collision.

Alaska has six times as many pilots per capita and 16 times as many aircraft per capita when compared to the rest of the United States. There are approximately 736 registered landing areas, in addition to the thousands of lakes and gravel bars where pilots land. Hazards can exist on the ground when aircraft are taking off or landing, and also in the air when sharing congested airspace. During 2011 there were three mid-air collisions in Alaska. One of these collisions occurred in the Matanuska-Susitna (Mat Su) Valley between two seaplanes, fatally injuring four people. In response, the Mat Su Mid-Air Collision Avoidance Working Group was formed with representatives from industry, the military, the Federal Aviation Administration (FAA), Aircraft Owners and Pilots Association (AOPA), Alaska Airmen's Association, the Alaskan Aviation Safety Foundation, the Alaska Air Carriers Association and the National Institute for Occupational Safety and Health (NIOSH). Mapping of Common Traffic Advisory Frequencies (CTAFs), which are the frequencies used by pilots to communicate while operating near an airport without a control tower, in the Mat Su Valley showed a substantial amount of overlap; pilots in an area could be on separate, but correct frequencies and therefore unable to communicate with each other. In addition, there were disparities between various sources of radio frequency information, including the FAA's Alaska supplement, flight charts, and individual airport listings. As a partner in the Mat Su Mid-Air Collision Avoidance Working Group, NIOSH analyzed data from FAA and Aviation Safety Reporting System databases and found very little overlap in reports of near misses and near mid-air collisions from Alaska. Alaska pilots were surveyed and locations of high traffic areas in the Mat Su Valley were identified. Radar data from air traffic control was reviewed to determine common traffic routes. Several scenarios were developed to clarify communication confusion; these scenarios were reviewed and evaluated by local pilots, commercial operators, flight instructors and airport operators.--NIOSHTIC-2
