Book excerpt: Despite research consistently demonstrating the benefits of breastfeeding, Native American women in North Central North Dakota have the lowest rate of breastfeeding in North Dakota with a rate of 8.24%. The reasons why these women are not breastfeeding are not known. There have been no studies regarding the attitudes and knowledge about breastfeeding of these women or their health care professionals. The purpose of this study was to assess the knowledge and attitudes about breastfeeding in this population. Thirteen pregnant women, ten women who have given birth, and twelve health care professionals who provide care to these women comprised the sample of this descriptive study. The results of the surveys demonstrated that these women knew the benefits of breastfeeding but the majority of them did not breastfeed their infants. Attitudes seem to be a more relevant factor than knowledge in influencing breastfeeding initiation in this sample of women. Health care providers should be aware that their own attitude toward breastfeeding may affect a woman's choice to breastfeed. Results also demonstrated the health care professionals had limited education about infant feeding, lactation, and breastfeeding. These health care providers may be providing conflicting and possibly incorrect knowledge about breastfeeding. Current evidence based breastfeeding recommendations and practices should be incorporated into continuing education so consistent and correct information is provided. Health systems should establish a baby friendly environment that supports and encourages breastfeeding. In addition, the unique characteristics of the Native American women in North Central North Dakota, or any community in which one lives or works, should be considered to better plan interventions that will be effective and sustainable. It is not one identifiable factor that the decision to breastfeed is dependent upon, but factors that may interact and overlap in ways to influence a women's decision to breastfeed. Understanding context is vital to designing and implementing successful interventions in breastfeeding promotion. Culturally relevant information gathered from this population may not be transferable to others in this particular tribe who may live somewhere else or to other Native American tribes, as their specific cultural attributes may be different from this particular tribe.

Book excerpt: ABSTRACT Infant feeding practices can greatly impact a child's life with far reaching implications based upon decisions a mother makes for her newborn. Breastfeeding is the preferred method of infant feeding because of its association with health benefits to both the mother and child. The prevalence of breastfeeding among rural, low income, disadvantaged groups to include Native American (NA) and African American (AA) adolescent mothers is low. NAs and AAs bear a disproportionate burden of illness and health risks compared to other races and ethnic groups in the United States. Breastfeeding is a targeted public health strategy to improve the health of infants and children and decrease or eliminate the gap of health disparities among the maternal-child aggregate populations in our nation. The purpose of the study was to (1) test the Theory of Planned Behavior by examining the relationship of the components (attitudes, subjective norms, perceived control/ self-efficacy, and breastfeeding knowledge) to determine breastfeeding intention and initiation of rural, low-income Native American (NA) and African American (AA) adolescent mothers in rural communities in southeastern North Carolina; (2) determine the significant similarities and differences between breastfeeding and formula feeding adolescent groups related to the constructs; (3) explore the relationship of intention to initiation, and (4) examine the relationship between sociodemographic variables with breastfeeding intention and initiation of adolescent mothers. The sample included 60 NA and 60 AA adolescents recruited through resources of care that were offered to prenatal clients in two public health departments. Findings included the constructs of the TPB were significant in predicting the probability of breastfeeding intention and breastfeeding initiation in rural, low-income NA and AA adolescent mothers. Overall total variance explained by the model related to the prediction of breastfeeding initiation at 4 days postpartum was 73% and correctly classified 92.2% of cases. Three control measures were found to be statistically significant, with Perceived Breastfeeding Control having the highest significance, followed by Negative Breastfeeding Sentiments and Social Professional Support. The model revealed a statistical significance in the relationship of breastfeeding intention and breastfeeding initiation at 4 days postpartum in rural, low-income NA and AA adolescent mothers. The strongest predictor of breastfeeding initiation was the Breastfeeding Control Scale. The Formula Feeding Group scored significantly higher on the Negative Breastfeeding Sentiments scale, whereas the Breastfeeding Group scored significantly higher on the Breastfeeding Control Scale and the Positive Breastfeeding Sentiment Scale. Significant differences were found related to ethnic groups, with NA adolescents initiating breastfeeding more often, being married more often, and living with a significant other more often than AAs. More adolescents who attended childbirth classes and breastfeeding classes breastfed than those who did not. Recommendations of the study included the utilization of the constructs of the TPB in development of research based assessments and interventions that promote breastfeeding behavior among rural, low-income NA and AA adolescent mothers, and identification of at-risk individuals for not breastfeeding and promoting programs to assist with providing help for resolving barriers to breastfeeding, and nurse educators emphasizing the importance of promoting breastfeeding among adolescents and vulnerable populations

This study used a convenience sample and participants were recruited at prenatal classes from three different geographical locations in North Carolina. A Prenatal Data Collection Tool was used to gather demographic data and information about intended feeding patterns. The Iowa Infant Feeding Attitude Scale was used to measure the variable of infant feeding attitudes. In addition to the instruments used for this study, an open ended question about culture was included as part of the survey Participants were contacted by the researcher after discharge by phone to determine breastfeeding initiation. Results suggest that race was not a significant variable and there is no difference in infant feeding attitude scores between African American and Caucasian women in this study. There was also no difference in the relationship between decision to breastfeed and breastfeeding initiation in African American and Caucasian women. Infant Feeding Attitude was found to be a predictor of decision to breastfeed and breastfeeding initiation. Study findings will be used to assist healthcare providers in understanding the relationship between infant feeding attitude and breastfeeding practices that may affect the disparity that exist in the breastfeeding practices of African American women."--Abstract from author supplied metadata.

Book excerpt: Despite the widely recognized health benefits of breastfeeding and its endorsement by leading health organizations as a preventative public health intervention, inadequate breastfeeding knowledge and lactation management skills among healthcare providers continues to be a major barrier for women who choose to initiate breastfeeding. Breastfeeding competencies are not standardized in healthcare education for any of the health professions. To address this gap, a few continuing education and professional development programs have been implemented, but paucity in research regarding the efficacy of these programs exists. The purpose of this study was to explore the changes in healthcare providers learning outcomes related to breastfeeding support and promotion. A non-experimental pre-posttest self-report survey design was used to assess the feasibility and preliminary efficacy of an online breastfeeding educational intervention for healthcare providers. The Theory of Planned Behavior (TPB) provided the framework for exploring the participants' psychological and behavioral outcomes. The research questions were: (1) What is the feasibility of an online breastfeeding course for healthcare providers? (2) What are healthcare providers' psychological and behavioral changes occurring after completion of an online course? (3) How do the post-intervention psychological and behavioral outcomes of the online format compare with those of the previous format (hybrid) of this breastfeeding course? Although participants favorably assessed the feasibility (i.e., acceptability) of the 45-hour course, several factors contributed to participants' satisfaction level: Previous online learning experience, connectedness with others, and the degree of structural support. Significant positive changes occurring in participants were increases in their knowledge and beliefs about breastfeeding; attitudes toward formula feeding; perceived behavioral control; perceptions about being able to perform breastfeeding supportive behaviors; and intentions to perform actions that are consistent with evidence-based breastfeeding supportive behaviors. Significant changes in the beliefs about formula feeding were not in the expected direction raising conceptual and pedagogical issues. Participants had negative perceptions about being able to implement what they learned in their workplaces or to affect policy. Findings support the use of online breastfeeding education programs for healthcare providers; changes at both individual and institutional levels are necessary to change provider practices.
