Peu d'études ont évalué la filière d'admission directe et celle des urgences. L'objectif principal était de comparer les caractéristiques des patients en fonction de leur mode d'entrée directe (ED) ou par les urgences (EU). L'objectif secondaire était de déterminer la concordance entre le diagnostic d'entrée et le diagnostic final. Méthode : Étude prospective bi centrique effectuée du 15 février au 25 juillet 2013 incluant des patients de plus de 75 ans adressés par leur médecin traitant. Les données ont été recueillies par questionnaire et analyse des courriers de sortie. L'analyse des diagnostics, regroupés en 33 items, a été réalisée séparément par les deux investigateurs principaux Résultats : 308 patients ont été inclus, 135 en ED et 173 en EU. Les deux groupes étaient globalement comparables sur les caractéristiques socio démographiques. Les ED bénéficiaient d'une hospitalisation moins longue et retournaient plus à leur lieu de vie antérieur. Les motifs « infectieux » et « chutes » prédominaient dans le groupe EU. Les « altérations de l'état général » et « troubles cognitifs » prédominaient dans le groupe ED. La concordance diagnostique est plus forte chez les patients ED. Conclusion : L'étude démontre l'intérêt de privilégier l'admission directe chez les plus de 75 ans. La collaboration ville-hôpital doit être encouragée pour optimiser le parcours de soin des personnes âgées.
