Book excerpt: It is commonly asserted by health service commentators that concentrating services into larger, centralized units will result in improved health outcomes. This report by the NHS Centre for Reviews and Dissemination describes how a systematic review of research was carried out in order to examine the evidence for a relationship between the volume of activity of a hospital and patient outcomes. Whilst a number of procedures were looked at, researchers carried out a detailed examination of coronary artery bypass graft (open heart) surgery. Whilst most studies report a positive relationship between hospital volume and outcome for several elective procedures the review found that the quality of the studies was poor. The size of the effect of volume on outcomes is inflated due to poor adjustment for the influence of differences in case-mix between high and low volume hospitals. Hospitals carrying out more procedures tend to admit patients who are less severely ill and hence have better outcomes. There is little evidence to suggest that merging hospitals to create larger units will result in improved outcomes over time.
