Book excerpt: Pursuant to a congressional request, GAO reviewed the potential benefits of and impediments to the use of automatic data processing (ADP) systems in managing medical patient care records. GAO found that automated medical records could: (1) greatly improve patient care management due to their quick accessibility, improved accuracy, higher quality, increased versatility, and decisionmaking and quality assurance support; and (2) enhance outcomes research and increase hospital efficiency by improving staff productivity and reducing operating costs. GAO also found that many health professionals did not fully use automated medical records because: (1) they perceived such systems as difficult to operate and foreign to their practice of medicine, although this could be alleviated by emerging technology and adequate training; (2) the necessary technology for completely automating those records seemed too costly or was not yet fully developed; (3) of undeveloped standards for automating those records and lack of agreement within the medical community; and (4) of unaddressed security and privacy concerns and their potential legal implications.

Book excerpt: The Composite Health Care System (CHCS) is a state-of-the-art, integrated medical information system the Department of Defense is considering for its 767 medical treatment facilities worldwide. Our objectives were to (1) determine whether Defense will be able to test and evaluate CHCS adequately before its planned procurement/deployment decision, (2) identify Defense's latest cost and funding estimates, and (3) evaluate the reasonableness of Defense's projected benefits. We visited Fort Knox, which serves as Defense's test site for new software development, and 6 of 11 OT & E sites; met with Defense and contractor representatives; and reviewed government and contractor documents. We conducted our evaluation from July 1989 to March 1990, in accordance with generally accepted government auditing standards. We have concerns as to whether Defense will be able to realize the projected CHAMPUS benefits. While CHCS may allow facilities to treat more patients, current CHAMPUS regulations allow beneficiaries to get outpatient care from civilian hospitals and physicians without Defense's approval and regardless of whether the military facility has excess capacity. GAO found that: (1) DOD completed deployment of CHCS to 526 medical treatment facilities worldwide, which was difficult because of the system's complexity and the number of sites involved; (2) two DOD officials ensured the deployment's success by providing leadership and using fundamental information management practices; (3) DOD expects CHCS benefits to exceed its costs by $1.3 billion over the system's expected life; (4) CHCS should improve scheduling, give greater and quicker access to patient information, and increase the timeliness of medical care; (5) DOD has made progress in addressing its two previously identified problems by developing a prototype clinically oriented graphical user interface to make patient order-entry less cumbersome and strengthening the tools and methodology needed to manage CHCS performance; (6) DOD has updated its CHCS performance management plan and developed performance simulation models for each CHCS computer platform; (7) the lack of an effective plan for rapidly repairing or replacing CHCS equipment damaged by disaster remains a problem; and (8) DOD did not address this problem because of cost concerns and a lack of accurate information, but it is reconsidering its options for providing equipment adequate backup.

Book excerpt: Our purpose in conducting this study was to gain additional information about integrated hospital information systems in non-federal hospitals to be used in evaluating the Department of Defense's (DOD) and Veterans Administration's (VA) efforts to develop and implement systems for federal hospitals. We used two methods to derive the information presented in this study--a review of literature and a survey of vendors and their client hospitals. Many publications we reviewed characterized hospital information systems as being less developed than analogous systems supporting other major industries but did not present convincing quantitative data to support these characterizations. Accordingly, we conducted a series of literature searches to identify possible reasons for delays for obstacles in developing integrated hospital information systems. In addition, to acquire independent quantitative data about the most sophisticated and comprehensive commercial systems, we developed and administered a series of data collection instruments at seven of the largest vendors and client hospitals using their most technically sophisticated and comprehensive integrated hospital information systems. We also administered these instruments at the two largest hospital chains and at one hospital which had developed its own system.
