Each in-service packet takes approximately one hour to complete and fully meets the Medicare in-service training requirements. As aides need training, you can make as many copies as you want - there's no restriction when used with aides assigned from your office location. Remember that Home Health Aides must have 12 hours of in-service training every year. LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: Name three diseases and injuries that often lead to hip and knee replacement Name three serious complications that must be reported to a supervisor immediately Describe three special requirements involved with caring for patients who have had joint replacement OVERVIEW Each year, nearly a quarter of a million elderly people suffer a hip fracture. Women, especially those who suffer from brittle bones related to osteoporosis, are twice as likely as men to experience a hip fracture. Often the result is loss of independence, even death. It takes a surgical procedure to repair the fracture, and the type of surgery is determined by the location of the fracture. Other factors such as the patient's age, health status, bone structure, and level of mobility also affect the decision. An alternative to this surgery is a complete hip replacement, called hip arthroplasty. This has become one of the most commonly performed and successful orthopedic operations. Successful recovery from joint replacement requires care after surgery to promote pain-free function and rehabilitation. The home health aide plays an important role in assuring the patient's recovery by understanding joint replacement surgery and knowing how to care for a patient's special needs. This in-service lesson looks at two of the most common joint replacement surgeries and discusses how the home health aide plays an important role in caring for those patients.

LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: Name two common types of knee surgery List three reasons a patient might require knee surgery Name three components of the knee joint, and List two observations the aide should report to the supervisor. OVERVIEW In the United States, the single most common reason for visiting orthopedic surgeons is because of a knee problem. It is not surprising that more joint replacement surgeries are performed on the knee than on any other joint. Surgery on the knee is a complex procedure usually followed by a prolonged rehabilitation period. Many patients require home health services following surgery and often need home health aides to assist with activities of daily living (ADLs) in the recovery period. The purpose of this in-service is to provide information about knee surgery.

LESSON OBJECTIVES After completion of this program, the home health aide will be able to: * Name two different types of arthritis * List three factors that contribute to arthritis pain, and * Describe three ways home health aides can help patients with arthritis. LESSON OVERVIEW According to the Centers for Disease Control (CDC), arthritis affects about one of every three adults, making it one of the most prevalent diseases in the United States. It is the nation's leading cause of disability. Homecare patients are very likely to have arthritis, and even when it may not the primary diagnosis for homecare, it is often the reason the patient requires a home health aide. Therefore it is very important for home health aides to understand this condition, and to know how to care for arthritic patients. The purpose of this in-service is to educate aides about arthritis and to provide information about the care of affected patients.

LESSON OBJECTIVES Upon completion of this program, the home health aide will be able to: * Name two common types of hip surgery *List three activities patients with hip surgery should not perform *Name three assistive devices most hip surgery patients need, and * List two occasions in which the aide should notify the supervisor. OVERVIEW The American Academy of Orthopaedic Surgeons reports that there are more than 300,000 hip fractures each year in the United States. It is estimated that women over the age of 65 have a 1 in 5 chance of fracturing their hips at some point during their lives. In addition to hip fractures, several other conditions necessitate hip surgery, including degenerative joint disease, trauma, osteonecrosis, and various types of inflammatory arthritis. Many patients require home health care following hip surgery. Because home health aides are so often assigned to care for these patients, it is essential that they understand the components of care, precautions to take, and when to contact supervisors.

LESSON OBJECTIVES After completion of this program, the home health aide will be able to: * Describe two major functions of the kidneys * Name the two leading causes of kidney failure * Identify two observations home health aides should report, and * List two treatment options for patients with end-stage renal disease. LESSON OVERVIEW About 7.4 million adults in the United States have physiological evidence of kidney disease. Nearly 100,000 of them are diagnosed with renal failure each year. Diabetes mellitus is the most common cause of renal failure, with more than 40% of new cases attributable to diabetes. Hypertension causes more than 15,000 new cases of kidney failure each year, making it the second most common cause. Both diabetes and hypertension are common among homecare patients so it is not surprising that home health aides are often assigned to patients with renal failure. The purpose of this in-service is to educate home health aides about renal disease and to provide information about caring for affected patients.

LESSON OBJECTIVES After completion of this program, the home health aide will be able to: * Describe two reasons why homecare surveys are performed * List three primary methods for surveying home health aide services * Explain the responsibilities of the home health aide during home visits LESSON OVERVIEW The survey of homecare agencies is an anxious time for all concerned. It is particularly troublesome for home health aides since they often do not understand how the survey is conducted and what role they are expected to play. Failure to meet the standard for home health aide services can result in a Condition-level deficiency for the agency. During a survey, home visits will be made to observe home health aide care delivery. Aide documentation and employee files will be reviewed. The topic of the survey is an important one for home health aides since they will always be directly and indirectly involved in the process. The information presented in this program will help to explain the process, as well as outline the responsibilities of the home health aide during a survey.

LESSON OBJECTIVES After completion of this program, the home health aide will be able to: * Define the term "asepsis" * Identify at least four occasions when gloves must be worn, and * Name three different types of personal protective equipment. LESSON OVERVIEW Information about bloodborne pathogens and infectious disease is provided to all home health aides during orientation and on a regular basis afterward. The sessions include a lot of information; use of personal protective equipment (PPE) is only one component. Home health aides must be proactive in obtaining and carrying necessary PPE. As a result, they are not always compliant with using PPE when it's called for. When they fail to use PPE appropriately every single time, they are putting themselves and others at risk of infection. The purpose of this in-service program is to emphasize the importance of PPE. It includes indications for use and a discussion of the possible negative outcomes for failing to be fully compliant.

LESSON OVERVIEW The Conditions of Participation (COP) require that written patient care instructions be prepared for home health aides. Some agencies call these written instructions the aide's care plan, while other agencies use such terms as assignment sheet or instruction sheet. [This in-service will use "assignment sheet" for consistency.] The standards regarding home health aide assignments are among the most common deficiency citations home care agencies receive. The purpose of this in-service is to emphasize the importance of the assignment sheet as well as to provide guidelines to follow when tasks on the assignment sheet cannot be completed. Additionally it will emphasize the role of the home health aide in providing input for updating the assignment sheet.

Each in-service packet takes approximately one hour to complete and fully meets the Medicare in-service training requirements. As aides need training, you can make as many copies as you want - there's no restriction when used with aides assigned from your office location. Remember that Home Health Aides must have 12 hours of in-service training every year. LESSON OBJECTIVES After completion of this program, the home health aide will be able to: *Define multiple sclerosis (MS) as a chronic progressive disease affecting the central nervous system * List three common symptoms of MS * Identify two findings home health aides should observe and report, and *Point out that MS is usually a disease with remissions and relapses. LESSON OVERVIEW About 400,000 people in the United States have multiple sclerosis. The symptoms vary greatly from person to person and from time to time in one person. When symptoms are present, they often reduce the person's ability to perform activities of daily living (ADLs). During relapses, many patients with MS require assistance from home health aides. For that reason it is important that aides understand the basics of the disease as well as their role in providing care.
