Book excerpt: The purpose of this cross-sectional survey study is to examine the frequency and severity of physical, sexual and emotional intimate partner violence (IPV) experienced by women before and after diagnosis of HIV/AIDS. This research also examines the effect of social support on the IPV experienced by these women. During the fall of 2011, women living with HIV/AIDS and receiving services from AIDS Outreach Center located in the southwestern part of the United States were recruited to participate in a study through the purposive sampling method. They were asked to complete surveys about their HIV/AIDS diagnosis, disclosure, level of IPV experienced, and their level of social support. Sixty-four women completed the survey. The majority (51.6%) of the research participants were African-Americans with a mean age of 46. Many women in the study were separated/divorced (34.4%); 29.7 % were married and 12.5% were cohabiting. Almost all the women (93.2%) had disclosed their HIV status to their current intimate partner and majority (71.9%) of the women's partners were HIV negative. The majority of the research participant reported experiencing all forms of IPV before their diagnosis of HIV/AIDS. Most women (71%) reported experiencing emotional abuse, 53% reported physical abuse, and 52% reported sexual abuse. Significantly more women reported experiencing IPV after their diagnosis of HIV/AIDS (71% emotional abuse, 61% physical and 53% sexual abuse). There was a statistically significant difference [t (63) = -2.57, p = 0.013] in the frequency of physical IPV experienced by women before (M=18.34, SD= 26.79) and after (M=26.68, SD=34.06) the diagnosis of HIV/AIDS. Similarly, there was a statistically significant difference [t (63) = -2.08, p = 0.041] in frequency of sexual IPV experienced by women before (M=5.43, SD= 6.41) and after (M=6.92, SD=7.98) the diagnosis of HIV/AIDS. The severity of all types of IPV experienced by women also significantly increased following HIV/AIDS diagnosis. The severity of physical violence [t (63) = -2.76, p = 0.008] sexual violence [t (63) = -2.34, p = 0.022] and emotional violence [t (63) = -2.54, p = 0.014] significantly increased following HIV/AIDS diagnosis. Evidence suggests that women who have been subjected to violence seek health care more often than non-abused women, even if they do not disclose the associated violence. They also identify health-care providers as the professionals they would most trust with disclosure of abuse. These guidelines are an unprecedented effort to equip healthcare providers with evidence-based guidance as to how to respond to intimate partner violence and sexual violence against women. They also provide advice for policy makers, encouraging better coordination and funding of services, and greater attention to responding to sexual violence and partner violence within training programmes for health care providers. The guidelines are based on systematic reviews of the evidence, and cover: 1. identification and clinical care for intimate partner violence 2. clinical care for sexual assault 3. training relating to intimate partner violence and sexual assault against women 4. policy and programmatic approaches to delivering services 5. mandatory reporting of intimate partner violence. The guidelines aim to raise awareness of violence against women among health-care providers and policy-makers, so that they better understand the need for an appropriate health-sector response. They provide standards that can form the basis for national guidelines, and for integrating these issues into health-care provider education.

Book excerpt: Intimate partner violence (IPV) and HIV treatment intersect as synergistic vulnerabilities for women. This study examines how women in two regions of Uganda live with and respond to both issues in the context of globalization, including the perspectives of policy makers, service providers, and HIV positive women. Women in Uganda have few formal means of dealing with IPV, and even where services are available, they may choose not to seek help or disrupt their relationships. Most women who seek help do so informally, through family or local and religious leaders. Providers also have limited ability to assist women because of a lack of resources and the limited legal sanctions available. Legal sanctions for responding to women's needs are in transition, but currently limited in their effectiveness. All of the women in this study are already living with HIV and enrolled in treatment. Those who face or have experienced violence are faced with another set of challenges. IPV frequently occurs in conjunction with economic difficulties, alcohol use, and polygamous marital practices within a home. Women only have legal rights for maintenance in formal marriages, but many are in informal or polygamous partnerships. Women in this study are part of an emerging era of HIV, in which HIV can be lived with as a chronic disease. Globalization impacts their daily experiences through the availability of life-saving treatment. In this context women's concerns are frequently outside the purview of health. They have a previously unknown luxury of being able to "not worry" on a daily basis about maintaining their health. Economic concerns are of the utmost importance and when women are deprived of resources from their partners in acts of economic violence they are more likely to seek assistance than when experiencing physical or sexual violence. This work contributes to the anthropological literature on globalization and health, HIV, and IPV. Women's experiences of both HIV and intimate partner violence in the Ugandan context are mitigated by global processes, local politics, service provision, cultural ideologies and interpersonal relationships.

Book excerpt: Intimate partner violence (IPV), is behavior within an intimate relationship that causes physical, sexual, or emotional harm. It is the most common form of gender based violence (GBV) and is a major public health problem, with a global and African prevalence of 30% and 37%, respectively. This was a cross-sectional analysis that enrolled HIV-seropositive women in serodiscordant relationships who were at least 18 years of age and provided a written informed consent to participate in the study. Participants were asked 13 questions adapted from the World Health Organization survey on violence against women about physical, sexual, or emotional violence in the past year, by the current partner. Correlates of IPV were assessed using standardized instruments. To estimate the association between correlates and past year IPV, univariate and multivariate logistic regression were used. Overall, 47/159 (29.6%, 95% CI 22.9-37.2%) reported experiencing IPV in the past year. In the multivariate analysis, pregnancy (aOR 9.78, 1.98-48.26), male partner controlling behavior (aOR 3.18, 1.44-7.03), past physical violence by someone other than the current partner (aOR 3.78, 1.32-10.78) and alcohol use (minimal aOR 3.78, 1.45- 9.87, moderate/high aOR 1.12, 0.20-6.14), were associated with higher likelihood of reporting IPV in the past year. Programs caring for HIV-positive women in serodiscordant relationships should consider screening these women for IPV. Additionally, the study identified a potential period of risk (pregnancy), an identifiable risk factor (previous violence), and potentially modifiable behaviors of women and their partners (alcohol use and partner controlling behavior) that were associated with a greater likelihood of IPV in the past year.

Too often this preventable form of violence is repetitive in nature, occurring at multiple points across the lifespan. The prevalence of intimate partner violence is on the higher end of this spectrum in East Africa, with in-country demographic and health surveys indicating that approximately half of all women between the ages of 15-49 in Uganda, Kenya, and Tanzania having experienced physical or sexual abuse within a partnership. It is now widely accepted that preventing intimate partner violence is possible and can be achieved through a greater understanding of the problem; its risk and protective factors; and effective evidence-informed primary, secondary, and tertiary prevention. To that end, on August 11-12, 2014, the Institute of Medicine's Forum on Global Violence Prevention, in a collaborative partnership with the Uganda National Academy of Sciences, convened a workshop focused on informing and creating synergies within a diverse community of researchers, health workers, and decision makers committed to promoting intimate partner violence-prevention efforts that are innovative, evidence-based, and crosscutting. This workshop brought together a variety of stakeholders and community workers from Uganda, Kenya, and Tanzania to engage in a meaningful, multidirectional dialogue regarding intimate partner violence in the region. Book excerpt: Intimate partner violence (IPV) is a serious social problem affecting both men and women worldwide that can lead to a variety of negative mental and physical health effects. This book contains nine chapters that address the problem of IPV, exploring methods of preventing IPV as well as treatment for victims of IPV. Chapter One centres on the issue of blame, reviewing current research on the associations between self-blaming attributions and psychological outcomes among survivors of IPV. Chapter Two proposes a theoretical reflection on the phenomenon of domestic violence based on its understanding as a problem sustained by cultural beliefs and discourses and which can be tackled through education and the promotion of public debate, by means of institutional advertisements. Chapter Three highlights the importance of capacity building Brazilian police forces to tackle cases of IPV as well as to protect and enable victims to fully exercise their rights. Chapter Four argues that the Domestic Violence Risk instrument used by Portuguese police to assess IPV cases should be reworked, as it tends to assign a medium level of risk in cases that the scientific community would likely associate with high risk. Chapter Five discusses the relationship between IPV and academic stress. Chapter Six reviews research on factors affecting women's treatment engagement in the aftermath of IPV, including characteristics of the violence/violent relationship, types of mental health problems following IPV, and individual differences in personality and demographics. Chapter Seven deals with the coping mechanisms available to women living with HIV/AIDS in Tanzania who experience IPV. Chapter Eight details the relevance of methodological and ethical aspects of studies on violence involving children and how these requirements may affect research validity in this domain. Book excerpt: Among the health disparities affecting the U.S. Hispanic population today are those relating to risky behaviors such as substance abuse, intimate partner violence (IPV) and HIV/AIDS. However, few studies have examined how these conditions may impact this population. The purpose of this dissertation was to explore the experiences that Hispanic women in South Florida have with regard to substance abuse, IPV and risks for HIV/AIDS, to describe how these conditions may be related, and to develop a model that can be used to guide research and interventions targeting this population. This dissertation uses data collected in Project DYVA (Drogas y Violencia en las Americas- Drugs and Violence in the Americas), a pilot research study that utilized both qualitative (Phase I) and quantitative (Phase II) research methods to describe the experiences of Hispanic women in South Florida between the ages of 18 and 60 with regard to substance abuse, violence and risky sexual behaviors. Three studies were conducted as part of this dissertation. The first study utilizes data collected during the qualitative phase of Project DYVA. During this phase eight focus groups were conducted and analyzed using qualitative content analysis (N = 81). The second and third studies utilize data collected during the second phase of Project DYVA. In this phase cross-sectional questionnaires collecting information regarding demographics, acculturation, self-esteem, depression, substance abuse, IPV and risks for HIV, were administered to 82 Hispanic women. Univariate and multivariate statistics were used to explore the relationships between substance abuse, IPV and risk for HIV (study 2) and between resource availability, IPV and depression (study 3). The findings from this dissertation suggest that substance abuse, IPV and risk for HIV are closely related intersecting health issues. IPV, the condition that emerged as the most salient of the three, also appears to be closely associated with resource availability (i.e., self-esteem and income) and depression. Additional individual, cultural, relationship and socio-environmental factors that may play a significant role in shaping the experiences that Hispanic women have with regards to these intersecting conditions were also identified and organized into a model.
