Book excerpt: Human immunodeficiency virus (HIV) is a lentivirus that causes acquired immunodeficiency syndrome (AIDS). The virus either directly eliminates infected cells, indirectly causes the apoptosis of unifected cells, or significantly decreases the surviving cells' ability to fight infection. It is this multifactorial mechanism of action that causes the severe immunosuppression and subsequent opportunistic inffection that is pathognomonic for HIV/AIDS. It is also the reason HIV is able to affect all bodily systems, including the eye. Ocular manifestations of HIV infected patients can range in severity from mild blepharitis to vision threatening cytomegalovirus retinitis. However, if detected at an early stage, advancements in AIDS' therapeutic management have made prevention of vision loss very likely. This is evident in the fact that industrialized countries (Such as America) are experiencing a significant decrease in the prevalence of HIV-related vision loss among patients who receive prompt treatment with highly active antiretroviral therapy.

Book excerpt: STUDY OF OCULAR MANIFESTATIONS IN HIV/AIDS CHILDREN IN HIGHLY ACTIVE ANTI RETROVIRAL THERAPY (HAART) ERA FROM BIKANER, NORTHWESTERN INDIAAbstract: Background: Human Immune deficiency Virus (HIV) has the capability to affect every organ system in the body. Ocular manifestations reflect systemic disease and may be the first sign of disseminated infection. This prospective study was executed to determine the spectrum of ocular manifestations in HIV positive children in Highly Active Anti Retroviral Therapy [HAART] era and to establish their correlation with CD4+Tcells counts and stage of the disease.Method: This study enrolled 300 HIV positive children from February 2015 to December 2017. These children were evaluated, irrespective of the presence or absence of ocular symptoms and their treatment status. The ocular examination protocol included Visual Acuity [VA] Testing, color vision testing, ocular motility assessment, eye lid and adnexal examination, complete anterior and posterior segment examination. Results: Ocular lesions were observed in 50.3% (151/300) children. Conjuctival microvasculopathy being the commonest finding seen in 9% (27/300) children followed by retinal microvasculopathy [7.3% (22/300)] and trichomegaly [6.3% (19/300)]. Herpes Zoster Ophthalmicus and Cytomegalo virus retinitis were observed in 1.33% and 1% children respectively. In this study 78.33% (235/300) children were on HAART. At the time of inclusion in the study, CD4+cell counts ranged from 9 to 1838 cells/mm3 [meanu00b1SD=375.6u00b1249 cells/mm3]. In the study, 28 children had CD4+cell counts u2264100/mm3, in whom 82.14% had ocular findings, while 71 children had CD4+cell count u2265500/mm3 in whom only 35.21% children had ocular findings. The association between low CD4+cell counts and ocular findings was statistically significant [pu22640.0001]. Conclusion: The pattern and severity of ocular manifestations was different than previous reports. Probably HAART has changed the pattern and severity of HIV related eye disease. Low CD4+cell count and advanced stage of disease are the strong predictors of ocular involvement.

Unlike any other book on the market, this book is specifically designed for ease of use and can cater to a variety of medical professionals and their needs. The text features brief cases that allow for quick readability, an appendix particularly designed for cross-referencing cases with common symptoms, exposures, and putative diagnoses, bulleted conclusion points, and differential diagnoses tables. Each case is written by an expert in the field and includes a discussion that leads the reader through the logical process of deduction to narrow the diagnosis as well as the laboratory testing, physical examination findings, and elements of the patient’s history and exposures utilized to make a diagnosis. Chapters conclude with a focused review on a specific topic related to diagnosis, treatment, or prognosis that the case illustrates, including references for further reading on the topics from the literature. The Infectious Disease Diagnosis is an outstanding resource for infectious disease specialists, internal medicine physicians, emergency room staff, primary care and general practice physicians, family practitioners, consultants in infectious disease, medical students, residents, fellows, and trainees who diagnose patients.

Book excerpt: As the AIDS epidemic continues to grow and patients live longer, it will be increasingly challenging for physicians to treat the many opportunistic infections that accompany AIDS. Eye doctors must sometimes make an initial diagnosis of AIDS, but most of the time they are asked to treat the ocular manifestations of AIDS-related infections. The challenge that many ophthalmologists and optometrists face is composed of three kinds of identifications. They must recognize and treat possible sight-threatening conditions, they must be able to determine unusual displays of known organisms; and they must be able to find manifestations that have never been described or have not been thought to occur in patients with AIDS. Clinical and autopsy studies show that up to 75% of patients with AIDS will have ocular findings. All parts of the visual system can be affected by AIDS. Ocular involvement, it is appropiated for primary and secondary care eye doctors to be knowledgeable concerning this condition. This review is intended to describe the most common ophthalmic manifestations of HIV infection.

Remaking a Life uses the HIV/AIDS epidemic as a lens to understand how women generate radical improvements in their social well being in the face of social stigma and economic disadvantage. Drawing on interviews with nationally recognized AIDS activists as well as over one hundred Chicago-based women living with HIV/AIDS, Celeste Watkins-Hayes takes readers on an uplifting journey through women’s transformative projects, a multidimensional process in which women shift their approach to their physical, social, economic, and political survival, thereby changing their viewpoint of “dying from” AIDS to “living with” it. With an eye towards improving the lives of women, Remaking a Life provides techniques to encourage private, nonprofit, and government agencies to successfully collaborate, and shares policy ideas with the hope of alleviating the injuries of inequality faced by those living with HIV/AIDS everyday.
