O'ROURKE One of the principal problems of hypertension is the precise definition of blood pressure as a cardiovascular risk factor. Clinicians indicate peak systolic pressure and end diastolic pressure in the brachial artery as the principal criteria for blood pressure measurement. Consequently, these values are as indicators for clinical management and therapeutic adjustment. This used methodology, based on indirect blood pressure measurements at the site of the brachial artery relates only to the highest and lowest pressure in that vessel, and does not give any information of the blood pressure curve itself; this carries more information than peak systolic pressure and end diastolic pressure. As a first step in better analysis of the blood pressure curve, research workers in experimental hypertension defined in addition to peak systolic pressure and end diastolic, another blood pressure value, mean arterial pressure, i. e. the average pressure throughout the cardiac cycle, and about which pressure fluctuates. This is the pressure recorded by Hales [1] and by Poiseuille [2] in their pioneering studies. By application of Poiseuille's Law, this definition of mean arterial pressure led to the concept that increased mean arterial pressure (and therefore hypertension) was related, at any given value of cardiac output, to an increase in vascular resistance, i. e. to a reduc tion in the caliber of the small arteries.

Public and even physician interest in the subject remained minimal until the early forties, and even then there were not more than a dozen "fulltime" investigators worldwide. The first organization devoted to communicating regularly the results of hy pertension research was created in Cleveland in 1945; it became the Council for High Blood Pressure Research of the American Heart Association in 1949. The early history of hypertension is not a story of progressive achievement. Richard Bright is given great credit, followed by Mahomed, Allbutt, Riva-Rocci, Bergman and Janeway, but it was not until Volhard, Fahr and Allen that some semblance of order emerged. They were followed by a younger, much more vigorous group of investigators who in fact initiated modern research in hypertension. Two more generations have fol lowed. In the course of some fifty years, we have seen emerge a magnificent body of evidence that has prescribed good treatment and contributed a considerable understanding of the many complex mechanisms involved in the hypertensions. Treatment is now actively pro moted by public health agencies, including the World Health Organization. This is an other case of treatment preceding full understanding of the nature of the disease.

This progress has made a major contribution to health care concerns such as quality of life, prevention of disability, and mortality. In the past, hypertension and hypertensive disease had been a "silent scourge," but it is presently an industry. Research on hypertension has expanded into a variety of fields including epidemiology, endocrinology, surgery, pharmacology, and behavioral medicine. Therapeutic accomplishments have made hypertension a leading source of income for the pharmaceutical industry; the field of clinical pharmacology originated with the development of drugs to treat hypertension. Increasingly, specific drugs to treat specific mechanisms which raise blood pressure have moved from the laboratory to the bedside. A constant awareness has been present that emotional stress, both from within the individual as well as from environmental sources, plays a role in the "three Ps" -- predisposition, precipitation, and perpetuation -- of hypertension. Arguments range from stating that such stress may be the major cause of at least some forms of hypertension, to allowing that although some effect is present from stress, it is only a minor perturbation of no significance in the overall pattern of the disease. Advocates of stress theory may be biased by a lack of detailed knowledge or experience with the physiology and biochemistry involved in the establishment of this disorder. On the other hand, those who deny the importance of stress factors may be unaware of the large body of data that indicate the role of these factors in any comprehensive understanding of hypertension. Following the Mosaic Theory, this book's approach to hypertension shows that multiple factors can be invoked in understanding the etiology and management of hypertension, where the strength of individual factors vary depending on genetic background, acquired diseases, and environmental influences. Stress can be involved in predisposition by affecting a genetically programmed person, in precipitation by supplying the stimulus to bring the disease to a clinical level, and in perpetuation by maintaining or exacerbating the clinical disease. This volume attempts to integrate what is known about the effects of stress on blood pressure with the overall mosaic of hypertension making use of the aforementioned "three Ps" as part of the framework for this integration.

It is esti mated that there are six to eight million hypertensive patients in the Federal Re public of Germany today. This corresponds with an incidence of 10%-15% in the whole population. Approximately between three and four million show organic eardiac manifestations of hypertension. Of all deaths occurring before the age of 65,30%-40% are attributable to hypertension, and the total morta lity from hypertension is about 20%-25%. Recent epidemiological and clini cal studies have revealed similar figures for other countries, such as the United States and Japan. Hypertensive hypertrophy and cardiac failure are found to be many times more frequent than eardiae hypertrophy and failure of any other origin. Despite the apparent clinical and social importance of the biologieal system "blood pressu re and the heart", data on eardiac performance, on systemic and coronary haemodynamics and on the morphological, bioehemical and contractile proper ti es of ventrieular heart muscle in both experimental and clinical hypertension are rare. This is the reason and stimulus for having organized this symposium, which is the first one coneeming this topie.

Book excerpt: The hemodynamic mechanisms of hypertension are often limited to the study of three dominant parameters: blood pressure, cardiac output and vascular resis tance. Accordingly, the development of hypertension is usually analyzed in terms of a 'struggle' between cardiac output and vascular resistance, resulting in the classical pattern of normal cardiac output and increased vascular resistance, thus indicating a reduction in the caliber of small arteries. However, during the past years, the clinical management of hypertension has largely modified these simple views. While an adequate control of blood pressure may be obtained with antihypertensive drugs, arterial complications may occur, involving mainly the coronary circulation and suggesting that several parts of the cardiovascular system are altered in hypertension. Indeed, disturbances in the arterial and the venous system had already been noticed in animal hypertension. The basic assumption in this book is that the overall cardiovascular system is involved in the mechanisms of the elevated blood pressure in patients with hypertension: not only the heart and small arteries, but also the large arteries and the venous system. For that reason, the following points are emphasized. First, the cardiovascular system in hypertension must be studied not only in terms of steady flow but also by taking into account the pulsatile components of the heart and the arterial systems. Second, arterial and venous compliances are altered in hypertension and probably reflect intrinsic alterations of the vascular wall.

A major problem in comparing blood pressure values from different centers is the standardization of the measurement. Complete standardization will never be achieved if one takes into account the numerous factors that may influence blood pressure in epidemiological studies. Whether blood pressures are measured by doctors or by technicians, are recorded at home, in the working place or in a hospital, in sitting, standing or supine position, and is taken during the same examination-all of these whether a blood sample factors can influence blood pressure measurement. But meals, time of day (blood pressure being higher in the evening), heart rate, cuff size, stethoscope used, digit preference, month of year, temperature, etc., can equally influence the measurement. Home reading of arterial blood pressure at standardized times is probably the best answer to all of these problems and has been used with gratifying results in a comparative study between Belgium and Korea.
