Book excerpt: The underlying technology and the range of test parameters available are evolving rapidly. The primary advantage of POCT is the convenience of performing the test close to the patient and the speed at which test results can be obtained, compared to sending a sample to a laboratory and waiting for results to be returned. Thus, a series of clinical applications are possible that can shorten the time for clinical decision-making about additional testing or therapy, as delays are no longer caused by preparation of clinical samples, transport, and central laboratory analysis. Tests in a POC format can now be found for many medical disciplines including endocrinology/diabetes, cardiology, nephrology, critical care, fertility, hematology/coagulation, infectious disease and microbiology, and general health screening. Point-of-care testing (POCT) enables health care personnel to perform clinical laboratory testing near the patient. The idea of conventional and POCT laboratory services presiding within a hospital seems contradictory; yet, they are, in fact, complementary: together POCT and central laboratory are important for the optimal functioning of diagnostic processes. They complement each other, provided that a dedicated POCT coordination integrates the quality assurance of POCT into the overall quality management system of the central laboratory. The motivation of the third edition of the POCT book from Luppa/Junker, which is now also available in English, is to explore and describe clinically relevant analytical techniques, organizational concepts for application and future perspectives of POCT. From descriptions of the opportunities that POCT can provide to the limitations that clinician’s must be cautioned about, this book provides an overview of the many aspects that challenge those who choose to implement POCT. Technologies, clinical applications, networking issues and quality regulations are described as well as a survey of future technologies that are on the future horizon. The editors have spent considerable efforts to update the book in general and to highlight the latest developments, e.g., novel POCT applications of nucleic acid testing for the rapid identification of infectious agents. Of particular note is also that a cross-country comparison of POCT quality rules is being described by a team of international experts in this field.

The role of governments in respect of public policy in reuse is discussed as well as the different international criteria and standards for reusing wastewater. The importance of public acceptance and the way to properly handle it is also considered. The fourth section of the book presents contrasting case studies; typical situations in the developed world (Japan and Germany) are compared to those in developing countries (Pakistan and Brazil) for agricultural and industrial reuse. Indirect planned reuse for human consumption (Germany) is compared with an unplanned one (Mexico). The Windhoek, Namibia case study is presented to emphasize why if the direct reuse of wastewater for human consumption has been performed with success for more than 35 years it is still the only example of this type around the world. To illustrate the difficulties of having a common framework for regulating water reuse in several countries, the Mediterranean situation is described. Other case studies presented refer to the reuse situation in Israel, Spain, Cameroon, Nepal and Vietnam, these latter countries being located in water rich areas. This book will be an invaluable information source for all those concerned with water reuse including water utility managers, wastewater policy makers and water resources planners as well as researchers and students in environmental engineering, water resources planning and sanitary engineering. Clinicians indicate peak systolic pressure and end diastolic pressure in the brachial artery as the principal criteria for blood pressure measurement. Consequently, these values are as indicators for clinical management and therapeutic adjustment. This used methodology, based on indirect blood pressure measurements at the site of the brachial artery relates only to the highest and lowest pressure in that vessel, and does not give any information of the blood pressure curve itself; this carries more information than peak systolic pressure and end diastolic pressure. As a first step in better analysis of the blood pressure curve, research workers in experimental hypertension defined in addition to peak systolic pressure and end diastolic, another blood pressure value, mean arterial pressure, i. e. the average pressure throughout the cardiac cycle, and about which pressure fluctuates. This is the pressure recorded by Hales [1] and by Poiseuille [2] in their pioneering studies. By application of Poiseuille's Law, this definition of mean arterial pressure led to the concept that increased mean arterial pressure (and therefore hypertension) was related, at any given value of cardiac output, to an increase in vascular resistance, i. e. to a reduc tion in the caliber of the small arteries.

Fourier transform methodology revolutionized signal acquisition capabilities. Superconducting magnets enhanced sensitivity and produced considerable improvement in spectral dispersion. NMR imaging promises to offer a noninvasive alternative to X rays. High resolution is now achievable with solids, through magic angle spinning and cross polarization, so that the powers of NMR are applicable to previously intractable materials such as polymers, coal, and other geochemicals. The ease of obtaining relaxation times brought an important fourth variable, after the chemical shift, the coupling constant, and the rate constant, to the examination of structural and kinetic problems i~ all fields. Software development, particularly in the area of pulse sequences, created a host of useful tech niques, including difference decoupling and difference nuclear Overhauser effect spectra, multidimensional displays, signal enhancement (INEPT), coupling constant analysis for connectivity (INADEQUATE), and observation of specific structural classes such as only quaternary carbons. Finally, hardware development gave us access to the entire Periodic Table, to the particular advan tage of the inorganic and organometallic chemist. At the NATO Advanced Study Institute at Stirling, Scotland, the participants endeavored to examine all these advances, except imaging, from a multidisciplinary point of view.
