Book excerpt: A. Alberta Centre for Health ServicesUtilizationResearch ACKNOWLEDGEMENT The authors gratefully acknowledge the support provided by the Alberta Centre for Health Services Utilization Research, which conducts studies of how health services are used and delivered to inform health policy development and management of health service delivery. [...] The cost of health services during the year prior to surgery and after surgery was then contrasted to the length of the waiting period. [...] The Alberta Centre for Health Services Utilization Research Page 2 Health Service Costs While on the Waitlist Outcomes Physician claims and emergency room claims during the year before the procedure, the year after the procedure and the difference between the two years. [...] In order to asses the association of wait with physician claims restricted to the actual wait period, we subtracted the waiting time physician claims from physician claims in a identical postoperative time period matched for each patient. [...] No attempt is made to balance the effectiveness of the specific intervention for one particular patient compared to providing another type of service to someone else 9. Focus on the target of wait time may divert attention from the quality, effectiveness, and appropriateness of the health service10.

Book excerpt: According to Transforming Health Care Scheduling and Access, long waits for treatment are a function of the disjointed manner in which most health systems have evolved to accommodate the needs and the desires of doctors and administrators, rather than those of patients. The result is a health care system that deploys its most valuable resource-highly trained personnel-inefficiently, leading to an unnecessary imbalance between the demand for appointments and the supply of open appointments. This study makes the case that by using the techniques of systems engineering, new approaches to management, and increased patient and family involvement, the current health care system can move forward to one with greater focus on the preferences of patients to provide convenient, efficient, and excellent health care without the need for costly investment. Transforming Health Care Scheduling and Access identifies best practices for making significant improvements in access and system-level change. This report makes recommendations for principles and practices to improve access by promoting efficient scheduling. This study will be a valuable resource for practitioners to progress toward a more patient-focused "How can we help you today?" culture.

These accounts, focusing on real experiences and real patients, transcend the rhetoric of political debate about health care delivery. The cases offer lessons for how we might draw on the virtues of other health care systems, understand strengths and shortcomings in our current system, and work toward potential improvements. All royalties derived from the sale of this book are contributed to the Harvard Macy Institute in support of the worldwide community of health care professionals innovating through education.

Available in PDF, EPUB and Kindle. Book excerpt: Many Americans believe that people who lack health insurance somehow get the care they really need. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Available in PDF, EPUB and Kindle. Book excerpt: Americans are accustomed to anecdotal evidence of the health care crisis. Yet, personal or local stories do not provide a comprehensive nationwide picture of our access to health care. Now, this book offers the long-awaited health equivalent of national economic indicators. This useful volume defines a set of national objectives and identifies indicatorsâ€"measures of utilization and outcomeâ€"that can "sense" when and where problems occur in accessing specific health care services. Using the indicators, the committee presents significant conclusions about the situation today, examining the relationships between access to care and factors such as income, race, ethnic origin, and location. The committee offers recommendations to federal, state, and local agencies for improving data collection and monitoring. This highly readable and well-organized volume will be essential for policymakers, public health officials, insurance companies, hospitals, physicians and nurses, and interested individuals.

Performance Measurement offers a comprehensive review of available measures and introduces a new framework to examine these measures against the six aims of the health care system: health care should be safe, effective, patient-centered, timely, efficient, and equitable. This new book also addresses the gaps in performance measurement and introduces the need for measures that are longitudinal, comprehensive, population-based, and patient-centered. This book is directed toward all concerned with improving the quality and performance of the nation's health care system in its multiple dimensions and in both the public and private sectors.
