Book excerpt: The focus on national health care reform has given rise to a number of proposals for revamping the health insurance marketplace so that all Americans would have affordable coverage. One option supported by President Obama, Senate Finance Committee Chairman Max Baucus and other health policy leaders involves the government establishing a public health insurance exchange to effectively organize an insurance market for those without coverage. Their theory is that a national health insurance exchange would provide coordination and guidance to insurance markets to help them comply with consumer protections and compete in cost-efficient ways that would result in more Americans obtaining coverage. Advocates of a public exchange say it could also help purchasers and insurers address some of the problems that currently exist in private health insurance markets, such as insufficient risk spreading, discrimination, out of control costs, poor delivery of subsidies, troubles facilitating and ensuring enrollment, and underinsurance. These fundamental challenges have long hindered the efficacy of our nationâ€TMs health care system while contributing to the growing numbers of Americans who either have no insurance or insurance that is inadequate to meet their current and potential health care needs. In this paper, researchers from the Urban Institute and Georgetown University review some of the key problems facing purchasers of insuranceâ€"whether they be individuals or employersâ€"and outline whether and how a public health insurance exchange might address them. The paper also highlights lessons that can be learned from the experience of prior efforts to create and operate exchanges, such as the Commonwealth Connector in Massachusetts. The authors conclude a well-designed exchange can help bring about changes that can move the system toward many of the nationâ€TMs most oft-stated health reform goals. Without an exchange, the authors assert that a patchwork of new agencies at the state and federal levelâ€"in addition to new roles for existing agenciesâ€"would be required to achieve similar reform objectives.

Some states had difficulties with the rollout and operation of their marketplaces, and some states that struggled with IT implementation are now using the federal marketplace IT platform. GAO was requested to review CMS's and states' actions to implement the marketplaces. This report (1) describes CMS's actions to assist states that have chosen to transition to a different marketplace IT platform and identify costs and challenges those states incurred in making this transition; (2) assesses CMS's actions taken to assist selected states to ensure that the development and operations of marketplace IT systems can be financially self-sustained; and (3) assesses CMS's steps to monitor the performance of the states' marketplace IT systems. GAO reviewed documentation from CMS and four states selected based on different types of marketplaces, federal grants provided, and enrollment numbers, and interviewed CMS and the states' officials.

It is not clear what impact these changes will have on medical and public health preparedness programs around the country. Although there has been tremendous progress since 2005 and Hurricane Katrina, there is still a long way to go to ensure the health security of the Country. There is a commonly held notion that preparedness is separate and distinct from everyday operations, and that it only affects emergency departments. But time and time again, catastrophic events challenge the entire health care system, from acute care and emergency medical services down to the public health and community clinic level, and the lack of preparedness of one part of the system places preventable stress on other components. The implementation of the ACA provides the opportunity to consider how to incorporate preparedness into all aspects of the health care system. The Impacts of the Affordable Care Act on Preparedness Resources and Programs is the summary of a workshop convened by the Institute of Medicine's Forum on Medical and Public Health Preparedness for Catastrophic Events in November 2013 to discuss how changes to the health system as a result of the ACA might impact medical and public health preparedness programs across the nation. This report discusses challenges and benefits of the Affordable Care Act to disaster preparedness and response efforts around the country and considers how changes to payment and reimbursement models will present opportunities and challenges to strengthen disaster preparedness and response capacities.

States have some flexibility with respect to exchanges by choosing to establish and operate an exchange themselves (ie: state-based), or by ceding this authority to HHS (ie: federally facilitated). States may also choose to enter into a partnership with HHS whereby HHS establishes the exchange and the state assists with operating various functions. According to HHS, 18 states will establish a state-based exchange, while 25 will have a federally facilitated exchange. Seven states will partner with HHS. This book examines the states' responsibilities for establishing exchanges, the actions selected states have taken, and the challenges they have encountered.

This brief includes data for states that are implementing their own Marketplaces (also known as State-Based Marketplaces or SBMs), and states with Marketplaces that are supported by or fully run by the Department of Health and Human Services (including those run in partnership with states, also known as the Federally-facilitated Marketplace or FFM). This brief also includes some preliminary data on the characteristics of persons who have selected a Marketplace plan (by gender, age, and financial assistance status), and of the plans that they have selected (by metal level). Cumulative enrollment-related activity during the first three months (10-1-13 to 12-28-13) of the initial open enrollment period is reported for several metrics, including: the number of visits to the Marketplace websites, the number of calls to the Marketplace call centers, the number of completed applications submitted to the Marketplaces, the number of eligibility determinations processed by the Marketplaces for enrollment in a Marketplace plan (used throughout this report to refer to a Qualified Health Plan or QHP), the number of persons who have been determined or assessed eligible by the Marketplaces for Medicaid or the Children's Health Insurance Program (CHIP),1 and the number of persons who have selected a plan through the Marketplace. Data related to Medicaid and CHIP eligibility in this report are based on applications submitted through the Marketplaces. October and November data based on applications submitted through state Medicaid/CHIP agencies were released by the Centers for Medicare & Medicaid Services in a separate report, “Medicaid & CHIP: November Monthly Applications and Eligibility Determinations Report, December 20, 2013,” which can be accessed at http://www.medicaid.gov/AffordableCareAct/Medicaid-Moving-Forward-2014/Downloads/Medicaid-CHIP-Monthly-Enrollment-Report-Nov-2013.pdf. Comparable December 2013 enrollment data based on applications submitted through state Medicaid/CHIP agencies will be released in a subsequent report. This report features cumulative data for the three-month period because some people apply, shop, and select a plan across monthly reporting periods. We believe that these cumulative data provide the best “snapshot” of Marketplace enrollment-related activity to date. Ongoing efforts are underway to eliminate duplication associated with counting people in more than one month. Future monthly enrollment reports during the initial open enrollment period will continue to provide updated cumulative data. The cumulative number of individuals that have selected a Marketplace plan between 10-1-13 and 12-28-13 (including those who have paid a premium and those who have not yet paid a premium) is nearly 2.2 million.
