The Medicare Access and CHIP Reauthorization Act of 2015 includes a provision for GAO to examine the use of quality measures across HHS programs and private payers, with a focus on reducing burden. In this report, GAO examined (1) what is known about the extent and effects, if any, of quality measure misalignment; (2) key factors that can contribute to misalignment; and (3) HHS's efforts to address any misalignment. GAO conducted a literature review to identify related studies; reviewed HHS documents; and interviewed HHS officials and experts from 16 organizations that represent a range of perspectives, including providers and payers. The scope of work for this project is to use the nine aims for improvement of quality in public health (population-centered, equitable, proactive, health promoting, risk reducing, vigilant, transparent, effective, and efficient) as a framework to identify quality measures for the Healthy People Leading Health Indicators (LHIs). The committee reviewed existing literature on the 12 LHI topics and the 26 Leading Health Indicators. Quality measures for the LHIs that are aligned with the nine aims for improvement of quality in public health will be identified. When appropriate, alignments with the six Priority Areas for Improvement of Quality in Public Health will be noted in the Committee's report. Toward Quality Measures for Population Health and the Leading Health Indicators also address data reporting and analytical capacities that must be available to capture the measures and for demonstrating the value of the measures to improving population health. Toward Quality Measures for Population Health and the Leading Health Indicators provides recommendations for how the measures can be used across sectors of the public health and health care systems. The six priority areas (also known as drivers) are population health metrics and information technology; evidence-based practices, research, and evaluation; systems thinking; sustainability and stewardship; policy; and workforce and education.

After five years of producing the NHQR and NHDR, AHRQ asked the IOM for guidance on how to improve the next generation of reports. The IOM concludes that the NHQR and NHDR can be improved in ways that would make them more influential in promoting change in the health care system. In addition to being sources of data on past trends, the national healthcare reports can provide more detailed insights into current performance, establish the value of closing gaps in quality and equity, and project the time required to bridge those gaps at the current pace of improvement.

The NHQR is built on 179 measures assembled across four dimensions of quality—effectiveness, patient safety, timeliness, and patient centeredness. This year's report focuses on the state of health care quality for a group of 46 “core” report measures which represent the most important and scientifically credible measures of quality for the Nation, as selected by the Department of Health and Human Services (HHS) Interagency Work Group. The distillation of 46 core measures for the 2005 report provides a more readily understandable summary and explanation of the key results derived from the data. Also included in the report are four new composite measures, which summarize data from a collection of individual measures. Composite measures were created for heart attack, heart failure, pneumonia, and patient centered care, in addition to an overall measure of the state of health care quality improvement. Four themes that emerge from the 2005 NHQR extend the meaning of those from the 2003 and 2004 reports and add new dimensions on understanding change over time: Health care quality continues to improve at a modest pace across most measures of quality; Health care quality improvement is variable, with notable areas of high performance; Health care quality is improving, but more remains to be done to achieve optimal quality; Sustained rates of quality improvement are possible.

The NHQR examines and tracks the quality of health care in the United States, using the most scientifically credible measures and data sources available. Measures of health care quality address the extent to which providers and hospitals deliver evidence-based care for specific services as well as the outcomes of the care provided. The measures are organized around four dimensions of quality—effectiveness, patient safety, timeliness, and patient centeredness—and cover four stages of care—staying healthy, getting better, living with illness or disability, and coping with the end of life. The NHQR is complemented by its companion report, the National Healthcare Disparities Report (NHDR), a comprehensive national overview of disparities in access to and quality of health care among racial, ethnic, and socioeconomic groups, as well as among subpopulations such as children and the elderly. Both reports measure health care quality and track changes over time but with different orientations. The NHQR addresses the current state of health care quality and the opportunities for improvement for all Americans as a whole. This perspective is useful for identifying where the Nation is doing well and where more work is needed. The NHDR addresses the distribution of improvements in health care quality and access across the different populations that make up America. This perspective is useful for ensuring that all Americans benefit from improvements in care. Both reports' perspectives are needed for a complete understanding of quality of healthcare, and both reports support HHS Secretary Mike Leavitt's 500-Day Plan to fulfill the President's vision of a healthier America, specifically in the areas of better transparency of health care quality information and eliminating inequities in health care. The NHQR comprises 211 measures. This large measure set is distilled to 42 core measures which are the major focus of the 2006 report; of these, 40 have data for 2 or more years. The measures are balanced across the four dimensions of quality and provide a more readily understandable summary and explanation of the key results derived from the data. Major additions to the core measures have been made this year. Among them are three new measures on prevention, including advice from health care professionals on eating, exercise, and vision care, and two new composite measures for patient safety, including measures on postoperative complications and adverse events. Also, new measures were added to the overall measure set in the areas of asthma, hospice care, and patient centeredness in hospitals. The Highlights section offers a concise overview of findings from the 2006 NHQR. Four themes emerge from the 2006 NHQR: Most measures of quality are improving, but the pace of change remains modest; Quality improvement varies by setting and phase of care; The rate of improvement accelerated for some measures while a few continued to show Deterioration; Variation in health care quality remains high.

Care can be delivered too late or without full consideration of a patient's preferences and values. Many times, our system of health care distributes services inefficiently and unevenly across populations. Each year since 2003, the Agency for Healthcare Research and Quality (AHRQ), together with its partners in the Department of Health and Human Services (HHS), has reported on progress and opportunities for improving health care quality, as mandated by the U.S. Congress. The information amassed for the National Healthcare Quality Report (NHQR) since its inception is a growing knowledge base that addresses two critically important questions: What is the status of health care quality in the United States? How is the quality of the health care delivered to Americans changing over time? The significance of tracking this sector's performance is evident from many vantage points. More than $2 trillion is spent each year on health care in the United States, and costs are escalating relentlessly, threatening the financial security of families and businesses. Quality and value are increasingly considered in decisions made by patients and payers. To help patients choose doctors and hospitals prudently, tools have been produced that gather information about and rate providers. To motivate providers to deliver high-quality care, purchasers are starting to reward superior performance and refusing to pay for additional care needed to correct medical errors. This is a dynamic area that will likely continue to develop and affect ways in which care is selected, delivered, and paid for. Monitoring the success of these efforts is crucial as stakeholders refine their quality improvement activities and reassure Americans that they are receiving the optimal health care they deserve. The NHQR is built on 220 measures categorized across four dimensions of quality: effectiveness, patient safety, timeliness, and patient centeredness. Guided by a subcommittee of AHRQ's National Advisory Council and an HHS Interagency Work Group, i this year's report focuses on the state of health care quality for a group of 45 core report measures that represent the most important and scientifically credible measures of quality for the Nation. By focusing on 45 core measures, the 2008 report provides a more readily understandable summary and explanation of the key results derived from the data.ii While the measures selected for inclusion in the NHQR are derived from the most current scientific knowledge, this knowledge base is not evenly distributed across the dimensions of health care quality. For example, there are many measures on the effectiveness of care for heart attacks, but few measures of medication safety or end-of-life care. The analysis in the following pages centers on measures for which data are available and fit within a framework provided by the Institute of Medicine. Three themes from the 2008 NHQR emphasize the need to accelerate progress if the Nation is to achieve high-quality health care. These themes also reflect the challenges that still remain: Health care quality is suboptimal and continues to improve at a slow pace; Reporting of hospital quality is leading improvement, but patient safety is lagging; Health care quality measurement is evolving, but much work remains.
