The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Book excerpt: This quality improvement (QI) dissertation is a case study of the homeless-oriented Patient Centered Medical Home demonstration program, referred to as the Homeless Patient Aligned Care Team (HPACT) at West Los Angeles VA Medical Center (WLA). The WLA HPACT program was implemented to address the complex needs of the homeless and at-risk for homelessness population using the Patient Centered Medical Home model (PCMH) - a multi-disciplinary, team-based approach to primary care. Unlike traditional PCMH models, the WLA HPACT demonstration program employed a series of complex care management interventions to tailor care for homeless Veteran patients. The primary goal of the WLA HPACT demonstration program was to decrease the number of emergency department (ED) visits among patients assigned to the WLA HPACT team by identifying tools and processes to perform complex care management for the WLA HPACT panel of patients. This dissertation identifies barriers and best practices to address needs of WLA HPACT patients to support the demonstration program's goal of decreasing their ED utilization. Tis problem-solving QI dissertation was carried out in three phases - literature review, diagnosis, and implementation. In the first phase, we conducted a literature review of the factors contributing to homeless Veteran ED utilization patterns. In the diagnosis phase, we undertook a local needs assessment (conducted by informal stakeholder focus groups) to determine the organizational context for the WLA HPACT program and how to best evaluate HPACT's objective to reduce ED utilization rates for HPACT patients. The implementation phase of this QI dissertation employed a mixed-method progress-focused formative evaluation to examine patient characteristics, intervention processes, ED utilization patterns, staff perceptions on perceived barriers and interventions to address gaps in care to decrease ED visits for current HPACT patients. The evaluation used qualitative key informant interviews and quantitative descriptive data analysis. The findings of this early implementation QI project inform clinical and organization processes for the WLA HPACT demonstration program that should be considered for on-going implementation. Recommendations for future implementation include complex care management interventions for patients with pain needs, substance abuse conditions, as well as modifications to organizational features of the program such as hours of operations, and team composition.

Book excerpt: Homelessness affects millions of Americans each year, and many homeless individuals have complex healthcare needs that place high demands on the United States healthcare system. This dissertation explores adverse health services utilization outcomes of the homeless population using the Healthcare Cost and Utilization Project's 2014 State Inpatient Database and the State Emergency Department Database from Florida, Maryland, Massachusetts, and New York. The first paper, "Association between homelessness and opioid overdose and opioid-related hospital admissions/emergency department visits," examines opioid overdose and opioid-related hospitalization/emergency department (ED) visit risks of homeless patients compared to a low-income housed comparison group. The study used multivariable linear probability models with hospital fixed effects for dichotomous outcomes. Outcomes were also stratified by sex and race/ethnicity. The study found that homeless patients had substantially higher risks for both outcomes, and the non-Hispanic white female homeless patients were particularly the highest risk group in this patient population. Implementing screenings for homelessness and opioid abuse may be critical for curbing the opioid epidemic in this population. The second paper, "Frequent emergency department use among homeless individuals: High risk of opioid-related diagnoses and adverse health services utilization outcomes," compares opioid outcomes, mechanical ventilation, mortality, and hospitalizations of homeless patients who had 4 or more ED visits, 2-3 ED visits and 1 ED visit in 2014, and identifies predictors for higher rates of ED use. Multivariable linear probability models with hospital fixed effects were used for the main analyses and a negative binomial regression model with hospital fixed effects was used for predicting higher rates of ED use. The study revealed that homeless patients who are high ED users were more likely to be hospitalized and have other adverse outcomes. These findings encourage targeted interventions for the high-utilizer homeless population to reduce the burden of serious outcomes and costs for the patient and society. The third paper, "Association between homelessness and delivery hospitalization outcomes: a multi-state population-based study," explores delivery hospitalization outcomes of pregnant homeless versus non-homeless women. This project used inpatient data from FL, MA and NY and overlap propensity-score weighing method and regression adjustment. Compared to non-homeless women treated within the same facility, homeless women had higher likelihoods of experiencing placental abnormalities, preterm labor and higher delivery hospitalization costs. A large majority of homeless women were treated in government-owned safety-net hospitals with lower average delivery costs compared to non-homeless women, who were mainly treated in not-for-profit hospitals. These findings highlight the importance of screening pregnant women for social needs, including homelessness, as well as developing policies that encourage partnerships between healthcare providers and community resources, such as local social housing programs. Taken together, all three papers highlight the adverse health outcomes of the homeless population and the need to quickly identify homeless patients and refer them to appropriate care.
