Book excerpt: Assists clinicians in managing their time to the greatest advantage and providing professional treatment to all patients simultaneously As inpatient psychiatric facilities face bed shortages and increasingly limited insurance coverage for their programs, partial hospitalization and intensive outpatient programs are growing within the mental health treatment community. This book provides clinicians (including social workers, counselors, therapists, and psychiatric nurses) new to partial hospitalization and intensive outpatient practice settings with a conceptual framework and practical skills for effective and efficient practice. It helps clinicians to develop realistic expectations for treatment in a managed care environment with limited time and funding. Partial hospitalization and intensive outpatient treatment programs (PHPs/IOPs) can present particularly stressful environments in which clinicians are engaged in multiple treatment processes with groups of diverse patients, all at different points in the treatment process. This book borrows the "ecology of games" concept from political theory to assist clinicians in managing their time to the greatest advantage and providing professional treatment to all patients simultaneously. It describes the key elements of a PHP/IOP, including treatment planning, discharge planning, group therapy, psychoeducation, and safety and confidentiality issues. It then addresses challenges unique to specific patient populations in the program--children, adults, older adults, the chronically mentally ill, the chemical dependent, individuals with co-occurring disorders--and offers strategies for coping with the multitude of issues they present. Key Features: Provides practical strategies for handling a variety of clinical issues in the PHP/IOP setting Covers specific problems and treatment planning goals for varied populations Guides clinicians in developing realistic expectations for treatment in a managed care environment with limited time and funding Includes chapters dedicated to coping with a death in a program, the personality-disordered patient, and conflicts among staff

Book excerpt: This book examines the intermediate level of mental health services with a focus on partial hospitalization program (PHP) and intensive outpatient program (IOP) models of care for youth. It reviews the history of PHPs and IOPs and highlights their current care models, demonstrating the increase in the development and implementation of evidence-based treatment (EBT) practices. The book explores issues relating to program development, implementation, and considerations for sustainability. It provides interventions designed to enhance the well-being of youth who are experiencing a range of mental health concerns as well as strategies to engage and involve their families. In addition, the book offers feasible strategies for measuring outcomes and applying these results to meaningful clinical evaluations in PHP and IOP settings. It describes the process of accessing and using these intermediate services as well as additional treatment resources that may be necessary in the continuum of mental health care for youth. Key areas of coverage include: The history and purpose of mental health care and the role of day treatment programs for youth. Working with program administration and other stakeholders, identifying a patient population, and engaging community and referral sources. The importance of family involvement, coordination of care, and simultaneously addressing the transactional relationship between physical and mental health. Transitioning youth from pediatric mental health services into the adult mental health system. Working with a diverse patient population in intermediate treatment programs. Providing practical information for families and practitioners navigating the pediatric mental health continuum of care. The Handbook of Evidence-Based Day Treatment Programs for Children and Adolescents is a must-have resource for researchers, professors, and graduate students as well as clinicians, therapists, course instructors, and other professionals in child and adolescent psychiatry, clinical child and school psychology, social work, counseling, public health, family studies, developmental psychology, pediatrics, and all related disciplines.

Available in PDF, EPUB and Kindle. Book excerpt: There was a time, not long ago, when the only treatment options considered to be worthwhile for patients requiring psychiatric care were the 50-minute hour on the one hand, or full-time hospitalization on the other. Most of us were convinced in those days that treatment could, and indeed should, take place with a minimum of involvement by the patient's family. Nor did we really consider that the community in which a patient lived was a significant contributor to either his illness or its cure. These naive assumptions were strongly challenged, of course, be ginning with the questions of social psychiatrists in the 50s and con tinuing with the quiet growth of the patients' rights movement. Thus it is no mere coincidence that when the community psychiatry movement emerged in the mid-60s as a powerful force for profound change in our traditional practice, the concept of partial hospitalization, which can be traced back at least 30 years, became a symbol of the new social psychiatry. Partial hospitalization had singular advantages well attuned to the times: it did not force a separation between the patient and his family; it cost far less to deliver than inpatient care; and it avoided the stigma of institutionalization while still providing far more care than the traditional psychotherapeutic hour. In a few years' time, several well controlled studies documented that virtually all patients who were cus tomarily treated on an inpatient basis could be effectively managed and treated in a day hospital.

Book excerpt: This new edition of Handbook of Mentalizing in Mental Health Practice reflects a vibrant field undergoing development along a number of dimensions important for mental health. As evidenced by the number of experts contributing chapters that focus on specialized approaches to mentalization-based treatment (MBT), the range of mental disorders for which this therapy has proved helpful has substantially increased, and now includes psychosis. Second, the range of contexts within which the approach has been shown to be of value has grown. MBT has been found to be useful in outpatient and community settings, and, more broadly, with children, adolescents, couples, and families, and the social contexts where they are found, such as in schools and even prisons. Finally, the framework has been shown to be generalizable to an understanding of the social context of mental health. The model advanced in this book goes beyond an understanding of the development of mentalizing and aims to provide an understanding of its role in a range of social processes.Key concepts, themes, and approaches clearly articulated throughout the book include the following: Mentalizing is a transdiagnostic concept applicable to a range of mental health conditions, including trauma, personality disorders, eating disorders, depression, substance use disorder, and psychosis. The chapters devoted to these disorders emphasize MBT skills acquisition and techniques for introducing mentalizing into psychotherapy. Mentalizing plays an important role in understanding how teams, systems, and services interact to facilitate or undermine interventions and service delivery. Chapters on mentalizing in teams and wider systems are included to help clinicians reduce negative impacts on clinical care and support reliable and responsive pathways to treatment. In an effort to encourage clinicians to integrate mentalizing into their clinical practice, empirical research on the developmental origins of mentalizing and how a focus on mentalizing can improve outcomes for patients is incorporated throughout the volume. Improved mentalizing increases resilience to adversity, perhaps protecting individuals from relapse, and improves therapeutic outcomes. The relevant research, as well as proven techniques for promoting resilience and trust, are discussed at length in the book. Finally, as an established component of the literature on neurobiology and higher-order cognition, mentalizing benefits from a number of different strands of research, ranging from neurobiology through child development to adult psychopathology. The book fully explores these relationships and their ramifications. Authoritative, comprehensive, and cutting-edge, the Handbook of Mentalizing in Mental Health Practice is the single most important resource for clinicians and trainees learning about -- and incorporating -- MBT into their therapeutic repertoire.

Book excerpt: "Group psychotherapy can be daunting, especially for students and others who have not led groups before. Choosing a model of therapy can be especially tricky. The best therapy, as Virginia Brabender and April Fallon explain in this book, is one that meshes well with the needs of the patient population, the therapist, and the environment in which treatment is taking place. They also emphasize the key role of process, the interactions between and among group members and the therapist, in the mechanics of group psychotherapy. For each approach, interpersonal, psychodynamic, cognitive behavior, acceptance and commitment, problem solving, and behavioral therapies, the auhtors examine theoretical underpinnings, characteristic interventions, relevant research, strengths and weaknesses across different clinical contexts, and the unique demands of the therapy setting. Using vivid clinical vignettes, Brabender and Fallon demonstrate how therapists can choose, adapt, and implement the model most suited for their group."--Page 4 de la couverture.

Book excerpt: In the past few years, there has been a major shift in the mental health care of child and adolescents from inpatient care to non-hospital community ambulatory settings. Economic pressures have primarily driven this change in service delivery. Insurance companies and managed care organizations have not only restricted access to hospitalization and limited length of stays but additionally have sharply reduced reimbursements for treatments. State and federal policies for inpatient treatment and reimbursement have followed similar trends. As a result, mental health planners have attempted to develop programs to deal with this trend of restricted inpatient care shifting treatment of children and adolescents to home and community settings. Some of these new programs are well planned and others are hastily planned and implemented. The pitfall to this community approach is that there is a population of chronically disturbed children and adolescents, and highly stressed parents often lacking adequate personal and family resource who may not respond to these new less restrictive, less costly community approaches which potentially may lead to an unsafe and dangerous situation for the child, adolescent, family and the community. With this in mind, the purpose of this book is to provide comprehensive and up-to-date information regarding child and adolescent outpatient, day treatment and community psychiatry. The emphasis of this book is to provide practical knowledge through clinical case illustrations and to explain various strategies in a detailed fashion.

Evidence-based and reflecting new developments in regulation and practice, this second edition is designed to provide key information for all immediate care practitioners, including doctors, paramedics, emergency medical technicians, and community responders. Reflecting the major advances in delivery of pre-hospital care, including the greater survival benefits for heart attacks and major trauma when delivering patients directly to higher levels of care, the evolution of the paramedic role into critical care paramedics, roadside rapid sequence induction of anaesthesia, and the introduction of mechanical chest compression devices, this new edition is the ideal companion for those involved in delivering pre-hospital care. It also links to relevant online databases and mobile apps that can assist with calculations, and contains key algorithms and formulae to ensure good care. The Oxford Handbook of Pre-hospital Care is the comprehensive quick-reference guide to all medical and trauma conditions regularly encountered in the immediate care setting.
