This expansion has been based on greater recognition of the prevalence and treatability of mental disorders, as well as the availability of a variety of forms of effective treatment. Indeed, throughout this period, our field has witnessed the introduction and the wide spread application of specific pharmacological treatments, as well as the development, refinement, and more broadly based availability of behavioral, psychodynamic, and marital and family interventions. The community mental health center system has come into being, and increasing numbers of mental health practitioners from the fields of psychiatry, psychology, social work, nursing, and related professional disciplines have entered clinical practice. In concert with these developments, powerful sociopolitical and socioeconomic forces-including the deinstitutionalization movement of the late 1960s and early 1970s and the cost-containment responses of the 1980s, necessitated by the spiraling cost of health care-have shaped the greatest area of growth in the direction of outpatient services. This is particularly true of the initial assessment and treatment of nonpsychotic mental disorders, which now can often be managed in ambulatory-care settings. Thus, we decided that a handbook focusing on the outpatient treatment of mental disorders would be both timely and useful. When we first began outlining the contents of this book, the third edition of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disor ders (DSM-III) was in its fourth year of use.

Available in PDF, EPUB and Kindle. Book excerpt: The practice of outpatient medicine in the 21st century has become fast-paced and challenging. The busy practitioner learns to make accurate decisions regarding diagnoses and treatments, individualizing them based on the patients’ particular characteristics and desires. Patients typically present with more than one issue, spanning from wanting information about a vaccine to having symptoms of an acute and serious infectious disease, while also needing care of their chronic conditions. In order to function in a timely manner, the provider needs a reliable resource to aid in making efficient decisions. This text is designed to fulfill this purpose. Now fully revised and expanded, Handbook of Outpatient Medicine, 2e provides a quick, portable, algorithm-based guide to diagnosis and management of common problems seen in adult patients. Written by experienced primary care practitioners, this text emphasizes efficient decision-making necessary in the fast-paced realm of the medical office. It covers general considerations such as the physical examination, care of special populations, and pain management and palliative care. It also focuses on common symptoms and disorders by system, including endocrine, respiratory, cardiac, orthopedic, neurologic, genitourinary, and gynecologic. For each disorder, symptoms, red flags, algorithms for differential diagnosis, related symptoms and findings, laboratory workup, treatment guidelines, and clinical pearls are discussed. One of the major updates in this edition is a chapter dedicated to COVID-19. This chapter focuses on COVID-19 diagnosis, care and sequelae, including what to do after discharge from the hospital. Since the global pandemic has affected medicine as a whole, many of the chapters also discuss COVID-19 as a differential diagnosis. Newer and higher quality photos have also been added in several chapters to help illustrate techniques more efficiently, including new imaging modalities for chest pain.

Some handouts ask questions and have spaces for your answers. Other handouts ask you to read and think about a subject or an idea, or they contain advice or reminders about recovery. It is a good idea to keep and review the handouts after you have used them. They will help you stay strong as you continue in your recovery. During each treatment session, your counselor will ask you to follow along on the handout while he or she goes over it with the group. The counselor will give you time to think about what it says and write your answers to questions it may ask. The group will then discuss the handout. You should share your thoughts and ask questions during this time. If you still have questions, there will be more time to ask questions during the last part of each session.

Book excerpt: This is the first book devoted to the steps from hospital discharge to outpatient therapy. Structured interventions are provided along with the pitfalls and roadblocks that threaten smooth transitions. The focus on the continuity of care goes beyond traditional aftercare planning and use of referral networks. The authors critically examine problems arising from schedule delays, selection of inappropriate therapies, relapse and rehospitalization, unauthorized disclosure of the case file, agency bureaucracy, and damaged therapist-client relationships. Case examples further illuminate failures of improper case management in contrast to step-by-step positive alternatives. The solutions offered are related to clients with specific hospitalized disorders (anxiety, anorexia, substance abuse) and special populations (children, adolescents, adults, and the elderly). Never before has a single work exposed the precarious transitions between programs and informed both clients and providers of ways to overcome mistakes by following simple guidelines.

Book excerpt: In the past few years, there has been a major shift in the mental health care of child and adolescents from inpatient care to non-hospital community ambulatory settings. Economic pressures have primarily driven this change in service delivery. Insurance companies and managed care organizations have not only restricted access to hospitalization and limited length of stays but additionally have sharply reduced reimbursements for treatments. State and federal policies for inpatient treatment and reimbursement have followed similar trends. As a result, mental health planners have attempted to develop programs to deal with this trend of restricted inpatient care shifting treatment of children and adolescents to home and community settings. Some of these new programs are well planned and others are hastily planned and implemented. The pitfall to this community approach is that there is a population of chronically disturbed children and adolescents, and highly stressed parents often lacking adequate personal and family resource who may not respond to these new less restrictive, less costly community approaches which potentially may lead to an unsafe and dangerous situation for the child, adolescent, family and the community. With this in mind, the purpose of this book is to provide comprehensive and up-to-date information regarding child and adolescent outpatient, day treatment and community psychiatry. The emphasis of this book is to provide practical knowledge through clinical case illustrations and to explain various strategies in a detailed fashion.

Counselors can support clients' movement toward positive changes in their substance use by identifying and enhancing motivation that already exists. Motivational approaches are based on the principles of person-centered counseling. Counselors' use of empathy, not authority and power, is key to enhancing clients' motivation to change. Clients are experts in their own recovery from SUDs. Counselors should engage them in collaborative partnerships. Ambivalence about change is normal. Resistance to change is an expression of ambivalence about change, not a client trait or characteristic. Confrontational approaches increase client resistance and discord in the counseling relationship. Motivational approaches explore ambivalence in a nonjudgmental and compassionate way.

Book excerpt: The Matrix IOP method was developed initially in the 1980s in response to the growing numbers of individuals entering the treatment system with cocaine or methamphetamine dependence as their primary substance use disorder. Many traditional treatment models then in use were developed primarily to treat alcohol dependence and were proving to be relatively ineffective in treating cocaine and other stimulant dependence (Obert et al. 2000). To create effective treatment protocols for clients dependent on stimulant drugs, treatment professionals at the Matrix Institute drew from numerous treatment approaches, incorporating into their model methods that were empirically tested and practical. Their treatment model incorporated elements of relapse prevention, cognitive?behavioral, psychoeducation, and family approaches, as well as 12-Step program support (Obert et al. 2000).
