In joint discussions in 1974, the Club worked out a new terminology that was acceptable to all members and will hopefully be acceptable to other pathologists. Since then, it has been called the Kiel Classification. We are also pleased that the clinicians belonging to the Kiel Lymphoma Study Group-foremost A. STACHER and G. BRITTINGER have been able to present preliminary data on the clinical relevance of the new classification. Finally, our optimism was fortified by the findings of R.J. LUKES and R.D. COLLINS, which largely concur with ours in both concept and practical significance. This book is divided into six main sections. First, there is a chapter on normal cytology that supplements and, in some respects, revises the presentation given in Part A of this Handbook (1961). H.K. MULLER-HERMELINK played a major role in the writing of the first chapter. The second chapter is a brief description of the light-microscopic techniques that are used in our laboratories and have proved to be suitable for a precise diagnosis of lymph-node diseases.

Book excerpt: For 10 years the book series Acute Leukemias has been providing updates on the rapid progress being made internationally concerning this group of diseases. The fifth volume mainly addressed experimental approaches, but the present issue presents both therapeutic and prognostic aspects of the most recent results from major multicenter clinical trials. Additional chapters report new trends in leukemia cell biology, the monitoring of minimal residual disease, and secondary leukemias, as well as new antileukemic drugs, antimicrobial strategies, and the use of cytokines. The combined efforts against acute leukemias described in this book explain the recent improvements in the outcome of patients suffering from acute leukemias.

Following the tradition of the Acute Leukemias series since 1987, this book bridges the gap between basic research and clinical studies and emphasizes that both aspects are equally necessary to achieve improvements, not only in understanding the disease but also in providing better therapy. As a forum for world-wide activities in the field of acute leukemias the volume contains invaluable contributions that provide the reader with new, previously unpublished information.

Available in PDF, EPUB and Kindle. Book excerpt: "Es ist ja ganz eineriei, wer es gejunden hat, die Hauptsache ist, daB es gejunden worden ist." This was said by Hermann Wilbrand, director of the department of opthalmology at the university hospital Hamburg-Eppendorf (1919-1923), when others claimed priority for the long searched for cortical visuell center, which he had discovered. Five years ago we tried in Wilsede an unusual experiment, bringing together for three long days and nights scientists and medical doctors to learn like students about each others work. The hope was that the participants in the workshop would discuss the whole problem of human leukemia, and coopera tive programmes among the different specialized research groups and medical centers would be stimulated. Now five years later we feel that the experiment was a success. Coope rative research programmes around the world were started and most of us understand more and more the Frederic Stohl man question about the pratical application of our research and its benefit for the patient. The chairman have done a tremendous job organizing an up to date scientific programme for the third Wilsede meet ing. I would like to thank all of them for a programme which includes all the important results and future aspects of human leukemia.

Available in PDF, EPUB and Kindle. Book excerpt: You see things, and sa)' why? But I dream 1hings that never were, and I say, 11'hy 110t? George Bernhard Shaw Far ahead of his time, June 1st, 1909, Alexander Maximov communicated in a lecture, given in the Charite in Berlin, the fundamental knowledge, that there exists a lymphoid hemopoetic stem cell. Alexander Friedenstein explained that during the following years, Maximov also showed that the idea of interaction between hemopoetic cells and their stroma to be one of the most significant experiences. Monoclonal antibodies, recombinant DNA technics and the improvement of tissue culture models are the major developments to improve our possibilities to clarify growth and differentiation functions of hemopoetic cells. During the last two decades it was shown that soluble products, released from T cells, were not only involved in inducing B cells to produce specific immunoglobulin secretion after antigen stimulation. Furthermore, lymphokines together with other cytokines regulate the growth and differentiation of hemopoetic cells. As I have learned from Dick Gershon, our knowledge of the cellular basis for immunoregulation has come a long way since 450 B.C. Thucydides comments on the possible role of immune response in controlling the Black Death. Dick Gershon speculated that no scientific interest for these interesting observations was put forth at that time. Perhaps the problems, the Athenians were having with the Spartans, converted money from basis research into the military budget.

Available in PDF, EPUB and Kindle. Book excerpt: The rates of acute leukemia cure have gradually improved over the last decade. Clinical study results reflect the impact of chemotherapy intensity and duration, the role of prolonged maintenance, intensified consolidation or very early intensification. Further progress has also been achieved in bone marrow trans plantation, and recent prospective studies and meta-analyses have contributed comparisons of the high antileukemic efficacy of bone marrow transplantation to that of improved chemotherapy. This allows a more successful combining of the two forms of treatment. New prognostic factors have emerged from both cytogenetic and molecular genetic research. Thus, the Philadelphia chromosome translocation and the bcr/abl gene rearrangement have proven to be the dominating risk factor in acute lymphoblastic leukemia. Since the frequency increases with age, differences in prognosis between children and adults can be explained. Evaluation of molecular and immunologic leukemia cell markers has provided a better understanding of residual leukemia in clinical remission, as a prognostic factor and in monitoring the effectiveness of the antileukemic strategy. Recent work on leukemic cell biology has resulted in novel therapeutic approaches such as terminal differentiation by all-trans-retinoic acid, modulation of chemotherapy by hematopoietic growth factors such as GM-CSF and enhancement of immunologic control by cytokines such as interleukin 2. New antimicrobial drugs and the application of mostly empiric anti-infectious strategies have helped reducing the therapeutic risk. Thus, a number of recent achievements have provided us with new options in the management of patients with acute leukemias.

Available in PDF, EPUB and Kindle. Book excerpt: We have studied 24 cases of secondarily leukemic (stage V) lymphosarcoma (LS), 31 cases of "d'emblee" leukemic LS, and ten cases of lymphoid leukemic neoplasias transitional between "d'emblee" leukemic LS and chronic lymphocytic leukemia (eLL). These cases only concern the common types ofthe WHO classification ofLS, i.e., the prolymphocytic, the lymphoblastic, and the immunoblastic. Some cases have also been classified by cell surface markers. The secondarily leukemic conversion occurred in 40% of the lymphoblastic types, in 14% of the prolymphocytic types, and in 17% of the immunoblastic types. It never occurred at stage I but could occur after any other stage. The mediastinal involvement was observed in three types, but most often in the lymphoblastic type. The prognosis after an acute lymphoid leukemia (ALL) treatment comprising active immunotherapy following chemo(radio)therapy is better for the leukemic prolymphocytic and lymphoblastic LS than for the immunoblastic type. Two patients (one of the lymphoblastic type) are in complete remission after 8 and 5 years, respectively. We have described ten cases of "d'emblee" leukemic LS with either large lymphoid or extra lymphoid masses, bone marrow leukemic cell involvement, and LS aspects of neoplastic cells. Mediastinal, abdominal, or other tumor masses are frequent.

Available in PDF, EPUB and Kindle. Book excerpt: 150 years after the first description of the clinical picture of "white blood" and the introduction of the term "leukemia" by R. Virchow it appears, that the leukemias, and the acute leukemias in particular, serve as an impressive example for the major improvements that have been achieved in the treatment but also in the understand ing of the biology of malignant dis orders. The international symposia "Acute Leukemia" which are held at Münster since 1986 have developed into an interna tional forum to review the current progress and the future perspectives of leukemia research and therapy at a high scientific and clinicallevel. Since the possibility for active participation in these symposia is somewhat restricted we are glad to have the opportunity to extend the information that was presented at the symposium "Acute Leukemias V - Experimental Approaches and Management of Refractory Disease" which was held from February 27 to March 2, 1994 to a broader audience of basic scientists and clinicians. This meeting was especiaIly designed to discuss experimen tal approaches and the management of refractory disease which allows to evaluate new experimental therapies on the basis of preclinical studies.
