Book excerpt: Testing and diagnosis of hepatitis B (HBV) and C (HCV) infection is the gateway for access to both prevention and treatment services, and is a crucial component of an effective response to the hepatitis epidemic. Early identification of persons with chronic HBV or HCV infection enables them to receive the necessary care and treatment to prevent or delay progression of liver disease. Testing also provides an opportunity to link people to interventions to reduce transmission, through counselling on risk behaviors and provision of prevention commodities (such as sterile needles and syringes) and hepatitis B vaccination. These are the first WHO guidelines on testing for chronic HBV and HCV infection and complement published guidance by WHO on the prevention, care and treatment of chronic hepatitis C and hepatitis B infection. These guidelines outline the public health approach to strengthening and expanding current testing practices for HBV and HCV, and are intended for use across age groups and populations.

The two diseases account for about a million deaths a year and 78 percent of world's hepatocellular carcinoma and more than half of all fatal cirrhosis. In 2013 viral hepatitis, of which hepatitis B virus (HBV) and hepatitis C virus (HCV) are the most common types, surpassed HIV and AIDS to become the seventh leading cause of death worldwide. The world now has the tools to prevent hepatitis B and cure hepatitis C. Perfect vaccination could eradicate HBV, but it would take two generations at least. In the meantime, there is no cure for the millions of people already infected. Conversely, there is no vaccine for HCV, but new direct-acting antivirals can cure 95 percent of chronic infections, though these drugs are unlikely to reach all chronically-infected people anytime soon. This report, the first of two, examines the feasibility of hepatitis B and C elimination in the United States and identifies critical success factors. The phase two report will outline a strategy for meeting the elimination goals discussed in this report.

Available in PDF, EPUB and Kindle. Book excerpt: The global epidemic of hepatitis B and C is a serious public health problem. Hepatitis B and C are the major causes of chronic liver disease and liver cancer in the world. In the next 10 years, 150,000 people in the United States will die from liver disease or liver cancer associated with chronic hepatitis B virus (HBV) or hepatitis C virus (HCV) infections. Today, between 800,000 and 1.4 million people in the United States have chronic hepatitis B and between 2.7 and 3.9 million have chronic hepatitis C. People most at risk for hepatitis B and C often are the least likely to have access to medical services. Reducing the rates of illness and death associated with these diseases will require greater awareness and knowledge among health care workers, improved identification of at-risk people, and improved access to medical care. Hepatitis B is a vaccine-preventable disease. Although federal public health officials recommend that all newborns, children, and at-risk adults receive the vaccine, about 46,000 new acute cases of the HBV infection emerge each year, including 1,000 in infants who acquire the infection during birth from their HBV-positive mothers. Unfortunately, there is no vaccine for hepatitis C, which is transmitted by direct exposure to infectious blood. Hepatitis and Liver Cancer identifies missed opportunities related to the prevention and control of HBV and HCV infections. The book presents ways to reduce the numbers of new HBV and HCV infections and the morbidity and mortality related to chronic viral hepatitis. It identifies priorities for research, policy, and action geared toward federal, state, and local public health officials, stakeholder, and advocacy groups and professional organizations.

WHO estimates that 58 million people had chronic hepatitis C virus (HCV) infection globally in 2019, and less than a quarter of them were diagnosed. New and innovative approaches are needed to accelerate progress toward the HCV elimination targets. Self-testing is one such approach. These guidelines provide a new recommendation and guidance on HCV self-testing to complement existing HCV testing services in countries. These guidelines also highlight operational considerations to support strategic implementation and scale up of HCV self-testing.

Available in PDF, EPUB and Kindle. Book excerpt: Individuals who donate their blood provide a unique and precious gift in an act of human solidarity. In order to donate blood, prospective donors should be in good health and free from any infections that can be transmitted through transfusion. Most blood donors perceive themselves to be healthy, but some are unsuitable to donate blood due to the potential risk of compromising or worsening their own health or the risk of transmission of infections to patients. Blood transfusion services (BTS) have a duty of care towards blood donors as well as to the recipients of transfusion. This duty of care extends to prospective donors who are deferred from donation--whether on a temporary or permanent basis--as well as those who donate blood and are subsequently found to have unusual or abnormal test results. BTS have a responsibility to confirm test results and provide information, counseling and support to enable these individuals to understand and respond to unexpected information about their health or risk status. Counseling is part of the spectrum of care that a BTS should be able to provide to blood donors--including referral to medical practitioners or specialist clinical services. Pre-donation counseling was recognized as one element of the strategy to reduce and, if possible, prevent the donation of blood by individuals who might be at risk for HIV and other TTI including hepatitis B and C viruses as well as to inform the donor of the donation process and testing of blood for HIV. Post-donation counseling was acknowledged to be a necessary element of donor management as an adjunct to informing donors of unusual or abnormal test results. Blood donor counseling by trained specialist staff is now considered to be a key component of the blood system in most countries with a well-developed blood transfusion service. It may be required at a number of stages in the blood donation process or following blood screening and should be available at any point at which the BTS has an interface with donors. In many countries, however, blood donor counseling is not yet available in a structured way. Blood Donor Counselling: Implementation Guidelines has therefore been developed to provide guidance to blood transfusion services that have not yet established donor counseling programs.

Available in PDF, EPUB and Kindle. Book excerpt: Chronic Hepatitis C Virus: Lessons from the Past, Promise for the Future documents the monumental advances that have been made in our understanding of chronic HCV during the past decade. The first section reviews the natural history of chronic HCV, how this virus can affect other organs in addition to the liver, and whether treating chronic HCV alters the natural history of this disease. Section 2 reviews the advances that have been made in the treatment of chronic HCV during the past decade with interferon based therapy. Separate chapters on response guided therapy and how to manage the adverse events associated with these medications provide the physician with the concepts required to more effectively treat chronic HCV now and in the future. As the genetics of virologic response have recently been elucidated, a chapter is devoted to helping the clinician understand how genes that modulate disease processes and their treatment are identified and utilized in clinical care. Section 3 deals with the future of HCV treatment and specific inhibitors of HCV. Specific chapters explain how targets for drugs are identified and how drugs are then developed and tested; how mutations of HCV develop and how anti-viral agents will affect this process; the most up to date data regarding the treatment of chronic HCV with peginterferon, ribavirin and anti-viral agents; and the potential to treat chronic HCV with just oral anti-viral agents and without peginterferon and ribavirin in the future. The final section of this book covers issues related to liver transplantation in patients with chronic HCV. Separate chapters review the natural history of chronic HCV in liver transplant recipients and the impact of utilizing HCV positive donors. The volume concludes with chapters that cover the treatment of chronic HCV both prior to and after liver transplantation with potent anti-viral agents. Chronic Hepatitis C Virus: Lessons from the Past, Promise for the Future is a valuable resource for all physicians caring for patients with chronic HCV.

Book excerpt: The field of HCV therapeutics continues to evolve rapidly and since the World Health Organization (WHO) issued its first Guidelines for the screening care and treatment of persons with hepatitis C infection in 2014 several new medicines have been approved by at least one stringent regulatory authority. These medicines called direct-acting antivirals (DAAs) are transforming the treatment of HCV enabling regimens that can be administered orally are of shorter duration (as short as eight weeks) result in cure rates higher than 90% and are associated with fewer serious adverse events than the previous interfere on containing regimens. WHO is updating its hepatitis C treatment guidelines to provide recommendations for the use of these new medicines. The objectives of these WHO guidelines are to provide updated evidence-based recommendations for the treatment of persons with hepatitis C infection using where possible all DAA-only combinations. The guidelines also provide recommendations on the preferred regimens based on a patient?s HCV genotype and clinical history and assess the appropriateness of continued use of certain medicines. This document also includes existing recommendations on screening for HCV infection and care of persons infected with HCV that were first issued in 2014. The key audience for these guidelines are policy-makers in low- and middle-income countries who formulate country-specific treatment guidelines and who plan infectious disease treatment programmes and services in addition to those people responsible for delivering treatment. The guidelines are appropriate for all countries including high-income countries.

Book excerpt: This policy brief, one of two on the updated HCV guidelines, focuses on the new recommendations on simplified service delivery for a public health approach to HCV testing, care and treatment. These recommendations include decentralization, integration and task-sharing, in addition to the use of POC HCV viral load assays and reflex viral load testing. Simplified service delivery (decentralization, integration and task sharing): Expansion of HCV testing and treatment services, ideally at the same site, through decentralization of care to lower-level facilities; integration with existing services, such as in primary care, harm reduction programmes, prisons and HIV ervices; and promotion of task sharing through delivery of HCV testing, care and treatment by appropriately trained non-specialist doctors and nurses. HCV diagnostics – use of Point-of-care (POC) HCV RNA viral load and reflex HCV RNA viral load testing: The use of point-of-care (POC) HCV ribonucleic acid (RNA) assays is now recommended as an alternative approach to laboratory-based RNA assays to diagnose viraemic infection. This is especially applicable to marginalized populations, such as persons who inject drugs, and hard-to-reach communities with limited access to health care and high rates of loss to follow-up.

Book excerpt: The Consolidated guidelines on HIV, viral hepatitis and STI prevention, diagnosis, treatment and care for key populations outline a public health response for five key populations (men who have sex with men, trans and gender diverse people, sex workers, people who inject drugs and people in prisons and other closed settings). They present and discuss new recommendations and consolidate a range of recommendations and guidance from current WHO guidelines. Particularly for key populations, social, legal, structural and other contextual factors both increase vulnerability to HIV, viral hepatitis and STIs and obstruct access to health and other essential services. These guidelines highlight the critical importance of addressing structural barriers in all settings as a priority. In most countries, inadequate coverage and poor quality of services for key populations continue to undermine responses to HIV, viral hepatitis and STIs. All countries should prioritise reaching key populations and supporting key population communities to lead the response and provide equitable, accessible and acceptable services. In most countries, inadequate coverage and poor quality of services for key populations continue to undermine responses to HIV, viral hepatitis and STIs. All countries should prioritise reaching these key populations and supporting key population communities to lead the response and provide equitable, accessible and acceptable services to these groups.

Book excerpt: In 2011, heads of state and government at the United Nations General Assembly formally acknowledged that NCDs are one of the major challenges to development and committed to integrate NCD prevention and control into other programmes, such as for HIV and broader maternal and child health programmes, especially in PHC. The commitment to integrate NCDs into existing disease programmes was reaffirmed again through its NCD resolutions at its Third High-level Meeting in 2018. This guidance outlines strategic actions and practical solutions in response to the challenges of integration of NCDs, as appropriate and relevant to the country context. The objective is to maximize the impact of health services and extend access to NCD care. The target readership is policymakers, programme managers and health providers. The guidance should also be useful for WHO and international partners (including donors and banks) that champion integrated support for NCD service delivery in countries and better management.
