Les infections du site opératoire (ISO) sont fréquentes (prévalence : 4,5%), graves (létalité : 2,5% à 5%), coûteuses (environ 1 semaine d'hospitalisation supplémentaire, environ 3 000 dollars) et en partie évitables (Senic Project). La législation française, l'évolution jurisprudentielle des responsabilités, l'accréditation et les contraintes budgétaires font de la surveillance épidémiologique des ISO un enjeu de management des risques nosocomiaux. Dans un établissement de santé, l'organisation de la surveillance nécessite une bonne définition préalable des objectifs, des moyens nécessaires et des méthodes envisagées pour les atteindre. L'efficience de la méthode de surveillance choisie dépend en partie du suivi des patients après leur sortie de l'hôpital. La seconde partie de ce travail est une étude descriptive des IS0 diagnostiquées lors de ce suivi. Matériels et méthodes : Nous avons utilisé les données recueillies par INCISO, réseau de surveillance des ISO coordonné par le C-CLIN Paris Nord. Résultats : 15 085 interventions de chirurgie digestive ont été étudiées. La durée moyenne de séjour était de 9,5 jours. 759 ISO ont été diagnostiquées, 35,4% après la sortie de l'hôpital. 57% des ISO à NNIS 0 l'ont été après la sortie. La proportion d'ISO après sortie était inversement proportionnelle à la durée d'hospitalisation et le délai médian de diagnostic était de 12,5 jours. La plupart des facteurs de risque classiques d'ISO étaient associés après analyses unifactorielles, au risque d'ISO après sortie (NNIS index, classe de contamination, score ASA, durée d'intervention, expérience du chirurgien, urgence, chirurgie coelioscopique). Conclusions : Le taux d'incidence des ISO à NNIS 0 est un indicateur de qualité des soins chirurgicaux. Le suivi après sortie des patients opérés est le gage d'une surveillance épidémiologique de qualité procurant des taux d'incidence d'IS0 à NNIS 0 fiable.

A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.
