For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. The Eighth Day reflects a conviction that the human trajectory, for all its uniqueness and indeterminism, will never be satisfactorily understood until it is framed in dynamics that are common to all of nature. The problem in doing this, however, lies in ourselves. The major social theories have failed to treat human social evolution as a component of broader natural processes. The Eighth Day argues that the energy process provides a basis for explaining, comparing, and measuring complex social evolution. Using traditional ecological energy flow studies as background, society is conceived as a self-organization of energy. This perspective enables Adams to analyze society in term of the natural selection of self-organizing energy forms and the trigger processes basic to it. Domestication, civilization, socioeconomic development, and the regulation of contemporary industrial nation-states serve to illustrate the approach. A principal aim is to explore the limitation that energy process imposes on human social evolution as well as to clarify the alternatives that it allows. Richly informed by contemporary anthropological historicism, sociobiology, and Marxism, The Eighth Day avoids simple reductionism and denies facile ideological categorization. The question of the benefits of some pain to communicate the presence of a potentially dangerous event is the subject of a thoughtful discussion in Chapter 4. Nociception may trigger appropriate physiological responses. However, severe responses may in themselves be deleterious as documented by the improved outlook when they are blocked during surgery. Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.
