Book excerpt: The purpose of this study was to investigate the transition experience of new graduate nurses during their first work experience after graduating from a baccalaureate undergraduate nursing program. A case study approach was used to examine a community hospital in Connecticut. A random sample of baccalaureate prepared new graduate nurses who graduated in May 2007, and were in the first year of working as registered nurses, was chosen. The research question examined how new graduate nurses perceived the supports provided within their transition experience. The study examined the supports through Benner's (1982) developmental theory of nursing. The four supports included: support during orientation, support from experienced nurses, supports to develop time management skills and support from their undergraduate nursing education. The four research questions addressed each of these supports. The study used three methods of data collection. Twelve new graduate nurses completed the quantitative survey, eight participated in the individual interview and four new graduate nurses participated in the focus group interview. The conclusions included the following: new graduate nurses were supported by the learning environment of the orientation; new graduate nurses perceived a supportive network was available to them within the hospital environment; time management skills are essential for the transition to entry nursing practice; new graduate nurses were assisted by their undergraduate school experiences during the transition period. Recommendations for practice have implications for making new graduates successful in a nursing career. Hospital orientations should provide preceptors who encourage autonomy and are consistent and provide emotional support and need to provide support in the transition period by mentoring and encouraging guidance from experienced nurses. Hospitals also need to provide clinician guidance and organizational resources to assist with time management, and offer intern programs to students prior to graduation. Undergraduate nursing programs should incorporate more clinical time to help the new graduate nurse transition into nursing practice. Recommendations for further research might involve a qualitative in-depth study regarding time management as well as an in depth case study examining new graduate nurses perceived level of support during the orientation period.

The purpose of this descriptive qualitative study was to explore how new graduate BSN nurses perceive their professional confidence and what contributes to it during their first year of practice in the hospital setting. Data collection involved one individual semi-structured recorded conference interview with each of the five participants through the Zoom application which was transcribed verbatim. Each transcript was analyzed using manual content analysis with a constant comparative technique. Five major themes as well as four facilitators and four inhibitors to the development of professional confidence emerged. The five themes related to new graduate nurses' perception of professional confidence included knowledge and experience, interprofessional collaboration, independence, ongoing learner, and critical thinking. The four facilitators were positive feedback, support systems, asking for help, and perseverance. The four inhibitors were self-doubt, not being confident, being unprepared, and fear. These findings have implications for both undergraduate nursing education programs and nursing practice in the hospital setting. Undergraduate nursing programs should evaluate implementing strategies that better prepare new graduate nurses for their transition into the professional role. Healthcare professionals involved in developing new graduate nurses in practice, specifically nurse managers and preceptors, should be aware that they are entering practice without professional confidence and should focus efforts on what they can do to help promote its development." -- From page v.

Book excerpt: Anecdotal evidence from third-year nurses suggests students experience high levels of apprehension about entering the workforce. Whether such levels of nervousness about initial professional experience can be attributed to perceived or actual lack of competence, or indeed a lack of confidence in ability, is unclear. Graduate nurse programs were set up to help orientate new graduates on entering the workforce. Difficulties occur when the organizations, from which new graduates initially receive their experience, reinforce prevailing traditional attitudes and ideologies which tend to overshadow, or at the very least, initially intimidate the 'raw recruit'. A phenomenological approach was used to explore garduate nurses' perceptions, experiences and reflections during their transition from students to registered nurses. The main aim of the research was to gain a depth of understanding of the process undergone by nurses as they 'lived through' the transition from student to graduate nurses. To achieve this, in-depth, one-on-one interviews were conducted. Five graduate nurses who attended the graduate nurse program at a large regional hospital participated in the study. Data analysis identified the majority of new graduates interviewed travelled along an intricate continuum in their transition year. As graduates progressed from one phase to the next, they identified opposing feelings. The reasons for such a dichotomy of experiences is discussed within emerging themes of anticipation, vulnerability, seeking support, high expectations, rites of passage, the induction process, internal struggle, reality versus self doubt, swimming against the tide, the process of remoulding, and surviving, and seeing the light. Evident throughout was the overwhelming desire of graduates to 'make it', to 'not give up'. Implications for academics and health organizations, relating to the concerns of new registered nurses are addressed in an attempt to put in place effective strategies that will assist in the smooth transition from student to confident registered nurse.000000000000.

Especially when the student nurse becomes a professional nurse and discovers a gap between the anticipated ideal world and the harsh reality of the real world, the student nurse suffers from reality shock. As early as 1974, the concept of "reality shock" was described in the literature (Kramer, 1974). The purpose of this study was to determine if a nurse intern program eases role transition from student to professional nurse. The perceptions of professional nurses who participated in a nurse intern program while still in nursing school were explored. Professional nurses who had graduated from nursing school within the last six to twelve months and were nurse interns in a hospital setting prior to graduation from nursing school were eligible for participation in this study. The nurse also must have passed the nursing boards and be working as a professional nurse. Nurses volunteered to be interviewed about their experiences. The sample included eight participants who were professional nurses at a large metropolitan healthcare organization. The eight participants were interviewed once by the researcher in a private and quiet area. An interview guide was used during the interview process. The interviews were audio-taped and transcribed verbatim. The researcher coded the transcripts and used content analysis to identify themes. The nurse intern program eases the transition from student to professional nurse. The professional nurses commented on how comfortable and easy their transition was. The overall theme that emerged was a "seamless transition" which was expressed by the participants. There were three main categories that coincided with the overall theme of seamless transition which included being supported, learning through experience and being oriented. Most of the participants felt they would have been more stressed without the nurse intern program. Overall the transition from student to professional nurse was a smooth process for all eight of the participants.

To measure the effectiveness of the final practicum the Preparedness for Graduate Nursing Practice Questionnaire was used to determine levels of self-reported preparedness using a senior group of associate degree nursing students. The results confirm the importance of the final practicum experience properly preparing student nurses for their transition into professional practice. Study results confirm the importance of evaluating the effectiveness of the final practicum and tailoring it to meet the needs of student nurses to maximize potential benefits.

Entry into practice is primarily via health services who offer employment via graduate transition programs in the form of one year fixed-term contracts. Transition programs and the graduate nurse have been the subject of research over a number of decades. The research focus has been on the nature of the support provided and the lived experience of the graduate nurse. The purpose of this study was to generate further knowledge and theory about graduate nurses in transition, and to investigate how nursing students successfully transition to become graduate nurses. -- This study reaffirms the failure of education institutions and health care organisations to effectively support the passage of graduate nurses in their transition from student to professional registered nurse. An integrative review, a critical review of the literature, a literature review, and two grounded theory studies were undertaken to explore the spectrum of factors and perceptions that have an impact on graduate nurses and their passage to competent professional nurse in Australia from the perspectives of the graduates and the published research. -- The findings of my research have revealed that graduate nurses are succeeding, not as previously assumed due to the transition programs provided, but from other factors which are filling the gaps. The reliance on transition programs is fraught with problems, because it is customary for graduate nurses to initiate their own support, engage in self-care, and action self-advocacy strategies. This has prompted the development of a new model which uniquely advocates a self-support framework for the purpose of preparing nursing students to successfully navigate their transition year. This unique theoretical construction reconceptualises both the nature of the transitional support provided to graduate nurses, and university preparation for practice. Recommendations include development of curricula by universities to ensure that nursing students are prepared with the essential self-support skills to successfully transition to being a professional nurse.

Although not new, accelerated second-degree baccalaureate nursing (ASDBN) programs have opened in record numbers in recent years in the wake of the current nursing shortage. Little is known about the experience of professional practice for accelerated second-degree baccalaureate graduate nurses (ASDBGNs). The stressful graduate nurse transition, current nursing shortage, and lack of an empirical base for ASDBN programs illustrate the significance of the research problem. This modified grounded theory study generated a substantive Theory of Overcoming: ASDBGN Transition to Professional Nursing Practice. Constant comparative method of joint data collection, analysis, theoretical sampling, and memoing was used. Data were collected through semistructured interviews using open-ended questions that were conducted over the telephone or in person. The identified basic social process (BSP), overcoming, encompasses 5 stages: reality check, goaling, getting started, coming out on top, and mastering. Study findings provide a beginning evidence-base for nursing education, policy, and clinical practice related to this growing student population.

NP students, near graduation, are transitioning into the role of novice nurse practitioners. They may experience nervousness and a sense of incompetence as they transition into the role. To ensure the successful transition of the RN to the NP role, identification of factors and strategies that positively influence this transition need to be identified and implemented.
