Book excerpt: Cardiovascular disease (CVD), once thought to be confined primarily to industrialized nations, has emerged as a major health threat in developing countries. Cardiovascular disease now accounts for nearly 30 percent of deaths in low and middle income countries each year, and is accompanied by significant economic repercussions. Yet most governments, global health institutions, and development agencies have largely overlooked CVD as they have invested in health in developing countries. Recognizing the gap between the compelling evidence of the global CVD burden and the investment needed to prevent and control CVD, the National Heart, Lung, and Blood Institute (NHLBI) turned to the IOM for advice on how to catalyze change. In this report, the IOM recommends that the NHLBI, development agencies, nongovernmental organizations, and governments work toward two essential goals: creating environments that promote heart healthy lifestyle choices and help reduce the risk of chronic diseases, and building public health infrastructure and health systems with the capacity to implement programs that will effectively detect and reduce risk and manage CVD. To meet these goals, the IOM recommends several steps, including improving cooperation and collaboration; implementing effective and feasible strategies; and informing efforts through research and health surveillance. Without better efforts to promote cardiovascular health, global health as a whole will be undermined.

CVD already accounts for almost 10 percent of the developing world's burden of disease and is likely to become the developing world's leading cause of death. There is reason for hope, however, given that huge potential exists for applying R&D to control this emerging epidemicâ€"both in creating powerful new interventions such as vaccines and dietary supplements and in guiding behavior. In addition, a considerable body of evidence suggests that current risk-factor prevention programs and low-cost case management of CVD offer feasible, cost-effective ways to reduce CVD mortality and disability in developing country populations. Large-scale CVD control efforts are lacking, however, and thus governments and individuals are left to make choices about health and health care services without the benefit of appropriate knowledge. This report was designed to promote a policy dialogue on CVD based on informed knowledge of R&D opportunities that offer effective, affordable, and widely applicable responses in developing countries. The report examines (a) the emerging burden of CVD in developing countries, (b) the future worldwide burden of CVD, (c) current prevention and treatment of CVD in developing countries, (d) R&D to support CVD control, (e) opportunities and priorities for R&D, and the need for institutional arrangements for collaboration in facing the epidemic.

With aging, the disease burden shifts toward noncommunicable diseases (NCDs) which can threaten healthy aging. However, securing the gains expected from the demographic dividendwhere developing countries working and nondependent population increases and per capita income thus rises is both achievable and affordable through efficiently tacking NCDs with prevention and control efforts. This book looks primarily at cardiovascular disease (CVD) and tobacco use since they account for a disproportionate amount of the NCD burdenthe focus is strategic, rather than comprehensive. The goal of this book is to encourage countries to develop, adopt, and implement effective and timely country and, where appropriate, regional responses that reduce both population-level risk factors and the NCD burden. The work develops (i) an NCD burden and risk factor profile for all countries and the region as a whole; (ii) a rationale for public policy and action for NCDs; (iii) a framework to guide the formulation of public policies and strategies for NCDs; (iv) a country profile, including capacity and ongoing NCD activities, as well as policy options and actions for NCDs that will help stimulate policy dialogue within and among countries; and (v) a regional strategy for NCD prevention and control where regional collaboration offers added value. A detailed biological and clinical framework is provided as a prerequisite for adequate modeling. Chapter 1 presents cardiovascular risk factors and markers, where the search for new criteria is aimed at improving early detection of chronic diseases. The subsequent chapters focus on hypertension, which involves the kidney among other organs as well as many agents, hyperglycemia and diabetes, hyperlipidemias and obesity, and behavior. The last of these risk factors includes altered circadian rhythm, tobacco and alcohol consumption, physical inactivity, and diet. The volumes in this series present all of the data needed at various length scales for a multidisciplinary approach to modeling and simulation of flows in the cardiovascular and ventilatory systems, especially multiscale modeling and coupled simulations. The cardiovascular and respiratory systems are tightly coupled, as their primary function is to supply oxygen to and remove carbon dioxide from the body's cells. Because physiological conduits have deformable and reactive walls, macroscopic flow behavior and prediction must be coupled to nano- and microscopic events in a corrector scheme of regulated mechanisms. Therefore, investigation of flows of blood and air in anatomical conduits requires an understanding of the biology, chemistry, and physics of these systems together with the mathematical tools to describe their functioning in quantitative terms.

There is no single pathway to attain NCD targets that fits all countries, as they are at different points in their progress in the prevention and control of NCDs and at different levels of socioeconomic development.
