Book excerpt: Successful management of periodontal inflammation in patients with diabetes mellitus requires monitoring the stability of their blood glucose. Self-monitoring glucose meters have provided the diabetic patient with a direct method for rapid monitoring of their disease. However, one of the undesirable features is the need to draw blood by a finger-puncture for the test. In contrast, periodontal therapy produces ample extravasated blood from gingival inflammation during diagnostic probing of the gingival crevice and during subsequent appointments of scaling and root planing. In this study 50 patients with diabetes mellitus (ages 18 to 79) had gingival crevicular blood from periodontal probing collected in small plastic pipettes. The pipettes transferred the crevicular blood to a non-wipe glucose self-monitoring instrument. At the same time, finger-stick capillary blood measurements were analyzed in the same glucose self-monitoring instrument, and venous blood was collected for measurement in a Beckman CX3 laboratory glucose analyzer. Each Beckman measurement was corrected from a serum glucose value to a whole blood glucose value by a function of the patient's hematocrit. This corrected glucose value allowed direct comparison of the laboratory measurement to the intraoral and finger-stick whole blood measurements. Among all the systemic conditions, diabetes is the strongest risk factor for periodontitis, leading to increase prevalence, severity, and progression of the disease. Periodontal disease is considered to be the sixth complication of diabetes. The diabetic state impairs the gingival fibroblast synthesis of collagen and glycosaminoglycan, enhances crevicular fluid collagenolytic activity, results in the loss of periodontal fibres, loss of the alveolar supporting bone, loosening and finally exfoliation of the teeth. Research has demonstrated the presence of elevated levels of pro-inflammatory mediators in the gingival crevicular fluid of periodontal pockets in patients with poorly controlled diabetes, compared with people without the condition or those with well controlled diabetes, resulting in significant periodontal destruction with an equivalent bacterial challenge. Oral hygiene of the patients with diabetes must be optimised to prevent further stimulation of an already activated host response. In this book we have reviewed the relationship of diabetes mellitus and periodontitis.

Periodontal therapies have already become a part of the treatment for glycemic control; however the benefits of different periodontal therapies are unclear. In the first phase of this study, we performed a systematic review of the current randomized controlled trials for the benefits of periodontal therapies. In the second phase, we analyzed the evidence for the benefits of periodontal therapies (serum HbA1c, fructosamine, periodontal pocket depth, clinical attachment level, bleeding on probing, plaque index, gingival index, C-reaction protein, and gingival crevicular fluid volume). In the third phase, we reported safety issues (total withdrawals, withdrawal due to serious adverse effects, serious adverse events and common adverse effects). In general, most included studies were poorly reported. Additional high-quality research studies into benefits of periodontal therapies are needed.
