Celles-ci sont communément appelées circuit du médicament. Elles regroupent les étapes de : prescription, dispensation, administration. La sécurisation du circuit du médicament est indispensable afin de maîtriser le risque iatrogène. La survenue d'évènements indésirables en lien avec l'utilisation des médicaments est appelée iatrogénie médicamenteuse. En France, toutes causes confondues, près de 6,2 événements indésirables graves surviennent pour 1000 jours d'hospitalisation (9,2 en chirurgie et 4,7 en médecine), soit environ un événement indésirable grave tous les cinq jours dans un service de 30 lits. Les erreurs médicamenteuses seraient estimées à près de 60 000 à 130 000 par an, dont près de la moitié seraient évitables.La pharmacie clinique est une discipline de santé centrée sur le patient dont l'exercice a pour objectif d'optimiser la prise en charge thérapeutique, à chaque étape du parcours de soins. Pour cela, les actes de pharmacie clinique contribuent à la sécurisation, la pertinence et à l'efficience du recours aux produits de santé.Les objectifs de cette thèse sont de décrire le rôle du pharmacien dans la prévention et la gestion des erreurs médicamenteuses à travers l'exemple de 2 parcours de soins singuliers et une prise en charge d'un patient dans un contexte de soins particulier :- le parcours de soins du patient initiant un traitement anticancéreux par voie orale ;- le parcours de soins du sujet âgé :o le rôle du pharmacien hospitalier dans la prise en charge et la continuité des soins du sujet âgé lors d'une hospitalisation aiguë en service court de gériatrie ;o la formation des pharmaciens d'officine à la réalisation des bilans de médication en milieu ambulatoire et un retour d'expérience sur leur mise en place ;- l'hospitalisation d'un patient en service de réanimation.Ce travail permet la synthèse de 3 exemples liés à la complexité de la prise en charge des patients au sein des parcours de soins ou dans des contextes de soins particuliers :- La prise en charge aiguë d'un patient avec une maladie chronique dans un service de réanimation médicale ;- La prise en charge chronique d'un patient d'oncologie médicale lors de l'initiation d'un traitement anticancéreux par voie orale ;- La prise en charge hospitalière et ambulatoire d'un sujet âgé polymédiqué.Les risques liés aux médicaments sont différents selon le type de prise en charge. Ainsi, le principal risque identifié lors d'une première prescription d'un anticancéreux par voie orale est l'automédication du patient. En réanimation, le pharmacien doit être expert afin de pouvoir répondre à certaines problématiques non vues dans d'autres services conventionnels. De nouvelles missions sont confiées aux pharmaciens d'officine, notamment dans le parcours de soins du sujet âgé.Face à ces nouvelles missions du pharmacien d'officine et au vu de l'expertise demandée à un pharmacien dans des services de soins spécialisés (oncologie, réanimation par exemple), il est nécessaire de faire évoluer certains enseignements universitaires.

Book excerpt: The Republic of Therapy tells the story of the global response to the HIV epidemic from the perspective of community organizers, activists, and people living with HIV in West Africa. Drawing on his experiences as a physician and anthropologist in Burkina Faso and Côte d’Ivoire, Vinh-Kim Nguyen focuses on the period between 1994, when effective antiretroviral treatments for HIV were discovered, and 2000, when the global health community acknowledged a right to treatment, making the drugs more available. During the intervening years, when antiretrovirals were scarce in Africa, triage decisions were made determining who would receive lifesaving treatment. Nguyen explains how those decisions altered social relations in West Africa. In 1994, anxious to “break the silence” and “put a face to the epidemic,” international agencies unwittingly created a market in which stories about being HIV positive could be bartered for access to limited medical resources. Being able to talk about oneself became a matter of life or death. Tracing the cultural and political logic of triage back to colonial classification systems, Nguyen shows how it persists in contemporary attempts to design, fund, and implement mass treatment programs in the developing world. He argues that as an enactment of decisions about who may live, triage constitutes a partial, mobile form of sovereignty: what might be called therapeutic sovereignty.
