Book excerpt: Over the past decade, Indonesia has implemented significant health sector reforms that include decentralizing responsibilities for service delivery, designing incentives for health providers, increasing the supply of midwives in remote areas, and analyzing demographic and epidemiological transitions causing changes in the patterns of disease prevalence. Financial protection against catastrophic expenditures has improved substantially, and legislation has been enacted to improve the quality of physician training and patient care.Despite the progress, substantial challenges remain and include comparatively low resources for the health sector, limitations in the supply of providers at the primary and hospital levels, inefficient payment systems, shortcomings in the quality of maternal and child and adult care, lack of oversight and effective licensing in an expanding private health sector, and ineffective planning for and recruitment and retention of health workers.Given the slow pace in improving health outcomes and limited evidence linking health performance and the health workforce, the need to make more information available about past experiences to inform future policy changes is pressing. Few studies have been undertaken to measure the actual impact of the reforms and the remaining challenges. 'New Insights into the Provision of Health Services in Indonesia: A Health Workforce Study' begins the process, providing real time evidence-based inputs to facilitate the Government of Indonesia's comprehensive health sector review. The authors' analysis of panel data from households and health providers will assist the government's assessment of the impact of past health work force policies and its consideration of policy changes.

Book excerpt: This open access edited book brings together new research on the mechanisms by which maternal and reproductive health policies are formed and implemented in diverse locales around the world, from global policy spaces to sites of practice. The authors – both internationally respected anthropologists and new voices – demonstrate the value of ethnography and the utility of reproduction as a lens through which to generate rich insights into professionals’ and lay people’s intimate encounters with policy. Authors look closely at core policy debates in the history of global maternal health across six different continents, including: Women’s use of misoprostol for abortion in Burkina Faso The place of traditional birth attendants in global maternal health Donor-driven maternal health programs in Tanzania Efforts to integrate qualitative evidence in WHO maternal and child health policy-making Anthropologies of Global Maternal and Reproductive Health will engage readers interested in critical conversations about global health policy today. The broad range of foci makes it a valuable resource for teaching in medical anthropology, anthropology of reproduction, and interdisciplinary global health programs. The book will also find readership amongst critical public health scholars, health policy and systems researchers, and global public health practitioners.

Book excerpt: This contributed volume motivates and educates across fields about the major challenges in global health and the interdisciplinary strategies for solving them. Once the purview of public health, medicine, and nursing, global health is now an interdisciplinary endeavor that relies on expertise from anthropology to urban planning, economics to political science, geography to engineering. Scholars and practitioners in the health sciences are seeking knowledge from a wider array of fields while, simultaneously, students across majors have a growing interest in humanitarian issues and are pursuing knowledge and skills for impacting well-being across geographic and disciplinary borders. Using a highly practical approach and illustrative case studies, each chapter of this edited volume frames a particular problem and illustrates how interdisciplinary problem-solving can address the greatest challenges in global health today. In doing so, each chapter spurs critical and creative thinking about emergent and future problems. Topics explored among the chapters include: Transforming health and well-being for refugees and their communities Governing to deliver safe and affordable water The global crisis of antimicrobial resistance Low-tech, high-impact interventions to prevent neonatal mortality Communicating taboo health subjects Alternative housing delivery for slum upgrades Transforming Global Health: Interdisciplinary Challenges, Perspectives, and Strategies is a vital and timely compendium for any reader invested in improving global health equity. It will find an audience with researchers, practitioners, policymakers, and program implementers, as well as undergraduate and graduate students and faculty in the fields of global health, public health, and the health sciences.

Available in PDF, EPUB and Kindle. Book excerpt: In 2004 the Indonesian government made a commitment to provide its entire population with health insurance coverage through a mandatory public health insurance scheme. It has moved boldly already provides coverage to an estimated 76.4 million poor and near poor, funded through the public budget. Nevertheless, over half the population still lacks health insurance coverage, and the full fiscal impacts of the government's program for the poor have not been fully assessed or felt. In addition, significant deficiencies in the efficiency and equity of the current health system, unless addressed will exacerbate cost pressures and could preclude the effective implementation of universal coverage (Ue and the desired result of improvements in population health outcomes and financial protection. For Indonesia to achieve UC, systems' performance must be improved and key policy choices with respect to the configuration of the health financing system must be made. Indonesia's health system performs well with respect to some health outcomes and financial protection, but there is potential for significant improvement. High-level political decisions are necessary on key elements of the health financing reform package. The key transitional questions to get there include: [ the benefits that can be afforded and their impacts on health outcomes and financial protection; [ how the more than 50 percent of those currently without coverage will be insured; [ how to pay medical care providers to assure access, efficiency, and quality; [ developing a streamlined and efficient administrative structure; [ how to address the current supply constraints to assure availability of promised services; [ how to raise revenues to finance the system, including the program for the poor as well as currently uninsured groups that may require government subsidization such as the more than 60 million informal sector workers, the 85 percent of workers in firms of less than five employees, and the 70 percent of the population living in rural areas.
